Nutritional status of children of urban leprosy patients staying at preventoria based on biochemical parameters.

Measurements of nutritionally relevant biochemical and endocrine variables were made on 60 apparently healthy children (group A) whose parents suffered from leprosy and who had been separated at the age of 4 years and brought up in preventoria. Most of the measurements were also made on a comparison group of healthy children from the same poor socio-economic class (group B). In both groups the serum concentrations of cholesterol and triglycerides were well below those found in Western populations. Almost all the children in both groups were anaemic, but serum iron and ferritin levels were satisfactory. Folate and vitamin B12 levels were measured in group A only and were low in a significant proportion. Deficiency of these water-soluble vitamins may be a cause of the anaemia. Low albumin levels were found in 40% of group A children, compared with 2% in group B. The concentrations of calcium and magnesium were lower and that of phosphate higher in group A than in B. In both groups one-third of the children had low levels of serum zinc. Fifteen per cent of group A children had biochemical evidence of vitamin A deficiency, but none were deficient in vitamin E. Levels of total T3 and total T4 were below the lower limit of normal in a substantial proportion of children in both groups. Concentrations of parathyroid hormone were increased in parallel with the low values for serum calcium. Radiological studies of ossification centres in 57 group A children showed delayed maturation in 11 cases. The relevance of these findings to previous studies of the children of lepers in India is discussed.

Allergic bronchopulmonary aspergillosis: a study of 46 cases with special reference to laboratory aspects.

A study of 46 cases of allergic bronchopulmonary aspergillosis has been presented with special reference to laboratory aspects. Episodic airways obstruction and dual skin reaction to aspergillin were present in all the cases. Furthermore, there were pulmonary infiltrations in 91.1%, blood eosinophilia in 80.4%, precipitins in 91.3%, and sputum cultures repeatedly positive for A. fumigatus in 82.6% of the patients. It was observed that the multiplicity of precipitin bands corresponded to the higher recovery of A. fumigatus in culture. A. flavus and A. niger were recovered from 58.7 and 80.4% cases of allergic bronchopulmonary aspergillosis respectively but dual skin reactions to these species occurred only in 47.6 and 26.2% and precipitins in 21.7 and 4.3% of cases, respectively. The findings are compatible with the poor allergenic and antigenic properties attributed to these two species in comparison with A. fumigatus. It is suggested that allergic bronchopulmonary aspergillosis may not be too uncommon in India and it deserves greater attention in the differential diagnosis of chest diseases.

Amoebic liver abscess rupturing into the chest: ultrasonographic appearance.

Two cases of amoebic liver abscess perforating into chest are reported. Both patients were males in their twenties. They presented with right upper quadrant pain in abdomen in one case and right lower chest pain in second case. Ultrasonography demonstrated hypoechoic liver abscess in superior subdiaphragmatic part of the right lobe of liver, with communication through a hole in the diaphragm into the thoracic cavity in both the cases. Both these cases were diagnosed as cases of amoebic liver abscess perforated into chest on the basis of ultrasonographic findings and both of them showed good response to specific antiamoebic therapy.

Ultrasonography in pericardial effusion.

Four cases of pericardial effusion diagnosed by echocardiography are reported. While one of the four cases was referred for echocardiography with a diagnosis of pericardial effusion, the other three patients were selected for echocardiography on the basis of cardiomegaly demonstrated in plain x-ray chest examination. Real time echocardiography was considered to be most rapid, safe, convenient, accurate and least invasive method of diagnosing pericardial effusion.
