Photochemiluminescent detection of antiradical activity; III: A simple assay of ascorbate in blood plasma.

A simple procedure to determine the ascorbate in blood plasma was elaborated based on a temporary inhibition of the photo-induced, O(-.)2-mediated, chemiluminescence emanating from oxidation of luminol. To remove the interferences, mainly from urate, a simple enzyme-free one-step method for plasma sample preparation by means of a gel filtration was combined with the photochemiluminescent investigation of the eluate. The results of the photochemiluminescent assay of ascorbate coincide well (r = 0.994) with a photometric method with ascorbate oxidase. The mean value of ascorbate in human blood plasma, obtained from 37 healthy volunteers, was 59.6 +/- 25.8 mumol/l. Patients with atherosclerosis having a myocardial infarction in their anamnesis displayed significantly lower values: 27.2 +/- 12.8 mumol/l (n = 32), P < 0.001.

The antioxidant system of the organism. Theoretical basis and practical consequences.

On the basis of the literature and our own investigations, the antioxidant mechanisms and substances of the human organism are described and classified as compartmentalization, detoxification, repair and utilization. The existence and the structure of an antioxidant system, which serves as support of the antioxidant homeostasis, are hypothesized. A parameter for characterizing the state of the antioxidant system and for detecting its changes under pathological conditions--the integral antioxidant capacity of blood plasma--and a method for its determination are suggested.

[Facial hemiatrophy--Romberg's disease].

Author present a care of acquired facial hemiatrophy--Romberg's disease, a sporadic disease of unclear etiology characterized by shrinking and deformation of one side of face, including superficial facial tissues, subcutaneous tissues, fatty tissue, musculature, cartilage and bones. The principle etiopahogenetic hypothesis suggests that Romberg's disease is the consequence of central and peripheral alterations in the sympathetic nervous system. The disease was first described by Romberg in 1846, and in 1963 Roger gave a detailed description and analysis of 1035 cases. In this study authors presented the latest information form literature (magnetic resonance image, biochemical investigations, experimental model of a sympathectomy at animals). A case of a patient treated with great number of available methods: computerized tomography, X-ray, Doppler spectral analyze, termovision, electromyography, electroencephalography, evoked visual potentials, brainstem's evoked potential is shown.

[The ultrasonic assessment of the cicatrix after a past cesarean section].

A supersonic study of the lower uterine segment of pregnant women which have had a Caesarean operation in the past has been made. The aim of this study is the confirmation of the supersonic scanning as a part of the struggle for diminution of the second abdominal deliveries. Twenty-six pregnant women with anamnesis of Caesarean operation were observed. The following conditions have been kept: precise probable term of delivery, gestational period--35-37 g.w., one foetus in head position, unspoiled amniotic fluid and full urinary bladder. It is obligatory a longitudinal and cross scanning in the area of the cicatrix to be made. For the assessment of the cicatrix sufficiency the author has applied supersonic criteria known in the literature. They include: shape, thickness, incessancy, outside lines and echo structure of the lower uterine segment. The results show: the scar is sufficient in 24 women--it has a triangular form in the lower uterine segment, even outside lines, the thickness of the cicatrix is above 3-5 mm it is incessant and with homogeneous echo structure. In 2 of th pregnant women there are data for inadequacy-the form is like a balloon, the cicatrix has got thinner under 3 mm, the incessancy of the lower segment is absent-it is with defects and uneven outside lines, the echo structure is heterogeneous with predominance of increased echogenicity. According to the way of delivery the results are confirmed pathohistologically, by microscope or by manual palpation. The effectiveness of the supersonic assessment of the cicatrix has been shown.

Cesarean Section

[A single-stage 2-layer suture in cesarean section--the effect of the surgical technic on postoperative febrile conditions].

The aim of the authors is to study the influence of the figure-of-eight suture of the uterine wall on the postoperative febrile complications. 1018 women, delivered with Caesarean were followed. 937 of them with conventional two layer suture of the uterine wall and the rest 81 with a figure-of-eight suture. Postoperative febrile complications were 15.58% in those with two layer suture against 3.5% in the rest 81. Using dispersion analysis the authors point that the on the type of uterine wall suture depends the rate of the postoperative febrile complications. The rate of influence of the figure-of-eight suture is up to 10%. Having in mind that this factor "suture", should be considered as 99% influential with a risk for mistake no more than 1%--F (8.06) > FT (6.64). The results presented show that the figure-of-eight suture is to be preferred to the conventional two layer suture of the uterine wall.

[Occupational damage to fertility in welders].

The aim of the present study is to establish whether there is difference in the quality of the sperm of welders in comparison with members of other professions without potential occupational risks. There were examined 30 welders and a control group of 159 males. In all participants in the study by means of the case history and clinical examination the presence of other pathogenic factors besides the occupational influences was excluded. The percentage part of the males with deviations in the sperm mobility and morphology was significantly higher in the group of welders as compared with the control group (p < 0.001). There were discussed the possible mechanisms which can be responsible for the found changes.

[The intrauterine ponderal index as a prognostic factor in fetal retardation].

The authors aim at finding the connection between the intrauterine ponderal index (IUPol) and the neonatal ponderal index (NPol) and to estimate the efficiency of IUPol in the prediction of the asymmetric growth retardation in the new born babies with NPol below 2SD for the corresponding gestational age. 142 patients were examined. The foetus weight was calculated by the formula: EFBW = 1,4 x BPD x FL - 200, and the foetus length was calculated by the formula: 6,18 x 5,9 x FL. The authors concluded that the good sensitivity and specificity of the application of IUPol in the diagnostics of the asymmetric growth retardation need wider use of this method and the IUPol can be used for the elimination of the fetal retardation, because of the high predictable level of the negative test.

[The role of routine ultrasonic screening in the antenatal diagnosis of fetal retardation].

The authors aim to evaluate the utility of ultrasound biometry in the diagnosis of IUGR. In a prospective study over 2.5 year period 142 pregnants at risk for IUGR were followed and their ultrasound results compared to those of 50 controls, investigated once after the 24 gestational week. The authors conclude that: BPD and FAC, evaluated only once, are unreliable in IUGR diagnosis; FAC is the most sensitive and specific index in onefold fetal biometry, particularly after the 34 gestational week; combing linear with non-linear parameters significantly increases.

[Motor organization in children with encopresis].

The paper reviews the degree of fecal retention in regard to organization of certain functions of central nervous system (motoric, vegetative and psychic), as well as to certain qualities of motoric organization by means of correlation analysis. Gathered results point to connection between poor tonic organization, especially of the axial muscle tonus and neuromuscular binds and the degree of fecal retention; correlations are positive, linear, so that poor motoric organization can be considered basic for the disorder of the pelvic floor muscles function and consequent encopresis.

Antioxidant effects of aqueous garlic extract. 1st communication: Direct detection using the photochemiluminescence.

The antioxidant effect of the aqueous extract from the garlic preparation Kwai was investigated using the method of photochemiluminescence. The method is based on the photo-induced, superoxide radical mediated autoxidation of luminol, and allows for the capability of substances to inhibit the free radical processes in this test system to be quantified, and hence for their antioxidant properties in respect of a standard substance (e.g. ascorbic acid, alpha-tocopherol) to be compared. The aqueous extract obtained from 1 mg of the garlic preparation was found to be anti-oxidatively as effective as 30 nmol of ascorbic acid and/or 3.6 nmol of alpha-tocopherol.

Antioxidant effects of aqueous garlic extract. 2nd communication: Inhibition of the Cu(2+)-initiated oxidation of low density lipoproteins.

The aqueous extract from the garlic preparation Kwai the antioxidant effect of which has been previously detected by the authors using the method of photochemiluminescence was tested in respect of the Cu(2+)-initiated oxidation of low density lipoprotein (LDL). The formation of conjugated diene which accompanies the lipid peroxidation process was detected photometrically. In this test system too, a dose-related oxidation-inhibiting effect of the extract was established. Further experiments will be required to find out whether such effect is due to the reduction of intrinsic antioxidants or to the increase of the integral antioxidant capacity of LDL.

[Perinatal infant mortality and cesarean section].

The authors looking for a connection between perinatal mortality (PM) and caesarean delivery (CD). A 15-year period has been analysed with 66,451 births, 943 PM cases and 3145 CD cases. The total PM and PM after CD show a tendency towards becoming equal towards the end of the investigated period. The authors consider that the margin of 10/1000 can not be influenced by the rate of the operation growing higher. PM after CD is mainly influenced by biological factors, doctor's qualifications, the timely stated reasons for the operation and the postoperative treatment and care for the baby. A conclusion has been drawn for the multiple factor caused of PM after CD.

[Doppler velocimetry of the fetal and the uterine-placental blood flows in pre-eclampsia and fetal retardation].

The aim of the authors is to point out the value of the Doppler velocimetry in the prediction of perinatal distress and the place of Doppler investigation in the management of high risk fetus. In 17 pre-eclamptic and 20 normotensive pregnant women, the blood flow velocity in the uterine and umbilical artery was investigated. It was proved that the changes in the blood flow velocity are feature of some placental disturbances that lead to fetal retardation. The place of that method is controversial for active obstetric management.

[Further comment on the problem of first-time delivery at an age over 30].

The authors concentrate on operative delivery--caesarean section, vacuum extraction and forceps of primiparous women over 30 years of age. Patients are divided in three groups by age--up to 29 years, 30-34 years and over 35. The data from 1992 is compared to already known results concerning the period 1983-1989. A rise in the percentage of caesarean section in the third group--over 35 up to 47.60% is found. The authors point that this is a result of usually unnecessary assurance in connection with age. There is no change in the operative delivery rate in the other two groups. The authors propose further discussion on the problem.
