Chemosensitivity in patients with diabetic neuropathy.

1. By using the rebreathing method, ventilatory responses to inhaled CO2 were studied in patients with diabetic neuropathy. 2 Their general respiratory functions including vital capacity (VC) and forced expiratory volume in 1 s (FEV 1.0) were within normal ranges and ventilatory responses to CO2 in the range 5.32--5.99 kPa were not significantly different between patients and normal subjects. However, ventilation in the patients was significantly less than that in normal subjects when the alveolar partial pressure of CO2 (PACO2) was above 6.65 kPa. 3. The slope of the CO2 response curves obtained during hyperoxic rebreathing was significantly less in the patients than in the normal subjects. In the patients CO2 response curves with 15% (v/v) O2 were either the same or had lower slopes than those measured with 100% O2. 4. The results suggest that both peripheral and central chemosensitivity, including the activity of the respiratory centre, are reduced in patients with diabetic neuropathy.

[Clinical studies on flow properties of blood in the gynecological patients with potentially hypercoagulable state (author's transl)].

The flow property of blood, the blood viscosity was measured under such gynecological conditions that were potentially hypercoagulable: in the post-operative state for myoma uteri corporis, in the use of oral contraceptives and in the advanced stage of malignant uterine and ovarian tumors. The viscosity was measured at a shear rate of 0.989 sec-1 and at 37 degrees C. Blood viscosity after operation rose to 15.6 +/- 3.7 cp (mean +/- SD), which was significantly higher than that of pre-operative state (12.4 +/- 3.7 cp). This was due to increased hematocrit and fibrinogen levels. The viscosity returned to preoperative level on the 3rd day of post-operation. The blood viscosity of users of oral contraceptives was 15.1 +/- 2.7 cp, not being significantly higher than that of control women (13.4 +/- 2.8 cp). The blood viscosity of patients suffered from malignant tumors was 10.5 +/- 2.0 cp and was significantly lower than that of control. Comparison of viscosity at the same hematocrits revealed, however, that the viscosity of patients of malignant tumors was significantly higher than that of control. Such rheological characteristics in the patients of malignant tumors were due to decreased hematocrit and markedly increased fibrinogen levels. It was suggested that dehydration and infection should be avoided to prevent thrombus formation in the gynecological patients with potentially hypercoagulable state.

Doxazosin improves insulin sensitivity in hypertensive patients.

Insulin sensitivity in terms of glucose disposal rate was measured by the hyperinsulinemic/euglycemic clamp technique in 10 hypertensive patients and 7 normotensive control subjects before and after 12 weeks of doxazosin therapy. Supine blood pressure fell significantly, from 150/98 mmHg before treatment to 136/90 mmHg after treatment (systolic P < 0.001/diastolic P < 0.01). The average dose of doxazosin at the end of the study was 3.3 +/- 0.4 mg/day. The glucose disposal rate during the last 30 minutes of the glucose clamp procedure was significantly increased, from 5.8 +/- 0.7 mg/kg/min before to 7.8 +/- 0.7 mg/kg/min after treatment (P < 0.02). Insulin sensitivity was 8.7 +/- 0.4 mg/kg/min in the normotensive subjects, which represented a significant difference when compared with that of the hypertensive subjects before treatment (P < 0.01). Steady-state serum insulin concentrations, as measured during the glucose clamp procedure, were 172 +/- 10.4 microU/ml before and 176 +/- 13.5 microU/ml after doxazosin treatment; these levels were significantly higher than the 137 +/- 7.0 microU/ml reading obtained in the normotensive subjects (P < 0.05). Study results show that hypertensive patients tend to be insulin resistant and that treatment with doxazosin improves insulin sensitivity.
