[Evaluation of surgical methods of treatment for retinal detachment].

During last year we have used episcleral buckling and cryopexy in 63 eyes with retinal detachment. Final reattachment of the retina after 6 months follow-up was achieved in 52 eyes (83%) after one or two surgical procedures. 4 patients refused further surgery. In 7 cases pars plana vitrectomy and silicone oil tamponade were used. In 4 cases complete retinal reattachment was obtained. In further 2 eyes central retina was attached but flat detachment in the lower part of the retina was observed. Visual acuity of 0-0.5/50 was achieved in 5 eyes, 1/50-4/50 in 5 eyes, 5/50-5/12 in 18 eyes and 5/10-5/5 in 35 eyes. Our results show that the majority of cases with retinal detachment can be successfully treated with episcleral buckling and cryopexy. Pars plana vitrectomy and silicone oil tamponade allowed some success in only the small number of eyes that otherwise would have a prognosis.

[Prognosis of the development of pathological conditions in open-angle glaucoma].

The authors prepared a matrix of probability of transition for each of the stages of the disease between the particular years of observation of such characteristics as: the visual acuity, visual field, intraocular pressure and the condition of the lens in 435 persons (854 eyes) with simple glaucoma. The greatest probability of an unchanged condition of the optic nerve and a normal visual field at the onset of treatment (0.45 and 0.74). For the severely damaged eyes the indexes amount respectively 0.07 and 0.34.

[Treatment of diffuse diabetic maculopathy with "grid pattern argon laser coagulation"].

Sixty eyes of 42 patients with diffuse diabetic macula edema were treated with argon laser in a "grid pattern" mode. The eyes were divided into three groups according to the preoperative visual acuity. In the first group with a preoperative visual acuity of below 0.1 (23 eyes), after 6 months of follow-up, 5 eyes improved in function by at least two lines on a Snellen chart. In the second group with an initial visual acuity of 0.2-0.4 (20 eyes) 5 eyes showed improvement and in the group with a pretreatment visual acuity of 0.5 or more (17 eyes) 6 eyes improved in function. In the first group, the visual acuity did not worsen; in the second group it worsened in 4 eyes. Two eyes in the third group showed a worse visual acuity after treatment. Similar results were found after 1 year of follow-up (27 eyes). After a 2-year follow-up (13 eyes) 9 eyes had a stable visual acuity compared to the preoperative visual acuity. Even though the number of cases with a long follow-up was relatively small, our results show clearly that early application of argon laser coagulation for diffuse macular edema allows stabilization to be achieved and, in some cases, even improvement in the visual acuity.

[Results of the treatment of diffuse diabetic macular edema by laser coagulation of the scatter grid type. Preliminary report].

The authors present a method of treatment of diffuse diabetic macular oedema by means of photocoagulation of scatter grid type. Discussed are observations in the period of 6 months to 1 year after intervention. The technique of surgery, indications and all other factors which could have an influence on the final functional results are discussed in detail. Small risk of this surgical intervention and a good efficacy of this method is evaluated as a stabilization of the visual acuity, they allow to suggest that this method may play an important role in the treatment of diffuse diabetic macular oedema.

[Ciliary circulation in glaucoma complicated by diabetes mellitus].

The speed of blood perfusion and the pulsation curve in ciliary arteries was checked by the method of Doppler's pulsating focused ultrasonography in 30 patients aged 46-78 years treated for glaucoma and diabetes. The data obtained were compared with analogical data in nondiabetic patients with glaucoma. It was established that the increase of the intraocular pressure caused a larger and earlier impairment of the regulating mechanism of the ciliary microcirculation in diabetic patients with glaucoma in comparison with the group of glaucomatous patients without diabetes. Particularly high values of pulsation indexes for ciliary arteries were observed in eyes with glaucoma and with diabetic changes rated as the fifth group of diabetic changes.

[Pars plana vitrectomy following accidental injection of depot steroids into the vitreous body].

The authors describe a case of accidental intraocular injection of depot steroids after treatment of a chalazion by an intralesional corticosteroid injection. The eye was treated by pars plana vitrectomy. Visual acuity improved postoperatively, ultimately attaining 1/16. As late complications, pathologic changes of the macula were observed, with macular pucker and optic nerve atrophy.

[Correlation of blood flow in the posterior ciliary arteries with visual field changes and the appearance of the optic disc in patients with primary glaucoma].

The velocity of blood perfusion and pulsation curve in posterior ciliary arteries was evaluated in 180 patients (476 eyes) with primary glaucoma, aged 22 to 81 years. The investigations were carried out by Doppler's focused pulsating ultrasonographic method with probes of 2 and 8 MHz frequency connected with a TC-2-64 apparatus (made by EME of German Federal Republic). The blood perfusion in the posterior ciliary arteries was compared with the changes in the visual field, in the aspect of the optic disc in patients with primary glaucoma and various levels of the IOP. In patients with primary glaucoma and advanced fundus changes (of 2-nd and 3-rd grade) the mechanisms of regulation of the ciliary circulation were normally functioning for quite a long time. In the period of breaking down of the mechanisms the changes in the visual field came into being.

[Comparative evaluation of blood flow in posterior ciliary arteries in patients with glaucoma after surgical and pharmacological treatment].

The perfusion of blood and pulsation curve in the posterior ciliary arteries in glaucomatous patients was evaluated before and after surgical and pharmacological treatment by Doppler's focused pulsating ultrasonographic method by means of a TC-2-64 apparatus with a probe of 8 MHz frequency. Twenty two patients were treated only conservatively, 27 by a surgical intervention. Before the installation of the treatment the intraocular pressure was higher than 35 mm Hg (averagely), the perfusion velocity was lowered and the indexes of pulsation elevated. After surgery the IOP was lowered--averagely beneath 15 mm Hg, the velocity of blood perfusion and pulsation indexes showed normal values in all the operated persons. After pharmacological treatment the IOP lowered but its mean value persisted in the 20-22 mm Hg range. Only in patients with early glaucomatous changes the blood perfusion remained normal with the intraocular pressure around 20 mm Hg. Instead in patients with advanced glaucomatous changes the velocity of the blood perfusion and the pulsation indexes showed pathological values with the IOP around 20 mm Hg.

[Evaluation of methods for reproducing doppler pulsating focused ultrasonography].

In 53 patients treated for primary glaucoma the authors performed 2 examinations of blood perfusion in the posterior ciliary arteries during 2 following days. The patients had been examined in the period of a normalized IOP. The difference in the IOP between the first and second examination did not exceed 3.3 mm Hg and the difference in the mean general blood pressure--10 mm Hg. The blood perfusion in the posterior ciliary arteries was checked by Doppler's pulsating focused ultrasonography, by means of a TC-2-64 apparatus with a probe of 8.0 MHz frequency. The velocity of the blood perfusion, the pulsating index and the transmission of pulsation were evaluated. The differences between respective parameters characterizing the blood perfusion in the posterior ciliary arteries between the first and second examination were statistically insignificant, on the importance level of 0.01.

[Value of vitrectomy and lensectomy in the treatment of complicated cataracts and complications of eye injuries].

Vitrectomy and/or lensectomy was performed in 33 successive cases of traumatic and complicated cataracts. The discussed cases were divided into 2 groups: 1. late reconstructions of the anterior eye segment after injuries and 2. complicated cataracts. After surgery the visual acuity amounted 6/60 or better in 27 from 33 eyes (81.8 per cent). Full visual acuity was attained in 18 patients (54.5 per cent).

[Vitrectomy in the early treatment of extensive penetrating eye injuries. Comparative studies].

Presented are early and late results of treatment of extensive perforating eye injuries treated by vitrectomy immediately after the patient reported for a hospital treatment. For comparison--a similar group of patients with corresponding posttraumatic lesions was selected; they were treated by traditional methods and in some cases also by vitrectomy but performed only for the treatment of late complications. The results obtained confirm the necessity of early vitrectomy as a method of reconstruction of the anatomical conditions after an extensive perforating injury.

[Results of the treatment of infectious endophthalmitis by vitrectomy].

Infective endophthalmitis is a severe ophthalmological condition and may occur as a consequence of a perforating injury or of a surgical procedure. The paper presents personal results of treatment of this disease with the use of vitrectomy and application of intraocular antibiotics. The visual acuity after completion of the treatment was 6/6 to 6/8 in 35.7% of cases. Besides--contemporary opinions on the treatment of infective endophthalmitis are presented on the basis of experience and data from the literature.

[Vitrectomy in the treatment of vitreous body prolapse after cataract extraction].

Vitreous prolapse into the anterior chamber after cataract extraction is a severe complication in cases in which there is a contact between the vitreous and the cornea. The present paper discusses the result of treatment by vitrectomy in 15 patients with this complication. Anatomical and functional improvement was attained in 12 cases (89 p.c.) and in 5 among them--full visual acuity was obtained.

[Evaluation of blood perfusion velocity in the ophthalmic artery of glaucoma patients].

One hundred and ten examination were performed in 71 patients with primary glaucoma aged 21-81 years (217 eyes). The blood perfusion speed in the ophthalmic artery was measured in each patient by the ultrasonographic pulsating Doppler's method with a probe of a 2.0 MHz frequency by means of a TC-2-64 apparatus of EME production. The ophthalmic artery was examined by transorbital technique according to Spencer and Whisler on a 40-50 mm depth. The examined orbits were divided into 4 groups in dependence of the grade of the optic nerve atrophy. A lowering of over 50 per cent of the blood speed (below 10 cm/s) was found in 10.6 per cent of the eyes. Optic atrophy of the 3d and 4th grade was revealed in 70 p.c. of these eyes; the intraocular pressure remained in normal limits.

[Evaluation of blood flow velocity and pulsation curve in the posterior ciliary arteries in glaucoma].

One hundred and thirty two examinations (253 eyes) were performed in 78 patients with primary glaucoma, 21-81 years of age. The blood speed and pulsation curve in the posterior ciliary arteries were evaluated by the ultrasonographic pulsating focused Doppler's method with a probe of 8 MHz frequency, connected to the TC-2-64 apparatus of EME production in eyes with a various grade of the optic nerve lesion and by various values of the IOP. It was shown that in eyes with early glaucomatous changes of the optic nerve the correct perfusion in the ciliary circulation is secured in the presence of a rise of the IOP up to 34 mm Hg. Instead in eyes with an advanced optic atrophy a normal perfusion in the ciliary arteries persists only when the IOP is lower than 15 mm Hg.

[Use of Betaxolol in patients with glaucoma and chronic nonspecific diseases of the respiratory system].

Betaxolol is a new antiglaucomatous drug which is blocking solely the beta 1 receptors and in contrast to Timolol may be applied in persons with spastic conditions of the bronchi. It has been given twice daily to 9 persons with glaucoma and long standing bronchial asthma. In the period of observation (1.5-6.5 months) neither of the examined patients showed any deterioration of the spirometric indexes nor bradycardia. In a part of the patients the intraocular pressure was normalized only by application of Betaxolol; over one half of them however needed additionally the use of miotics.
