Astigmatism in Bangladeshi and white school entrants in East London: a prospective comparative study.

A prospective comparative study was undertaken to determine the prevalence of significant astigmatism in school entrants living in the same area of East London. Thirty-one Bangladeshi and thirty-one white school entrants were refracted in two local state schools. In the Bangladeshi group, 7 right and 5 left eyes had more than 1 dioptre of astigmatism. Most of this astigmatism was with the rule. In the white group, 1 left eye had more than 1 dioptre of astigmatism. This study has shown a statistically significant incidence of astigmatism in an ethnic group in East London. This difference between two racial groups living in the same area of East London has not previously been documented. In our study, Bangladeshi school entrants may be at greater risk of amblyopia if their refractive error is not identified and corrected.

Orbital ultrasound in the ocular ischaemic syndrome.

The ocular ischaemic syndrome illustrates well the effects of hypoperfusion of the globe and is a useful model for studying disorders of the orbital circulation. Recent advances in orbital ultrasound techniques have provided a non-invasive method of examining flow velocities in the orbital vessels, particularly the ophthalmic artery. Orbital ultrasound studies were performed on 3 cases of the ocular ischaemic syndrome. Continuous or intermittent reversal of blood flow in the ophthalmic artery was seen in all cases. How this phenomenon relates to the symptoms and signs in affected patients, and the role of orbital ultrasound in the investigation of the ocular ischaemic syndrome, are discussed.

A case of ocular tilt reaction and torsional nystagmus due to direct stimulation of the midbrain in man.

A case is presented of a patient who underwent insertion of a periaqueductal grey electrode for the relief of chronic pain. Shortly after insertion, the patient developed a left-sided ocular tilt reaction (OTR). The electrode tip, initially to the right of the midline, was then withdrawn slightly so that it was now on the left side. Stimulation at this point gave rise to a worsening of the tilt reaction, in addition to bilateral counter-clockwise torsional nystagmus. Computerized tomography and stereotactic coordinates indicated that the tip of the electrode was finally situated in the region of the left interstitial nucleus of Cajal (INC). The production of an OTR by stimulation in this region is similar to the phenomenon previously reported in monkeys and cats. The side of the OTR is consistent with previous evidence, suggesting that the utricular pathways cross between medulla and midbrain.

White sponge naevus and ocular coloboma.

White sponge naevus occurred in association with coloboma of the iris in six members of one family, across three generations. The association of these two dominantly inherited conditions has not to our knowledge previously been described.

Transient uniocular visual loss on deviation of the eye in association with intraorbital tumours.

Five patients with unilateral orbital tumours are described in whom transient loss of vision occurred on deviation of the affected eye from the primary position. Other presenting features were diplopia, proptosis, poor visual acuity, visual field defects, pupillary abnormalities, fundal changes and altered colour vision. Abnormalities on fluorescein angiography suggest that the visual loss is due to transient ischaemia. Temporary uniocular loss of vision on eye movement may be an early sign of an intra-orbital mass.
