Overnight clonidine suppression test in the diagnosis and exclusion of pheochromocytoma.

In a prospective study designed to differentiate pheochromocytoma from other forms of hypertension, urinary catecholamines were measured after sleep and clonidine administration in 12 patients with pheochromocytoma, 19 hypertensive patients in whom pheochromocytoma was suspected but later excluded, and 31 hypertensive patients in whom pheochromocytoma was never suspected. The test correctly identified all 12 patients in whom pheochromocytoma was present. Four of these had equivocal plasma levels of both norepinephrine and epinephrine, suggesting that overnight clonidine suppression may be of particular value when tumor secretion is intermittent or low. When pheochromocytoma was not present, urinary norepinephrine and epinephrine levels were suppressed below 60 and 20 nmol/mmol creatinine, respectively, after sleep and clonidine, the two in combination giving better suppression than sleep alone. Since urinary catecholamines can be determined relatively easily by high-pressure liquid chromatography with electrochemical detection, this test may be more widely applicable than suppression tests based on plasma measurements.

Serum cholinesterase in the mothers of neural tube defect progeny.

Human Serum Cholinesterase activity and polymorphism at its two gene loci, CHE1 and CHE2, were compared in maternal serum from neural tube defect pregnancies, normal pregnancies and a non-pregnant control group. Variants at the CHE1 locus were identified by dibucaine, fluoride and R02 0683 inhibition. The CHE2 phenotype was demonstrated by DISC polyacrylamide gel electrophoresis. Total HSChE activity in the pregnant groups was slightly less than in the control group but there was no difference in activity between the affected and the normal pregnancies. Three variants were identified. All were found in the non-affected pregnant group. One variant at CHE1 was identified, an I phenotype, and two C5+ phenotypes, the CHE2 variant. No obvious relationships were found between HSChE activity or a particular genetic variant and NTD progeny.

Determination of cyanide and thiocyanate in biological fluids by gas chromatography-mass spectrometry.

A sensitive, specific and accurate method is described for the determination of cyanide and thiocyanate in biological fluids. Potassium cyanide containing 95% 15N is added to the sample as internal standard and both endogenous cyanide and standard are separated by gas-phase diffusion into sodium hydroxide solution. Cyanogen chloride is prepared by reaction with sodium p-toluenesulphonchloramide and extracted into n-heptane. Quantitative analysis is carried out by gas chromatography-mass spectrometry (GC-MS) with selective ion monitoring of the molecular ions at m/e 61and m/e 62. Thiocyanate levels are determined by a similar procedure in which 15N-labelled sodium thiocyanate is used as internal standard. The method is applicable to a wide range of biological samples and is free from interference by other sample components.

Relationship between crystallin mRNA expression in retina cells and their capacity to re-differentiate into lens cells.

Differentiating vertebrate embryo tissues pass through a stage when they have the potential for several pathways of differentiation, some of them revealed only by experimental manoeuvres. As differentiation proceeds, the potential for alternative pathways becomes progressively restricted and is eventually lost. We have investigated the molecular basis for this phenomenon, using the chick embryo retina, which has a potential for differentiation into lens cells. This potential requires special conditions for its manifestation; it declines during development and is finally lost before hatching. Lens protein (crystallin) mRNA is expressed in early embryo retina: its levels decline in parallel with the decline in lens-forming potential. There is also evidence of an ontogenic change in post-transcriptional regulation. Such a relationship between heterologous mRNA and heterologous differentiation potential may be of general application.

Cyanide exposure in fires.

The toxic hazard from cyanide produced in fires was assessed in casualties and in firemen exposed to the fire atmosphere. The levels of cyanide and its principal metabolite, thiocyanate, were measured in blood samples from fireman, non-fetal and fatal casualties, and from controls. Although firemen did not differ significantly from controls, casualities showed significant elevation of blood-cyanide, and in a small proportion of fatalities blood-cyanide reached toxic levels.
