Ashley's hypothesis revisited.

Nowadays it is widely accepted that cervical cancer is a virus related, sexually transmitted disease, but no complete agreement exists about its etiological factors. They may be identified in HSV or in HPV or in both. Moreover, its is accepted that cervical cancer does not arises ex abrupto from normal epithelium, but often arises from the epithelia with well codified morphological lesions now defined as "Cytohistological Cervical Cancer Precursors". Epidemiological data available at the moment, also concerning risk age, suggest that two different risk classes do exist. They range respectively between 20 and 40 years and between 50 and 70 years of age. When considering sexual promiscuity, younger patients refer a higher and more intensive level than that referred by older; the latter often claim for a low level of sexual activity. Starting from this observation, as well as from clinical and histological evidence, Ashley first suggested in 1965 the existence of two different types of cervical cancer. In the Ashley hypothesis the first type, slow-growing, is often preceded by a precursor, non-invasive stage, while the second, fast-growing, rapidly invades, often without evidence of a previnvasive stage. In our series of 1,010 invasive and 210 in situ cervical cancers, indeed, two different ranges of risk-ages exist, fulfilling Ashley's hypothesis. Morphological evaluation of lesions shows that in the younger patients, invasive cancer is, in a high percentage, associated with HPV-VCE and CIN in the surroundings, while in the older, this feature is less evident.(ABSTRACT TRUNCATED AT 250 WORDS)

Concerning a new pathogenetic hypothesis of the E.P.H. gestosis (note I).

The Authors, after having evaluated the literature data, report, on the basis of their clinical experience, a new pathogenic hypothesis of the E.P.H. gestosis which would be: "Disease due to missed materno-foetal adaptation:, correlated to the activation of the maternal immunocompetent system against the "not self" antigens, carried by the trophoblast and codified by the genoma of the father's origin. It is also due to missed destruction of the tunica media of the placental bed arteries and to the thromboplastinic activity of the trophoblastic cells damaged by the immunologic reaction. This disease, clinically represents, the appearance of a "temporaneous" (pre-eclampsia) or "definitive" (eclampsia) defeat of the fibrinolytic control mechanisms against a disseminated intravascular coagulopathy.

Histopathology of spontaneous regression in virus-induced murine leukemia.

The histopathology of the spontaneous regression of murine leukemia induced by a particular strain of Friend leukemia virus was studied in Swiss ICR/Ha mice. Animals inoculated with the regressing strain of Friend virus exhibited an initial pathologic response identical to that induced by conventional strains of Friend virus. Unlike the fatal leukemia produced by conventional Friend virus, the pathology of the disease induced by the regressing strain of Friend virus appeared to be self-limiting. The histopathology of the two diseases is compared in this report.

[Realities and uncertainties in normal and pathological physiology of the lymphatic system of the extremities].

In the special literature, values of pressures in the lymphatics trunks vary within very large limits, frequently in a contradictory manner, a fact that may generate confusion in the case of interpretation of pathological data. Following an analysis of numerous lymphographic aspects an explanation can be provided, for the radiological modifications of lymph vessels in various pathological circumstances, starting from the most plausible data concerning the lymph circulation in the limbs.

Successful surgical treatment of polycystic liver disease.

A patient with multiple liver cysts, erroneously diagnosed as suffering from echinococcal infection, was treated with mebendazole for four months. When fever appeared, she was treated with antibiotics, but her condition further deteriorated. On laparotomy polycystic liver disease was diagnosed and four liters of pus (due to peptostreptococcal infection) was evacuated from one of the cysts, which was also deroofed. The other cysts were excised and the patient recovered. These unusual events are discussed in the light of the pertinent literature.

[Influence of alpha and beta blocking agents on recovery of intestinal transit and postoperative complications in aged patients].

Following the recovery of intestinal peristaltic movements in medium and advanced age patients in whom major surgery had been performed, after an adequate preparation with and blocking agents, as compared with the control group, a shortening was noted, of the intestinal paresis period in both age groups. The early recovery of intestinal transit had positive effects on the postoperative evolution of the patients, especially in the aged ones, and allowed for an early resumption of oral alimentation, reducing abdominal meteorization, as well as the number and the severity of local and general complications.

[Pathogenesis of primary lymphedemas].

The role of the involvement of regional limph-node relays is already known, in the genesis of secondary lymphatic oedemas. With regard to the primary lymphatic oedemas however the role of regional lymph-nodes is considered to be limited. Following a complex investigation of an important clinical material the authors conclude that, in cases of primary lymphatic oedemas the determinat lesion is in the regional lymph nodes, and that it consists in an alteration of the filtration capacity, as well as in a reduction of the processing capabilities by the lymph nodes of the lymph flowing from the afferent lymph vessels. Simultaneously with the functional alterations lymphography revealed a series of changes, and macro- and microscopic examination also showed morphological pathologic alterations. The clinicat evolution and the prognosis of the oedema in the limb involved will depend on the extension and the evolution of these lesions.
