Inter-hospital mobile oesophago-gastro-bulboscopy emergency service.

Oesophago-gastro-bulboscopy was performed in 102 cases of acute massive haematemesis and/or melaena in different hospitals or clinics in the town of Pécs, the condition of the patients having been in general incompatible with transport to the endoscopic laboratory. The aetiology of the bleeding was clarified in 90 cases, it remained unidentified in 8 cases, though it was successfully localized. Fibreoptic endoscopy is regarded as an essential tool of early aetiological diagnosis of upper digestive tract bleedings even in centres lacking modern equipment.

Fiberscopic diagnosis of gastric carcinoma: results of a two-stage screening.

After preliminary screening, 162 cases of gastric carcinoma were diagnosed by fiberscopy. Microscopic study revealed that 13 of these cases, i. e. 8% were in the early stage. The clinical features of these early cases are analysed. It is concluded that 1. persistent gastric symptoms call for fiberscopic investigation even if the previous finding was negative. 2. The relative youth dose not exclude early gastric cancers. 3. Improvement of the detection rate of early gastric carcinoma rests with the use of fiberscopy in combination with gastric biopsy.

Endoscopic assessment of gastric polyps.

The results of fibre gastroscopy in 100 patients with gastric polyp are reported. The polyps were removed in 38 patients. All cases were followed up. No malignant transformation of the polyp was demonstrable in any of the cases. The polypous growth proved to be early gastric carcinoma in two cases, neurogenic sarcoma in one case. The polyp was associated with gastric carcinoma in 11 cases, with gastric sarcoma in one case, and with tumours of other digestive organs in five cases. It has been concluded that 1. diagnosis and correct assessment of gastric polyps rest on the evidence of endoscopy. 2. In the majority of cases polypoid lesions of the stomach are either benign or malignant from the very outset. Endoscopy coupled with biopsy directed at the representative sites of the polyp permits differentiation between benigh and malignant cases with a fair degree of accuracy. 3. Removal of polyps should be selective. 4. Association of gastric polyps with gastric carcinoma is far more frequent than malignant transformation of the polyp. 5. Endoscopic follow-up of polyps is imperative even in polypectomized cases. 6. Detection of a gastric polyp necessitates a search for tumours of other organs.

Male gonadal function in survivors of childhood Hodgkin and non-Hodgkin lymphoma.

The aim of this study was to investigate the impact of therapy on long-term gonadal function of young people cured of childhood lymphomas and to assess whether a prepubertal state during the treatment protects the gonads from chemotherapy and/or radiotherapy late effects. Clinical evaluation, semen analysis, and endocrine status were studied in 20 survivors of childhood lymphomas. Five patients received Inverted Y radiotherapy, 2320 cGy (1550-4000); all 20 received chemotherapy as follows: MOPP/ABVD protocol, 9 patients; COMP protocol, 5 patients; MOPP protocol, 3 patients; other protocols, 3 patients. Semen analysis results were as follows: normal values, 4/20 patients; oligospermia, 8/20 patients; azoospermia, 8/20 patients; FSH above normal level, 10/20 patients; 4/5 who received Inverted Y irradiation were azoospermic and 1 was severely oligospermic. Treatment damage to the testis involves tubular germinal elements. Radiotherapy and chemotherapy combinations that included nitrogen mustard or cyclophosphamide were associated with high rates of oligospermia and azoospermia. MOPP/ABVD combination did not have a significant better outcome of sperm counts compared to MOPP alone. Age at chemotherapy did not correlate with the sperm count; hence a prepubertal state did not protect the gonad from the late effects of treatment.
