The effect of brief intrascleral or episcleral application of mitomycin C on the ciliary epithelium and pressure in the rabbit eye.

Ten albino rabbits underwent bilateral Cairns' trabeculectomy (group 1). A sponge soaked with 0.5 mg/ml mitomycin C solution was applied under the scleral flap for 5 min followed by irrigation with 20 ml 0.9% sodium chloride solution in the left eyes. In the right eyes a sponge soaked with 0.9% sodium chloride solutions was applied. In 9 albino rabbits a 3 x 2 mm episcleral area at 12 o'clock position was exposed to 0.5 mg/ml mitomycin C solution on one side and to 0.9% sodium chloride solution in the contralateral eye (group 2). Five, 10 and 30 days (group 1 and 2) and 3 months (group 1) following surgery specimens of the ciliary body adjacent, beside and opposite to the site of the operation were excised and studied with light and electron microscopy. No stromal or epithelial morphological changes of toxic origin have been revealed in the ciliary processes, even in specimens adjacent to the surgical area. In 5 albino rabbits (group 3) intraocular pressure was recorded before surgery and 5, 10 and 30 days after unilateral, episcleral mitomycin C application described in group 2. No significant changes or interocular difference has been recorded. The results suggest that long-term postoperative hypotony following trabeculectomy with mitomycin C is not caused by reduced aqueous humour secretion of toxic origin.

The surgical correction of refractive errors of the eye.

Authors review the available surgical methods, the radial keratotomy, the keratophakia, the keratomileusis, the epikeratophakia and the excimer laser technique which are available presently in order to change the refractive power of the eye. The newest excimer technique is discussed in detail. During the past one and a half year period authors performed and closely followed 37 photorefractive keratectomies with the use of Aesculap Meditec MEL 60 excimer laser. They present the postoperative results, according to their experiences, the method is highly effective and predictable in treating myopic refractive failures. The side effects were of no clinical importance.

Vogt-Koyanagi-Harada syndrome.

The case history is presented of a 40-year-old White women with the Vogt-Koyanagi-Harada syndrome, who was successfully treated with a combination of low-dose prednisolone and cyclosporine. A. Cellular and humoral hypersensitivity to uveal and retinal antigens were demonstrated in the patient.

[Examination of the orbit using a non-ophthalmologic ultrasound equipment].

Ophthalmic examinations were performed for determining the suitability of seven different types of non-ophthalmic ultrasound equipments for ophthalmological purposes. According to the results, the instruments with probes of 5 MHz or higher frequency are appropriate for certain screening ophthalmological purposes in hospitals where ophthalmic ultrasound instrument is still missing. The possible indications are: approximate estimation of axial eye length for intraocular lens calculation, and visualization of gross intraocular and orbital diseases, such as retinal detachment, tumours, foreign bodies etc. In case of pathological result or in case of axial eye length different from the average value, the high resolution ophthalmological ultrasound examination is mandatory. The modern colour-coded Doppler ultrasound equipments were found suitable for the examination of the circulation of the eye and orbit.

Diffuse type alteration of the ocular wall in different eye diseases.

The ocular wall dimensions (thickness and volume) were measured by means of ultrasonography in 61 eyes of 61 patients with different eye diseases and 31 eyes of 31 healthy subjects with or without refractive error. The results confirmed, that in spite of the great variance in the ocular wall thickness, depending on the axial eye length, the volume of the ocular coats is constant in healthy eyes (ca. 1.65 cm3). In patients with ocular hypotony (eyes after intraocular surgery or trauma, and eyes with uveitis or with phthisis bulbi) and in patients with exophthalmus, the ocular wall was thicker and larger in volume than in the healthy control groups. In contrast, the ocular wall dimensions (thickness and volume) were smaller in patients with glaucoma. The results of the study showed that an alteration in the volume of the ocular wall implies a pathological condition of the eye, and thus ultrasound examination of the ocular wall dimensions might play a role in the diagnosis and in the follow-up of the patient.

Sclerokeratoplasty in recurrent pterygium.

There are many methods for the treatment of recurrent pterygium, and lamellar keratoplasty seems to be the most effective of them. A new method has been developed for the surgical treatment of recurrent pterygium. The most important steps in the procedure are: total removal of the cicatrix and then covering the bare corneoscleral surface with a single piece of corneoscleral tissue taken from a cadaver bulb. The operation was carried out in 11 cases. The follow-up period varied from 10 months to 4 years, the average being 28.7 months. No recurrences were observed. Pseudopterygium developed following the ninth removal of the recurring pterygium; however, even 2 years after the operation, it did not reach the area of the pupil.

Glaucoma capsulare in patients with open-angle glaucoma in Hungary.

The data of 403 inpatients with open-angle glaucoma were selected between 1977-1986, 155 had glaucoma capsulare (GC) and 248 suffered from primary open-angle glaucoma (POAG). The incidence rate of GC was 38.4% in the total material. In the GC group, the peak of age distribution was between the ages 70-79 (45.8%). The male/female ratio of the total GC material was 1.67, that in POAG was 0.85. The difference between the two glaucoma groups was significant.

[The histopathology of keratoconus posticus circumscriptus].

The histopathology of keratoconus posticus circumscriptus is described. On the basis of electron microscopic studies a corneal development anomaly may be suspected. This hypothesis is also supported by the intercellular junctions (gap junctions) between the basal epithelial cells and the differences in the diameters of the collagen fibers (10-30 nm). The disturbance in fibrillogenesis is the result of stromal cell degeneration.

Morphological changes in "dry eye syndrome".

The authors present the pathologic changes of the conjunctiva in "dry eye syndrome". They state that stratification of the epithelial cells and microplicae, change in the goblet cell population, and inflammation of the basal tissue cannot be regarded as characteristic symptoms.

[The endothelial cells in acute keratoconus (author's transl)].

In the light of electron microscopic examinations the author emphasizes the role of the endothelial cells in the development of acute keratoconus. The increased activity of the endothelial cells results in disintegration of Descemet's membrane and in the formation of "Descemet ruptures".
