[Fulminating hepatitis A in children. Apropos of 4 cases].

We report four cases of fulminant hepatitis in children (4 to 15 years) who developed an hepatic encephalopathy grade III to IV, 4 to 13 days after the onset of their illness. Three patients recovered without sequelae. The complications were neurological: one child showed elevation of the intracranial pressure, successfully treated after monitoring of extra-dural pressure; one suffered from cerebral death. Hepatitis A was diagnosed by the presence in serum of the IgM component of hepatitis A antibody, but another etiologic factor was present in two cases: an halothane anesthesia and an Epstein Barr virus infection which could explain the severity of the hepatitis.

[Adult-type respiratory distress syndrome in pediatrics. Apropos of 20 cases].

Twenty cases of adult-type respiratory distress syndrome (ARDS) observed in children aged less than 15 years (excluding neonates) are reported; 55% survived and 45% died. A comparative study of both groups showed that prognosis is not related to the child's condition at the time of admission. On the contrary, evolution during the first 72 hours seems to be of prime importance. Treatment included artificial ventilation with PEEP, the optimal level of which was defined by empirical means; maximal PEEP reached 13.2 +/- 4.7 cmH2O and 20.3 +/- 4 cmH2O in the surviving and the deceased groups, respectively. Maintenance to a steady level was reached by using cuffed endotracheal tubes. No correlation was found between maximal PEEP levels and pneumothorax frequency, the latter being identical in both groups. Cardiac catheterization, performed in 5 cases, was helpful in defining adequate additional therapy which improved hematosis and, when done early, appeared to favorably influence the prognosis.

[Bacterial meningitis with initially clear CSF].

16 bacterial meningitis on 150 observations of purulent meningitis have clear or normal CSM at the initial lumbar puncture. Neisseria meningitidis is the principal germ and there is often purpura and moderate shock. Bacteremia is present in three of the six observations with normal CSF. CSF in controlled a second time 9 to 48 h after the first control. In 14 cases CSF is purulent. Treatment is delayed in eight cases.

[Hypoglycemia and hyperlactacidemia in relation with a new case of fructose-1,6-diphosphatase deficiency. Determination of enzymatic activity in leukocytes].

A 11.5 month-old girl with recurrent episodes of hypoglycemia and lactic acidosis was identified as having fructose-1,6-diphosphatase deficiency. The diagnosis may be realised with a simple way by an oral fructose tolerance test and the activity dosage of this enzyme in white blood cells. The fructose and sucrose-free diet and avoidance of prolonged fasting resulted in a decrease of hepatomegaly and normal values of lactate between the episodes.

[Hyponatremia and convulsions in the postoperative period in children].

Four children undergoing surgery presented with seizures and hyponatremia 12 hours after anesthesia. One child died with cerebral oedema, the others recovered. The respective roles of hypo-osmolar perfusions and ADH secretion are discussed. Ionogram before surgery, supervision of weight and diuresis and correct fluid management should prevent these postoperative complications.
