Mosaicplasty for the treatment of articular cartilage defects: application in clinical practice.

This article describes a one-step operative technique for the treatment of circumscript cartilage defects of weight-bearing surfaces of the knee. Since 1992, a total of 227 patients were treated using this technique for full-thickness lesions resulting from chondropathy, traumatic chondral defects, and osteochondritis dissecans; the procedure was evaluated in 57 patients who had > 3 years of follow-up. Magnetic resonance imaging, computed tomography arthrographies, ultrasound, and arthroscopy were used to evaluate the technique. Using the modified Hospital for Special Surgery (HSS) knee scoring system, 91% of the patients achieved a good or excellent result. The operative technique, clinical results, and complications are detailed.

Comparison of results of different types of knee arthroplasties.

We summarise our experience gained with knee arthroplasties over 18 years. Between 1976 and 1994 1103 knee arthroplasties (1044 primary cases, 59 revisions) were performed at the Orthopaedic Department of the National Institute of Rheumatology and Physiotherapy in Budapest, Hungary. The diagnoses were osteoarthritis (OA) in 50.9%, rheumatoid arthritis (RA) in 40.7% and other causes in 8.4%. The average age of the patients at the time of the operation was 57.6 years (range 14-81 years). The types of primary implant were as follows: 60 constrained (hinge) prostheses, 876 unconstrained (sledge) prostheses, 108 semiconstrained (total condylar) prostheses. The mean follow-up period was 11.4 years for the hinge-type prostheses, 10.3 years for the unconstrained prostheses and 1.6 years for the semiconstrained prostheses. Reviewing the 59 revision cases, we conclude that complications with the constrained prostheses reached 17.8% and, most presented within the 1st year. Because of this high complication rate, the use of hinge prostheses has been reduced in this department to only selected cases. After sledge prosthesis implantation most of the complications (overall 5.3%) appeared after 1 year in aseptic circumstances. Given the short follow-up period of the semi-constrained total condylar knee replacement, apart from one infection in a patient with rheumatoid arthritis no other complication has been recorded. Summarising these data, it can be concluded that on average the knee function, using a standardised scoring system, improved from 38% to over 80% by introducing the semiconstrained total condylar knee prosthesis.

Arthroscopic autogenous osteochondral mosaicplasty for the treatment of femoral condylar articular defects. A preliminary report.

An arthroscopic technique for the use of mosaicplasty osteochondral grafting in the treatment of femoral condylar articular defects is described. During the procedure, small cylindrical grafts are harvested from the nonweightbearing periphery of the patellofemoral joint and implanted in mosaiclike fashion into the focal cartilage defect of the weightbearing condylar surface. This report describes the arthroscopic technique using newly designed instruments and the early results of 44 patients treated by this method. Good to excellent results have been obtained based on follow-up ranging from 1 to 5 years. The modified HSS scores are compared with a control group consisting of patients treated by abrasion arthroplasty. Evaluation was done clinically and, in some cases, by ultrasound, magnetic resonance imaging, 3-dimensional computed tomography. The results of 10 control arthroscopies and the histological evaluations of 6 biopsies demonstrated the hyalinelike cartilage surface at the recipient area and the fibrocartilage formation at the surface of the donor holes.

[Orthopedic surgery in patients with rheumatoid arthritis].

Studying their subject the authors have been found that among the orthopedic surgical interventions the most frequently applied procedure were the alloarthroplasty and the synovectomy. Half of the operations are performed on the hand and the knee. Because of the polyarticular and multilocular characteristic of the disease very often more than one operations are necessary in case of rheumatoid patients. In case of sequence operations attention is called constant communication with rheumatologist and individual assessment of each case.

[Management of early avascular femur head necrosis by implantation of a muscle-bone graft].

Authors describe the results of 10 patients operated for avascular necrosis of the femoral head. The first operation was performed in October 1987. The follow-up was in average 41.9 (24-54) months, minimally 2 years. 7 patients improved, 2 deteriorated, 1 was unchanged. The analysis of the results showed that this operation is indicated only in strictly stated conditions.

[Management of early avascular femur head necrosis by flexion osteotomy. 2-year follow up].

Authors report on the results of treatment in 19 patients, operated for avascular necrosis of the femoral head. The first operation was performed in October, 1989. The average follow-up was 28 (24-42) months, the minimal follow-up 2 years. The state of one patient grew worse, one was unchanged, the rest improved. Operation in early cases is suggested if less than 50 per cent of the femoral head is attainted.

[A newly developed cementless elbow joint prosthesis].

Authors describe briefly the history of the development of elbow arthroplasty. They report on their experiences with the silicone, cementless, unconstrained elbow prosthesis developed by them and implanted in 23 cases during 3 years. The prosthesis is described, the operative indication and the problems of the operative technique are reviewed. On the basis of their results and the low number of complications this method of operation is suggested first of all for the successful treatment of elbows with rheumatoid arthritis.

[Knee arthroplasty with total condylar endoprosthesis].

Authors report on experiences gained with the Total Condylar knee prosthesis, representing the third generation in the development of the knee endoprosthesis. Based on results of 17 operations it is stressed that after the implantation in the severely degenerated or destroyed knee joint they succeeded to reach a significant decrease of pain and increase of the range of movement. After the implantation of the prosthesis adequate stability can be gained in knee joints without cruciate ligaments.

[Low contact stress knee prosthesis system. Results and complications].

This paper reports on the global results of the low contact stress knee prosthesis system with mobile weight bearing surfaces. The follow-up result of 918 cemented and 963 cementless knee prostheses was in more than 90 per cent excellent and good 2-8 years after the operation. The ratio of complications connected with the construction decreased to 2.8 per cent in cementless prostheses. In the cemented series less variations of size were available and the problems with the tibial component were under 2, the complications with the patella remained under 1 per cent. The more than 12 years clinical use of the mobile weight bearing knee prosthesis system proved the success of the solution concerning the basic construction.

[Transplantation of a bone with muscle pedicle in a case of pathological fracture of the femur neck].

Authors performed an operation for danger of fracture in case of extensive cystic fibrous dysplasia, localized in the femoral neck. The cyst was filled after refreshing its walls, with cancellous bone. To increase ossification a cortico-spongious bone cube, gained from the trochanter major and with retained connection with the insertion of the anterior third of the medial gluteus muscle was inserted in the area filled. The transplanted bone was fixed with an autologous fibular graft. Half a year after the operation bony transformation and good functional result could be observed.

[Role of orthopedic surgery in the management of rheumatoid arthritis].

After describing a few pathological and clinical characteristics of the rheumatoid arthritis author outlines some of the cares connected with the operative treatment of these, in the majority severe cases. The more frequently used operations and their results to be expected are described.

Surgical solution of femoral fibrosarcoma. Experience obtained by a 9-year follow-up.

Biopsy followed by tumour removal and replacement by autologous bone tumour at the distal end of the femur were performed. Histologically, the tumour proved to be benign. Three years later progression was observed and epiphyseodiaphyseal resection was made because of on excessive tumour. Histology revealed a fibrosarcoma. Consolidation was hindered by osteomyelitis and fracture, recovery could be achieved only by a series of operations. These histological and operative difficulties are documented and the importance of extremity-saving operations is discussed. Our patient has been free of complaints 9 years after epiphyseodiaphyseal reseaction.

Arthroplasty combined with cup-plastics in the surgical treatment of congenital dislocation of the hip.

The authors review the arthroplasty combined with cup-plastics in the treatment of congenital dislocation of the hip. The indications and surgical exploration are discussed in detail. Good results are reported. The complications and their solutions are also described. In the recent five years, 100 patients were operated by them. Based on their experiences, they suggest this intervention at an earlier age, too (i.e. 30 to 40 years). Moreover, they believe it to be an important possibility to provide an adequate rehabilitation for patients being still of working age.
