Interpretation of the fetal ECG during labor: the effect of uterine contractions.

This study was performed in order to investigate the fetal electrocardiogram (FECG) during uterine contractions associated with normal labor. Twenty-five patients with low risk pregnancy between 38-41 weeks gestation were studied during the active stage of labor. Both FECG and intra-uterine pressure are obtained in a conventional manner and are continually sampled into the computer. The FECG is averaged point-to-point, synchronized to the peak of the R-wave. This is performed by a QRS detection algorithm which is based on a digital analysis of slope, amplitude and width. A digital band-pass filter composed of cascaded high-pass and low-pass filters reduces false detections and permits the use of auto-adjustable low thresholds. A separate averaging is performed on the T-wave in order to prevent attenuation due to variable R-T interval. The T wave is subsequently aligned in time and position to the rest of the QRS complex. A significant increase was observed in the T/QRS amplitude ratio during the first half of the uterine contraction. Such an increase was also observed in the short and long-term FHR variability. No significant changes were observed in the other components of the FECG. In conclusion, by implementing a computer based system it is possible to analyse the FECG during labor. Based on this and previous studies it may well prove to be a sensitive indicator of fetal condition.

Effect of acute hypoxia on human fetal heart rate. The significance of increased heart rate variability.

The fetal heart rate response to acute hypoxemia was observed during an accidental drop in oxygen concentration inspired by the mother in labor. There was a marked increase in FHR variability in the early stage of hypoxia and also during the recovery phase. While an increase in fetal heart variability is generally considered to be a normal physiological event, it may also indicate a state of fetal hypoxemia associated with an intact fetal circulation, and should be treated with caution during labor.

Increased alpha-fetoprotein levels in mixed mesodermal tumor of the ovary.

2 Cases of mixed mesodermal tumor of the ovary with elevated peripheral levels of alpha-fetoprotein (alpha-FP) were studied. The morphology of these tumors is presented. The finding of hyaline droplets of alpha-FP is of special interest. The histogenesis of the tumor was reappraised in light of the increased level of the marker.

Treatment of ovarian hyperstimulation syndrome: the physiologic basis for a modified approach.

A case of severe ovarian hyperstimulation syndrome (OHSS) secondary to human menopausal gonadotropin-human chorionic gonadotropin therapy is presented. Draining 4000 ml of exudate by abdominal paracentesis under real-time B-scan imaging induced a marked improvement in the patient's condition. Fluids from the third space were rapidly excreted, renal function improved, and the patient's weight decreased substantially. The underlying physiologic factors responsible for these changes are discussed. Other modes of treatment, including salt and water restriction and the use of volume expanders and diuretics, had no significant effect on the course of the syndrome. Paracentesis has a definite therapeutic value and is recommended in cases of OHSS with tense ascites.

Effects of maternal cigarette smoking on fetal breathing and fetal movements.

The effect of cigarette smoking on fetal breathing and fetal movements was studied in 10 women with low-risk pregnancy of 34 to 38 weeks' gestation. A continuous Doppler ultrasound method was used to measure fetal breath intervals. Smoking two cigarettes caused a small increase in the rate of fetal breathing (p < 0.01), which persisted for 60 minutes; the proportion of time the fetus spent breathing was not changed. There was a decrease in the number of fetal movements felt by the mother. These findings are discussed in relation to the alleged pathophysiologic effects of tobacco smoking on the fetus.

Serum and amniotic fluid heat labile alkaline phosphatase and aminotransferases in association with meconium stained liquor. On their origin, distribution and clinical use.

A method for detecting meconium in the amniotic fluid is described. Samples of maternal serum, cord serum and amniotic fluid were tested for alkaline phosphatase and transaminases activity in 32 healthy, term gravidas during labor in whom meconium stained amniotic fluid was found. The values obtained were compared to those of a control group consisting of 32 normal term pregnancies in whom the amniotic fluid was clear. Mean alkaline phosphatase activity was significantly higher in the study group in all three compartments - maternal, fetal and amniotic fluid. A significant and positive correlation between levels of alkaline phosphatase in the amniotic fluid and in maternal serum was found in the study group. Alkaline phosphatase value of 550 IU/L or more in maternal serum was diagnostic and levels between 350--500 IU/L were highly suggestive for the presence of menconium in the amniotic fluid. Mean levels of transaminases (SGOT and SGPT) were similar in the two groups and all were within normal range. The origin of alkaline phosphatase and the mode of transfer of the three enzymes are also discussed. It is suggested that this method can also be used during the third trimester of pregnancy.

Intrauterine device and pelvic inflammatory disease.

Using a retrospective case control design on 101 women with a first episode of acute pelvic inflammatory disease (PID), it was found that 15% were wearing an intrauterine device, as compared to 7% out of a control group of 101 women matched for age, marital status, and interval since their last pregnancy termination. No statistically significant correlation between IUD usage and PID was demonstrated. A significant correlation (P less than 0.01) between previous induced abortion and subsequent PID was found. In the PID group, a significantly higher proportion of previous abdominal and pelvic operations (P less than 0.005) was found as compared to the control group, but the numbers were small. In the absence of a higher frequency of IUD wearers among PID patients as compared with matched controls, we do not believe that there is an increased risk of pelvic inflammatory disease.
