[A case of chromomycosis in a nonendemic region].

A 43-year-old miner of the Donetsk coal fields is described, who developed in a month after a microinjury of the skin of the thoracic left side an intumescence and skin reddening. Later an agglomeration of nodules formed there with small ulcers regularly appearing, that did not heal for a long time. Histologic and bacteriologic analysis confirmed chromomycosis. The disease focus was resected. No recurrences were recorded in the following 8 months. This observation points to a wider prevalence of chromomycosis in a nonendemic area, the European part of the USSR, than it is reported in literature.

[Obstetric and neonatal complications in patients with various clinical types and morphologic forms of primary chronic glomerulonephritis].

Clinical progress and outcomes of 151 pregnancies have been evaluated in 123 patients with chronic primary glomerulonephritis (GN). The incidence of gestational and neonatal complications was evaluated for a relationship with preconception findings of proteinuria, hematuria, arterial pressure, blood creatinine and, in 45 women, histological patterns of disease. Major preconception proteinuria (over 3 g/day), hypertension, decreased renal function and especially clustering of these findings were prognostically poor. Gestational progress and outcomes in mesangiocapillary GN were worse as compared with mesangial proliferative GN.

[Stomach cancer with tumor endocrine cells in the presence of chronic ulcer].

A case of stomach carcinoma was examined in a 45-year-old male by using histochemical and electron microscopic techniques. The carcinoma had arisen from chronic gastric ulcer accompanied with fundic gland hyperplasia and located in the body of the stomach. Histologically, it was a nondifferentiated scirrhous carcinoma involving signet ring and minor polymorphic cells with eosinophilic cytoplasm. Electron microscopically, the tumors had cells containing a large body of mucoidal granules, great quantities of cells having various endocrine granules, and cells showing mixed endocrine-exocrine secretion. It was suggested that there might be a pathogenetic relationship of fundal cell hyperplasia and signet ring cell carcinoma development, on the one hand, and the existence of tumor endocrine cells, on the other.

[Morphoenzymatic changes of the optic nerve in cardiovascular diseases].

Results are described of a histological and histoenzymatic investigation of visual nerves with a history during life of acute vascular optic neuropathy. It was found that in vascular pathology of the visual nerve due to atherosclerosis, both in the involved and clinically healthy eye different degrees of visual nerve atrophy develop due to sclerotic changes in the blood vessels. The genesis of vascular lesions and atrophy of the visual nerve is to a certain degree determined by immunological changes as evidenced by infiltration of the tissues around the vessels and in the nerve fibers by immunocompetent cells with a pronounced activity of unspecific esterase and acid phosphatase.

[Clinical aspects and treatment of primary focal segmental glomerulosclerosis and hyalinosis].

The authors studied the clinico-laboratory values and efficacy of pathogenetic treatment in 8 patients with primary segmental glomerulosclerosis and hyalinosis (PSGH). The disease is characterized by distinct disorders in the T-system immunity. Glucocorticoid treatment did not essentially effect the course of PSGH. Cyclophosphane treatment allowed to achieve essential stable clinical improvement in certain patients.

[Clinico-morphologic comparisons in primary diffuse mesangio- proliferative glomerulonephritis].

Data are reported of a clinical and morphological including electron microscopical study of 132 patients with primary diffuse mesangioproliferative glomerulonephritis (DMPGN). A comparison of morphological signs using methods of non-parametric statistics revealed a complex dependence between glomerulosclerosis and sclerosis of the stroma, tubular atrophy and dystrophy; sclerosis of stroma and its cellular infiltration. Comparison of morphological aspects and clinical manifestations of DMPGN allowed to establish that development of disorders of partial functions of the kidneys has a complex genesis as evidenced by dependence of disorders on the presence and severity of glomerulosclerosis, stroma sclerosis, tubular atrophy and frequency of crescents/microcrescents.

[Morphologic evaluation of the effectiveness of local immune reactions in gastric cancer].

Upon the analysis of 50 cases of cancer of the stomach, the authors compared the size and composition of stromal cellular infiltrate, the size of active vascular bed in the tumor, mitotic activity of the tumor cells, the severity of regression in adenocarcinomas and signet ring cell polymorphocellular carcinoma of the stomach. The authors established the necessity of a comprehensive investigation of the tumor parenchyma and specificities of parenchymal blood supply for determination of local immune response in the tumor in relation to its efficiency.

[Hyperuricemia as a risk factor of nephropathy in gout].

Among 196 patients with primary gout examined in hospital or earlier stable or transient hyperuricemia was noted in 160 (81.6%). These patients were entered into the study group. The control group included 36 persons in whom the level of blood uric acid did not exceed normal values. The familial pattern of disease was established in the patients of the study group. Urolithic, proteinuric and hypertensive types of nephropathy as well as chronic renal insufficiency were more frequently observed in hyperuricemia patients. Alongside with severe tubular, interstitial and vascular changes, glomeruli in the form of mesangioproliferative or mesangiocapillary glomerulonephritis were regularly involved in the pathological process. In the rest of the patients vascular lesions were less marked and less frequent, renal glomerular changes reminded the picture of mesangioproliferative glomerulonephritis only; urolithiasis in them took a more favorable course. Thus, a high level of blood uric acid is one of the risk factors of renal affection in gout determining in many ways prognosis of disease.

[Parenchymatous-stromal interrelations in tumors and methods for their study].

The major components of tumor stroma and parenchyma are described and their reciprocal influences are demonstrated. A scheme of parenchyma-stroma relationships is proposed. Knowledge of how the stromal and parenchymal components of a tumor relate to one another is of considerable diagnostic and prognostic value, for it leads to a better understanding of the nature of tumor growth and of the tumor-host relationships.

[Primary mesangiocapillary glomerulonephritis].

The etiology and clinical and histologic features of primary mesangiocapillary glomerulonephritis (PMCGN) are reviewed, as are its management and prognosis. The problem of PMCGN, which is a well-defined disease entity, is shown to be one of major concern for nephrology today. A number of issues concerning the etiology, natural history, morphology, and treatment of this disease remain unresolved or require closer study.
