Extrastriate area V5 (MT) and its role in the processing of visual motion.

Extrastriate area V5 was identified in many primates including humans. V5 represents an important substrate for visual motion processing. Neurons in this area show a sharp preference for direction and speed of visual stimuli. Neuronal activity correlates with the ability to detect and discriminate motion in several psychophysical paradigms. The nature of neuronal representation of motion in V5 is much debated currently. The following article provides a summary of basic anatomical, neurophysiological, neuropsychological, and behavioural data related to the role of area V5 in the processing of visual motion information.

[Treatment of pseudoaneurysms with injection of thrombin].

OBJECTIVES: To introduce and assess the efficacy and risks associated with a new treatment method of post-catheterization pseudoaneurysms. METHODS: Thrombin injection was indicated in patients with failure of compression therapy during 24 hours. Patients with a cavity volume < 1 cm3, with a pseudoaneurysm causing compression or haemorrhagic shock were excluded. A needle is introduced under local anaesthesia with ultrasound guidance, its tip is placed thoroughly in the centre of the cavity. Subsequently, under colour-flow mapping, thrombin solution with concentration of 1000 IU/ml is cautiously injected, which results in instantaneous thrombosis of the pseudoaneurysm cavity. RESULTS: Thrombin injection was used in 17 patients during a period of 12 months. 12 of them were women, 6 patients with valvular disease, 2 after intervention and 5 anticoagulated. The method was successful in 16 patients. In 5 of them, a small residual cavity persisted, which was not suitable for re-injection, and was closed completely after a few minutes of compression by a ultrasound transducer. There were no thrombotic complications. CONCLUSION: Although compression remains the method of choice, thrombin injection is a highly effective, quick and, with necessary precautions, safe alternative to surgical treatment of pseudoaneurysms, mainly in anticoagulated patients.

[The importance of electrocardiographic changes at rest during exercise in deciding to perform immediate ("ad hoc") percutaneous transluminal coronary angioplasty].

In 100 patients (75 men and 25 women, mean age 53 years) who had "ad hoc" percutaneous transluminal coronary angioplasty (PTCA) on one or two vessels retrospectively the ECG records were evaluated (signs of necrosis or ischaemia) at rest and/or after exercise and compared with the finding of major obstruction (complete occlusion, stenosis > 50%) on coronarography. In obstruction of the ramus interventricularis anterior the positive ECG finding was consistent with the area of the impaired blood supply in 87%, in case of obstruction of the right coronary artery in 79%, in obstruction of the ramus circumflexus in 90%. In concurrent obstruction of two coronaries despite a 100% positive ECG no close topical correlation was found between coronarography and ECG. On the whole no reliable consistency was found between ECG and kinetic disorders detected by echocardiography. The results indicate that topical analysis of ECG ischaemic findings in patients with coronary arterial disease before planned coronarography contributes to the decision to make "ad hoc" PTCA in affections of one artery. In affections of two arteries ECG is of no particular value.

[Founding of the Society of Czechoslovak Physicians in Bratislava in 1920--contribution of Czech physicians in the mutual development and cooperation in Czech and Slovak medicine].

Foundation of the Society of the Slovak Medical Doctors (SSMD) in Bratislava in June 13, 1920 (till 1938 Society Czechoslovak Medical Doctors) belongs to the most important milestones in the modern history of the Slovak medicine and it can be directly related to the foundation of Comenius University and its first faculty--Faculty of Medicine. SSMD started in October 1921 publishing the Bratislava Medical Journal, which represented the first scientific medical journal in Slovakia after the foundation of the Czechoslovak Republic. Eighty years of existence of SSMD, which belongs with its almost 2500 members to the largest professional society within the Slovak Medical Society, testifies its contribution to the development and growth of the Slovak medicine. The Society had been for many years the only professional forum where the Slovak medical doctors could meet and where the scientists of the Medical Faculty in Bratislava professionally matured. It represents also an important evidence of the material and spiritual assistance and collaboration of the Czech and Slovak medical doctors during the First Czechoslovak Republic.

Echocardiography and exercise electrocardiography in the assessment of long-term outcome after percutaneous mitral valvuloplasty.

Before and immediate (invasive examination) and long-term follow-up (4.5 year; echocardiography, exercise electrocardiography) studies after percutaneous transvenous mitral valvuloplasty (PTMV; Inoue technique) were prospectively performed in 123 consecutive patients with pure or dominant mitral stenosis. These patients consisted of two groups: Group A with suitable valve morphology (n = 108, mean age 49.6 years, echocardiographic score 8.2 +/- 1.5), and Group B with less favourable valve morphology (n = 15, mean age 59.3 years, echocardiographic score 11.3 +/- 2.3) rejected for surgery because of high surgical risk. PTMV is a safe and effective nonsurgical method of treatment for symptomatic mitral stenosis and has long-lasting results in patients with pliable mitral valve as well as in patients with less suitable valve morphology. Echocardiography (Doppler approach) is the most efficient noninvasive method for assessment of PTMV effect. Exercise electrocardiography test is a valuable complementary method for objective measurement of patient working capacity and to confirm the success of PTMV.

[Pulmonary complications after heart surgery].

The authors present a detailed analysis of pulmonary complications after open-heart surgery and of their risk factors. The results of the presented study indicate that COPD patients with functional impairments are more sensitive to complicated postoperative outcome and to respiratory infections, in general. There is a clear relationship between artificial violation of the pleural cavity, diaphragmatic dysfunction and pleural effusion. Diaphragmatic dysfunction appears more frequently in mammarocoronary bypass patients in consequence of pleurotomy. The incidence of dysfunction could be reduced by the use of phrenic nerve insulation. Patients with pulmonary complications suffer from serious pulmonary function deficit. The possibilities of identification of the patients at risk, as well as the prevention and prophylaxis are analysed. (Tab. 7, Fig. 2, Ref. 71.)

[ACE-inhibitors in the treatment of chronic heart failure: pathophysiologic principles of protective effects].

The aim of modern therapy of heart failure is not a pure removal of symptoms but an improvement of survival. The angiotensin-converting enzyme (ACE)-inhibitors reduced morbidity and mortality in several large clinical trials in patients with dysfunctional left ventricle or manifestant heart failure. Thus, ACE inhibitors are recommended for heart failure treatment as a drug of primary option, unless contraindications are present, and if tolerated by patient. The mechanism of action of ACE inhibitors in heart failure is hypothetical. Participation of three factors is supposed: improvement in the pumping function of the failing heart, reduction on the risk of sudden death and reduction in myocardial infarction incidence. Reduction of hemodynamic load, antiischemic action and reduction of fibrotic tissue proliferation in failing myocardium are responsible for heart function improvement. These mechanisms together with potential antiatherosclerotic, antiaggregative, fibrinolytic and protective effect on endothelial function are supposed to participate in reduction of acute myocardial infarction and sudden death origin. The mentioned effects are determined by interaction with both circulating and local renin-angiotensin systems. The negative hemodynamic effects and undesirable restructuralisation of the affected ventricle are thus influenced on systematic and local-tissue level. Similarly as any other therapy also the treatment of heart failure with ACE inhibitors needs experience and a rational well tailored individual approach. (Fig.1, Ref. 32.).

[Personal experience with determination of endogenous, digoxin-like substances in patients with myocardial infarct and other cardiopathies].

A group of 65 patients with acute infarction of myocardium (IM) who were not treated with digitalis preparations were subdued to examination to the presence of digitalis-like substances in their urine by means of radioimmuno-analytic method with use of anti-digoxin antibodies. The control group was constituted of 69 healthy subjects. Patients afflicted with IM had significantly increased concentrations of DLS in serum in comparison with health subjects. No significant relations of DLS to the activity of creatinkinase, IM localisation, occurrence of dysrhythmias, heart insufficiency and IM mortality were discovered. An increase in DLS in the blood of patients with acute IM probably coincides with a decreased cardiac output, with the activation of the stress axis and retention of sodium and fluids. The second examined group of patients was constituted of 20 subjects with other severe cardiopathies (inborn and acquired heart defects, chronic ischemic heart disease, inflammatory and degenerative diseases of the heart, and hypertension), who were subdued to catheter examinations. The authors discovered no significant differences of DLS concentrations in the blood during catheterization of individual compartments of inferior vena cava, superior vena cava, and the right ventricle. They were not successful in defining the particular site of DLS secretion on the basis of this examination. The authors pay attention to interaction of DLS during the radioimmuno-analytic examination of the digoxin serum concentration.

Dipolar electrocardiotopographic evaluation of ventricular activation in patients with various degrees of coronary artery disease.

Dipolar electrocardiotopographic imaging of instantaneous vectorcardiographic QRS vectors represents the ventricular activation front as a bounded area on the spherical image surface. Fuzzy set treatment of the elements of this area allows for the recognition of five classes of ventricular activation: I, abnormal; II, abnormal with normal component; III, normal with abnormal component; IV, marginally normal; and V, normal. In a group of 71 patients with chest pain and various degrees of coronary artery stenosis classes II, III, and IV were most frequently observed in patients with one- or two-vessel disease. The frequency distributions of classes II and III throughout the QRS were bimodal with peaks at 20 and 60 ms after QRS onset, while the frequency distribution of class IV was unimodal with a peak at 30 ms after QRS onset. The advantage of this method is the smoothing of threshold values on a quantitative basis.

[Blood levels of Fe, Zn, Br and Rb determined by radionuclide x-ray fluorescence analysis in patients with dilated cardiomyopathy].

In 68 patients (50 men between the age of 18-59, 18 women between the age of 20-53) inflicted by dilatation cardiomyopathy in the stage of severe decompensation (functional NYHA group III-IV, the concentrations of Fe, Zn, Br, and Rb in whole blood were determined by means of radionuclide X-ray-fluorescence analysis. The Fe concentration (X = 401.26 +/- 14.58) was significantly lower (alpha = 0.074), the Zn concentration (X = 8.20 +/- 0.95) was not significantly altered, the trace elements concentration Br (X = 32.30 +/- 4.61) and Rb (X = 4.58 +/- 0.38) were remarkably and significantly higher (alpha = 2.10(-16), resp. alpha < or = 1.10(-16)) when compared with concentrations of these elements in healthy controls. The method of radionuclide X-ray-fluorescence analysis is suitable for element analysis of blood. Its advantages reside in its rapidness, polycomponence, sample nondestructiveness and low requirement of material. (Tab. 3, Fig. 4, Ref. 17.)

[Revascularization procedures on coronary vessels. Development of techniques, indications and results].

Hitherto assembled experience indicates that the principles and immediate results of surgical and reconstructive intervention surgery of coronary arteries are very close and similar. Criteria of indication and technical possibilities in both these areas are developing and improving steadily. Accurate definition of the optimal use of these techniques does not exist so far. At present several perspective investigations are under way and they could provide within the next few months an answer and define the areas of these two basic reconstruction operations of the coronary arteries. Regardless of that the two methods are nowadays already a routine procedure in the care of patients with coronary disease.

[The first selective coronarographies in the former Czecho-Slovakia].

The study observes the history and the beginning of selective coronarographies performed in the territory of the Slovak Republic. The first aortocoronary bypass in Slovakia was performed in Bratislava. It was realized by the team of the Surgical Clinic of Academician Dérer in november 1969, namely on the basis of coronarographic examinations which were performed by Smrecanský, Cagán and Krajcovic. Coronographies have been performed in the Central X-ray Department of the Hospital and Clinic of Academician Dérer since the end of the year 1968. During the period of december 1968 to june 1970 selective coronographies were realized in 70 patients and they represented the first performances of this method in former Czechoslovakia. (Ref. 8.)

[Non-invasive assessment of the efficacy of long-term vasodilator therapy of cardiac decompensation in patients with dilated cardiomyopathy].

In 18 patients (14 men, 4 women, in the age of 37-52 years, mean = 43 years) with dilated cardiomyopathy allocated into NYHA functional classification groups III-IV, the efficacy of ambulatory long term (mean = 6 years) vasodilator therapy by means of non-invasive methods was evaluated. NITRO-MACK retard (venodilator) in the dosage of 20 mg per day and DIHYDRALAZIN (arteriodilator) in the dosage of 100-200 mg per day were combined simultaneously with continuous administration of cardiotonics and diuretics. Vasodilator therapy had a favourable effect on systolic and diastolic function of the left ventricle with the mostly marked effect at the beginning of treatment. Amidst non-invasive parameters, the diastolic amplitude time index and left ventricular hypertrophy-dilatation index had the greatest prognostic value regarding the treatment efficacy and survival of patients. This objective evidence of the efficacy of vasodilator therapy in patients with dilated cardiomyopathy coincided with the improvement of clinical status, subjective relief, moderation of decompensation, and thus with the improvement of life quality. [Lungs and their function in heart disease].

Pulmonary manifestations of cardiac diseases are reviewed and the contribution of pulmonary function assessment in diagnosis, management and prognosis of these diseases is being emphasized. Inborn and acquired diseases of the heart exert pathological effects on pulmonary hemodynamics and later on also on lung structure, clinically manifested by pulmonary symptomatology and abnormalities in pulmonary function. The basic mechanisms of pulmonary function derangement include pulmonary congestion and hyperreactivity of the airways as a consequence of increased blood pressure or flow in the pulmovascular network (postcapillary or hyperkinetic pulmonary hypertension). The severity of the given cardiac disease determines also the condition of pulmonary function. Thus examinations of pulmonary function substantially contribute to the classification of the functional state and to optimal choices in therapeutic strategy. Appropriate surgical or medical therapeutic procedures can improve the hemodynamic conditions in the pulmonary vascular network. This, in turn, leads to partial regression of morphological changes in the pulmonary parenchyma and thus to gradual improvement of pulmonary function. (Tab. 4, Fig. 4, Ref. 62.)

Alterations of lipid metabolism in men with hypotestosteronemia.

Studied relationship in serum concentration of testosterone and lipids in 35 men with a positive coronarographic finding and 30 sterile men, in comparison with a control group. From lipid parameters we determined serum concentration of cholesterol, triacylglycerols and apolipoprotein B, and lipid distribution in lipoprotein fractions. Serum testosterone concentration was found decreased in both the group as against the controls. As to lipid parameters, cholesterol concentration was depressed in the high-density lipoprotein (HDL) fraction of both the clinical groups. As increased apolipoprotein B level was seen in men with the positive coronarographic finding. The results point to a negative effect of lowered testosterone concentrations on lipid metabolism.

[Indications for cardiac surgery on the basis of noninvasive examination methods with major emphasis on echocardiography].

Over the years 1983-1988 cardiosurgical procedures with extracorporeal circulation without catheterization of the heart were indicated in 191 patients, representing 19.8% of all the patients operated on at extracorporeal circulation (n--962). Indications for surgery were made on the basis of the clinical state of the patient and of the results of noninvasive methods, with main emphasis on echocardiographic findings. The close relationship between the methodological level of ultrasound diagnosis and the increasing number of patients operated on without preceding hemodynamic examination is documented (13% in 1983, 35% in 1988). With a few exceptions, noninvasive techniques provide a sufficient amount of relevant information definitely deciding on the indication for operation without catheterization in heart tumors and infective endocarditis, in about one half of the patients with dissecting aneurysm of the aorta and with deranged function of valvular prostheses, in one third of the patients with mitral defects, and in about one third of the patients with atrial septal defects. On the basis of their own results, the authors present the criteria for indicating cardiosurgical procedures without catheterization of the heart in the diagnoses studied. The diagnostic conclusion was confirmed by the operative finding in 99.5%. The obtained results are comparable with those reported by authors from abroad and demonstrate the significant medical and economic contribution.
