Diclofenac epolamine is effective in the treatment of acute migraine attacks. A randomized, crossover, double blind, placebo-controlled, clinical study.

Hydrosoluble diclofenac epolamine (DHEP) represents an interesting approach to acute migraine attacks, where gastrointestinal motility and drug absorption are often reduced. Its efficacy was investigated in a randomized, crossover, double-blind trial on 155 patients who treated four consecutive mild-to-moderate migraine attacks, either with DHEP (65-mg sachet) or placebo. If pain was not relieved within 1 h, a second dose was given. The total number of treated attacks was 481. A pain-free condition was achieved within 2 h in 45.8% and 25.1% of attacks treated, respectively, with DHEP or placebo (P < 0.0001), with a therapeutic gain of 20.7%. Time to attack resolution, light and noise sensitivity and impact on working ability were significantly reduced by DHEP compared with placebo. Moreover, significantly fewer patients required a second drug dose or a rescue medication when treated with DHEP than with placebo. No adverse reaction was recorded. In conclusion, DHEP was effective and safe for pain relief in patients with an acute mild-to-moderate migraine attack.

[Caries, gingivitis and dental abnormalities in patients with cleft lip and palate].

Oral health was studied in 31 children aged 10-12 years with cleft lip and palate and compared to 31 noncleft controls matched for sex and age. We found a difference in the prevalence and activity of caries with increase in cleft lip and palate patients. The unilateral crossbite and the congenitally missing teeth were more common in cleft palate children and the supernumerary teeth were fewer. Almost all children used fluoride tablets.

[Attempted suicide with verapamil].

A 26-year old woman took 4000 mg (100 tablets) of VerpamilR (Orion Pharmaceutica, Helsinki) with suicidal intestions. As a consequence of the poisoning cardiogenic shock, 3rd grade AV block and cardiac arrest occurred. The disturbance of stimulus formation and conduction was treated with a temporary pacemaker. Discussing the therapeutical possibilities it is stated that cardiovascular symptoms may be influenced successfully by Ca++ preparations (Calcium gluconicum, Calcium chloratum) and beta-receptor stimulants (isoproterenol, dopamine, orciprenalin) possibly also with glucagon.

Effect of different prostaglandin analogues on gastric acid secretion and mucosal blood flow in the dog: action on stimulated and resting mucosa.

The effect of PG-E2, 16-16-DMPGE2, PG-F2 alpha and PG-I2 was investigated on histamine stimulated and resting canine gastric mucosa. PG-F2 alpha had no effect on mucosal circulation or acid secretion. PG-E2 and 16-16-DMPGE2 showed an increasing effect on blood flow of the resting mucosa. PG-E2, 16-16-DMPGE2 and PG-I2 decreased of acid secretion; the effect seemed to be due to a direct action on the parietal cells.

Measurement of gastric mucosal blood flow in dogs by the 99mTc-4-methylaminophenazone clearance technique.

In anesthetized dogs, correlation was found between the 99mTc-4-methylaminophenazone and aminopyrine clearance measured in the same animal after administration of the gastric secretory stimulants vasopressin or glucagon. Similar correlation was observed between the 99mTc-4-methylaminophenazone and [14C]aminopyrine clearance during histamine administration. The clearance values were always corrected by the actual degree of dissociation of the radioactive molecules. The results suggest that 99mTc-4-methylaminophenazone is suitable to study circulatory changes in human gastric mucosa. Advantages of this compound include a much lower dose of radiation absorbed by the patient and his surroundings, a simplified measurement technique, and considerably reduced expenses.

[Results of measuring the blood flow in the gastric mucosa of dogs by the 99mTc-4-methylaminophenazone clearance method].

In anesthetized dogs, a good correlation was found between the [99mTc]-4-methylaminophenazone and aminopyrine clearances measured in the same animal after administration of gastric secretory stimulants vasopressin or glucagon. A similarly good correlation was observed between the [99mTc]-4-methylaminophenazone and [14C]-aminopyrine clearance during histamine administration. It has the advantages of a much lower absorbed dose of radiation for the patient and his surroundings, a simplified measurement technique and considerably reduced expenses.

Interrelation between gastric blood flow and HCl secretion in dogs. The basal condition and influence of secretory stimulants and vasoactive substances.

Total and mucosal blood flow in the nonsecreting stomach and the interrelation between local blood flow changes and gastric hydrochloric acid secretion as influenced by various drugs or hormones were investigated in 188 anaesthetized dogs. Substances acting on gastric acid secretion (histamine, pentagastrin, atropine and metiamide) and those showing vasoactive properties (norepinephrine, epinephrine, Hypertensin, nicotinic acid and glucagon) were used. Instillation of 0.1 N HCl solution into the stomach provided a good estimate of the mucosal blood flow of the nonsecreting stomach as measured by the aminopyrine clearance technique of Jacobson et al. Simultaneous recording of the total gastric blood flow with an electromagnetic blood flowmeter revealed the distribution of blood flowing through the mucosal and non-mucosal (submucosa-muscle) tissues of the resting stomach. During acid stimulation a shift of the gastric blood flow to the mucosa was observed, which may reach even 75--80% of the total amount of the blood supply during a given period. Metiamide entirely inhibited the gastric acid secretion induced by both histamine and pentagastrin, but did not parallelly diminish mucosal blood flow. Given during histamine infusion, glucagon strongly inhibited acid secretion while it did not decrease mucosal blood flow.
