[The mechanism of Na+--K+ pump dysfunction in the erythrocytes of patients with glomerulonephritis and kidney failure].

The mechanisms of Na, K-ATPase dysfunction in red blood cells of patients suffering from glomerulonephritis and renal failure were studied by means of "functional loads" of an enzyme, enabling not only the activity but also the routine of Na, K-ATPase to be determined. Evidence is provided for the presence of immunotoxic factor on the patients' red blood cells, in the plasma of venous blood and serum of arterial blood. Its excretion from the patients' body and recovery of the routine of Na, K-ATPase have been simulated in vivo and in vitro. The cold immunoglobulin nature of the inhibitor related to the M-chains of IgM was established. It has been shown that the mechanisms of formation of the immunopathogenesis of glomerulonephritis are associated with the appearance of cold autoantibodies and their fragments on blood cell membranes, bringing about a decrease of the activity of Na, K-ATPase and modifying its routine.

[Evaluation of the indicators of dynamic renal scintigraphy using the radiopharmaceutical preparation In-113m-DTPA in patients with chronic glomerulonephritis before and after heparin therapy].

In 70 patients with a morphologically verified diagnosis of chronic glomerulonephritis, functional computer-aided scintigraphy of the kidneys was performed before and after heparin therapy. Improvement in the scintigraphic parameters following heparin therapy was apparent as the reduction of the time of the radiopharmaceutical passage and the decrease of spastic excretory disturbances. A certain deterioration of the scintigraphic parameters after heparin therapy was found in patients with fibroplastic and focal sclerotic glomerulonephritis associated with renal insufficiency.

[Tubulointerstitial nephropathies].

The term tubulointerstitial nephropathy (TIN) means a renal disease, during which the tubules and interstice form a substrate of the primary injury or a substrate whose involvement is dominating in the disease clinical picture. The incidence of TIN is fairly high, with different etiology and pathogenesis. Drug, virus, bacterial and immune patterns are among the most common types of acute TIN. It is shown that the incidence of chronic drug TIN ranges from 0.2% among urban population to 0.6% among rural population. The incidence and gravity of renal injuries in hyperuricemia depend on the blood uric acid content. Study of the incidence and character of TIN in hypercalcemia in patients with sarcoidosis, hyperparathyrosis and multiple myeloma demonstrates the character of tubulointerstitial lesions to depend on the structure of paraprotein. In systemic lupus erythematosus, tubulointerstitial lesions are detected in 50% of cases. Such a comprehensive treatment of TIN opens up new vistas in the prophylaxis and therapy of this illness.

[Fine-needle aspiration and puncture biopsies in the diagnosis of lesions of kidney transplants].

270 fine needle aspiration and 41 puncture biopsies were fulfilled after 92 cadaveric renal transplantations to diagnose the status of the grafts. With the clinical and ultrasonographic data, the first method proved to be effective in diagnosing acute graft rejection in 79.9% of cases and in determining the status of graft reversibility or irreversibility, but the fine needle aspiration biopsy failed to differentiate the cellular or vascular types of the rejection. The method was also effective in diagnosing the tubular necrosis, acute cyclosporin nephrotoxicity, infectious transplant complications. The paracentetic biopsy essentially supplements the fine needle one and allows one to diagnose chronic cyclosporin nephrotoxicity and chronic transplant rejection. It appears to be the only method revealing the magnitude of graft rejection during oligoanuria caused by ischemic graft lesion. It was concluded that the results of fine needle aspiration biopsy were the most informative within the first postoperative month because later on it becomes difficult to obtain biopsy specimens adequate to interpret the results correctly.
