[Lumbrical plus syndrome].

A rare case of paradoxical extension of the medius finger - the so-called lumbrical plus finger syndrome-is described. At operation was found a fibrous adherence of the third lumbrical at the sheath of the flexor digitorum profundus. Some of pathological mechanisms are discussed. A satisfactory result was obtained after the resection of the tendon of the second lumbrical.

[Group tuberculous contamination in preschoolers].

The study of a new collective tuberculosis contamination is proper for us to report news about the antituberculosis fight. In 1997, an ill teacher with cavity produced 19 cases of tuberculin turnings in a class with 25 children, but none of them had tuberculosis. We tried to find an explanation. We studied another 3 epidemiological events. In 1970, a teacher from Holbav produced 80 tuberculin turnings with 42 ill children. In Săcele another teacher with cavity produced 39 tuberculin turnings with 5 ill children. In Prejmer, in 1988 a sick child with cavity made himself 28 tuberculin turnings with a single case of illness. We tried to explain these differences: similar sources (patients with cavities) and the same conditions of illness but different effects. Our epidemiological investigations showed that in Holbav only 3.6% of children had BCG vaccine; in Săcele were 18%; in Prejmer were 98%; in Codlea 96%. BCG vaccination prevents tuberculosis. But in this case is another problem, did all the tuberculin turnings signify a virulent infection? After a year when we tested again the children, we found that for 58% of tuberculin turnings the reaction diminished considerably. We consider that, this is the particularity of the evolution of the postvaccinal allergy. We conclude that hyperreactivity may be a transitory exacerbation of the postvaccinal allergy and not an evidence of the infection. BCG vaccination reduces the risk of tuberculosis even when the infection is important and virulent.

[HIV infection case finding in children hospitalized in a pediatric pneumophthisiology department].

The study deals with HIV infection in children admitted to the "Phthisiopediatrics" Department of Braşov Hospital. They represent about 1% of the hospitalized cases (5 of 564). Considering, however, the age group of interest (0-5 years), they constitute 4.6% of hospitalized cases. Four of the five children were born in 1989, and all of them show a rather rich pathological history, with many hospitalizations in pediatrics departments, treatments by injections and even transfusions (2 cases). Vertical transmission being excluded--the respective mothers were negative at testing--the iatrogenic infection seems most probable. Together with the study on AgHBs carriers, where the results were similar, the present study made us think over the elementary prophylaxis measures in current medical practice.

[The clinical, radiological and therapeutic aspects of endotracheobronchial foreign bodies in children].

During about 30 years of activity in the Bronchology room of the Phthisiopediatrics Unit-Braşov, 87 children having inhaled a foreign body (FB) in their airways were assisted. A number of 85 of them were solved through endoscopic interventions, performed by simple or complex (with ORL and intensive care) teams. The 85 FB were mostly (55) of vegetable nature (grains of beam and maize, seeds of pumpkin and sun flower), fragments of bone (10), of metal (10), and of plastic (8). FB localisation was: tracheal (30), in the right side bronchi (31), and left ones (26). Infancy was quite often interested: 60 children under 3 years of age and 9 aged less than 1 year. Clinical-radiological manifestation of FB were grouped in: without clearcut syndrome, well tolerated; with a clinical picture of acute pneumonia; with syndrome of complete or incomplete ("with valve") obstruction. Early diagnosis and prompt endoscopy solving prevent immediate or late complications.

[Combination of radiophotographic and tuberculin detection in groups of young people].

Tuberculin pre-testing between 1971 and 1973 in profesional schools, as well as in high schools from Brasov, showed that by the radiophotographic examination of tuberculin reactors only, all the new cases of tuberculosis were detected, as well as those requiring surveillance and chemoprophylaxis. In this way over one half of the adolescents from professional schools and one quarter of those from high schools could be excepted from radiophotography, with the corresponding effects of radiation prevention and economic efficiency.

Chromosome abberations in rat embryo cells transformed after infection with ultraviolet-irradiated herpes simplex virus.

Two rat embryo cell lines (TR1 and TR2) transformed after infection with ultaviolet-inactivated herpes simplex virus type 2 showed modal chromosome numbers in the tetraploid range. In the TR1 cell line a near-diploid cell population replaced during subcultivation the hypotetraploid cells thus explaining the loss of immunofluorescent viral antigens. Both transformed cell lines showed frequent achromatic gaps. In the TR1 cell line such gaps were preferentially located in specific chromosome regions. The occurrence of achromatic gaps is presumably associated with the activity of a viral gene.

[Detection of tubercular infection after BCG vaccination].

The author has followed several groups of children over a period of many years and evidenced the general decrease of the level of tuberculin reactivity. The implications are discussed, of the B.C.G. vaccination on the tuberculous allergy and the possibility to differentiate post-vaccinal allergy with the aid of skin testing, using 2 and 10 PPD units. The importance is stressed, of the vaccinal scar as an indication of a good immune reactivity, and thus of protection against contamination with virulent tuberculous baccili. The time is foreseen, when systematic vaccination with B.C.G. will reduce to such a significant degree the number of cases of tuberculosis in children that differential diagnosis on the basis of tuberculin allergy will not be necessary.

[Intra-articular teratoma of the knee].

An unique case is presented, never mentioned before the world specialized literature, concerning an intraarticular teratoid formation in the knee. Initially a sessile, pediculated formation 3/3 cm in size, on the interior aspect of the synovial sac, became free following an accident, and blocked the knee in the flexed position at an angle of 30 degrees, simulating rupture of the medial meniscus and intercondylar luxation.
