Rare choroidal tumour simulating a malignant melanoma.

A 15-year-old boy had been treated for a central chorioretinitis of his right eye. The other eye had always been normal. Two years later he presented a relapse with deterioration of central visual acuity. All clinical tests, including fluorescein angiography, suggested an inflammatory process this time again. Thereafter, the patient did not show up for 6 years. Then he came to hospital because of strong pains in his right eye. Now, a blind and inflamed eye was found with a huge tumour from the optic nerve extending to the temporal periphery. All clinical investigations, amongst others ultrasonography, computerized tomography and magnetic resonance tomography, suggested a choroidal melanoma with extrabulbar extension. An enucleation with partial exenteration was performed. A demonstration and discussion of the clinical findings and the rare and complicated histological diagnosis of a very unusual ectopic meningioma-like lesion are given.

[The dry eye in kidney transplant patients].

Fifty-three patients with severe renal disease and/or kidney transplants underwent full ophthalmological examinations, including tear breakup time, Schirmer's test, and impression cytology of the conjunctivae. Although only some 50% of the patients complained of typical dry-eye symptoms, pathologic changes of the outer eye were found in almost 70%. Tear break-up times and Schirmer's test values were greatly reduced. Morphological changes of the conjunctiva were found in almost all the patients; in 50% of these cases the changes were moderate to severe.

[Effect of commercial beta blockers and their combination with artificial tears on the duration of tear film stability].

The influences of 8 different beta-blockers on the tear film stability-measured on the breakup time (BUT) of normal and healthy volunteers-was compared. All beta-blockers led to BUT reduction, however, the lowest influence was observed with levobunolol. The duration of this effect was compared with that of levobunolol, timolol and metipranolol. While the minor BUT alterations induced by levobunolol recovered after 30 min and were completely restored after 1 h, those of the two other beta-blockers lasted for over 2 h. The combination of timolol 0.5% with different artificial tears containing mainly polyvinyl alcohol (PVA) 1.4%, methylcellulose and polyacrylic acid initially led also to BUT reduction. The best results were obtained with PVA 1.4%.

[Orbital lipogranuloma].

Orbital lipogranulomas are rare inflammatory pseudotumors. Clinical and histopathological findings of 4 own cases are demonstrated and discussed.

Complications by liquid plastics in ophthalmic surgery. Histopathologic study.

Retrospective morphological studies of ocular tissues after treatment with liquid plastics reveal many complications induced by the material. Cyanoacrylates for punctal occlusion glide into the lacrimal sac and lead to obstructive dacryocystitis and even to reactive xanthogranuloma. The use of this glue in combination with a hard contact lens as 'artificial epithelium' provokes a remarkable stromal and endothelial cell loss of the cornea. Intraocular silicone oil sometimes induces a massive epiretinal macrophage activity with storage of the material or reactive fibrosis when it gets into the subretinal space. Liquid alloplasts for intraocular lens surgery of the future are tested in experimental studies but are still not satisfactory.

[Immediate effect of a topical beta-blocker on breakup time].

The comparison of different beta-blocking agents and their influences on the breakup time (BUT) reveals that levobunolol reduces the BUT in the same way as physiologic saline solutions, while other beta-blockers lead to an enormous reduction of BUT.

Adrenergic beta-Antagonists

[Timolol: long-term results, tolerance test and early-morning measurements (author's transl)].

In a study lasting 16 months, Timolol was tested on 57 eyes of a group of selected patients with open-angle glaucoma. Patients whose IOP could not be stabilized with the 0.25% solution were treated with 0.5% drops twice a day. In five separate cases, where regulation was inadequate, pilocarpine 2% twice a day was prescribed in addition. At the end of 16 months' treatment pressure had been lowered to a statistically highly significant degree in 66% of the patients. While the degree of regulation in the remainder was not statistically sifnificant, most of them were adequately stabilized. Further, following stabilization with Timolol, 42% of the patients were subjected to tolerance tests under constant conditions (darkness test, water test, coffee and tea tolerance) and their 6.00 a.m.values were measured. While good pressure levels were recorded in the tolerance tests, the early-morning IOP rose to critical threshold levels in some patients. However, during the entire period of observation visual fields and optic disk findings remained unaltered in all cases. Only a few cases were subjective and/or objective side-effects seen. Ten percent of those patients who had been treated previously required new glasses (+0,5 to +1 D over the previous correction).

[Conjunctival myxomas (author's transl)].

Myxomas are rare mesenchymal tumors appear everywhere in the body, but occur very seldom in orbital or subconjunctival regions. Clinical and histopathological demonstration of two cases of epibulbar myxoma and discussion of possible pathogenetic mechanisms.

Aged

[Aggressive eyelid basalioma with erythroderma ichthyosiforme congenita sicca].

Report on an agressive eyelid basalioma which led, in a patient with Erythrodermia ichthyosiformis congenita sicca, to exenteration of the oribt. Presentation of clinical and histopathological findings and discussion about the histogenic character of both diseases considering also present literature on this subject.
