[Analgesics-induced chronic renal failure in patients on dialysis therapy in Hungary].

In recent years elaboration of the diagnosis of analgesic-nephropathy (ANP) with the help of imaging techniques significantly increased the possibility of diagnosing the disease. Therefore, evaluating the prevalence of ANP has become more accurate in our country as well. The prevalence of ANP has been investigated in patients who have newly been taken into the dialysis program due to renal disease of unknown aetiology in 22 dialysis centers between December 1994-December 1997. The diagnosis of ANP has been based on revealing chronic drug abuse in the history and positive results of renal imaging (decrease in length of both kidneys combined with either bumpy contours and/or papillary calcification). Among 284 patients dialysed with unknown diagnosis 42 (14.8% of all cases) proved to have ANP. All patients except 2 took analgesic mixtures containing phenacetin/paracetamol, phenason derivatives, acetilsalysilic acid, caffeine and/or codeine. According to their investigations, ANP is a common disease resulting in end-stage renal failure in Hungary as well.

Body composition in hypertensive elderly and middle-aged patients.

The occurrence of hypertension, leading to various life-threatening complications in the elderly is a widely recognized problem. The changes of body composition were determined in 120 control and untreated hypertensive subjects of various ages. In middle-aged hypertensive males, the total blood volume and plasma volume increased significantly compared to those of the healthy controls, while this increase was not significant in the case of hypertensive middle-aged females. In contrast, in the elderly hypertensive male subjects, the volume of all fluid components decreased, except the total body fat and the vascular volumes. There was a slight, statistically non-significant, increase in all the vascular volumes except in the red cell mass. The elderly hypertensive females showed the same tendency compared to the healthy controls of the same age. The vascular compartments seem to be decreased in elderly males, compared to those of the middle-aged males, while slightly increased in elderly females.

[Management of life threatening diuretic-resistant ascites by ultrafiltration and reinfusion of the ascitic fluid after dialysis].

The case of a young patient is described who suffered from hepatic cirrhosis with vascular decompensation. In earlier hospital treatment the ascites was resistant to diuretics. After a 2-month-long unsuccessful conservative therapy owing to the perilous state the ascites was reinfused in the circulation after ultrafiltration and dialysis. Remission was reached after 19 treatments and the patient is after 8 months still in balanced condition with the oral administration of diuretics.

Blood laboratory parameters of carefully selected healthy elderly people.

Hematological and biochemical parameters were measured in carefully selected healthy elderly (50 males and 50 females, aged 60-97 years) and young (10 males and 10 females, aged 18-23 years) populations. Most of the parameters measured did not show any age-related variations. It means that any deviation from the present normal ranges established for healthy young subjects should be considered as pathologic in the case of elderly, too. Some parameters showed age-specific changes and hence there is a possibility that their actual normal ranges might be extended towards the upper limit (globulin, IgG, IgA, CIC, C3, BUN, AP for females), or towards the lower limit (creatinine clearance, albumin). Verification of these modifications may indicate that a lot of unnecessary investigations in the case of elderly could be avoided.

Alterations of the FMLP-induced Ca2+ efflux from human monocytes with aging.

The Ca2+ with the calmodulin system plays a major role in the receptor-induced intracellular biochemical events. Thus, as in our previous studies, we found an altered postreceptorial coupling in the case of elderly, we were interested to elucidate the Ca2+ transport in the case of healthy young (less than 25 years) and aged (greater than 65 years) subjects. The chemotactic peptide FMLP was used for receptor stimulation. Our data show that both spontaneous and FMLP-triggered Ca2+ extrusion measured on monocyte monolayers is decreased with aging. The FMLP-triggered Ca2+ extrusion nevertheless did not occur through the TFP-sensitive pathway. These alterations of Ca2+ transport seem to indicate an impaired Ca2+-calmodulin function in monocytes with aging.

Age-dependent variations of intralysosomal enzyme release from human PMN leukocytes under various stimuli.

The intralysosomal beta glucuronidase and elastase release from polymorphonuclear leukocytes (PMNL) of young and aged male subjects were determined after 60-min incubation with 10 micrograms/ml Cytochalasin B (CB), 10(-6) M of Ca ionophore A 23187 and various concentrations of human low density lipoprotein (LDL). The beta glucoronidase secretion was triggered by both A 23187 and LDL; however, no significant differences were found between the enzyme release from PMNLs of young and aged subjects. In contrast, a marked elastase release was triggered in the young group only by LDL, whereas in the aged group, all of the applied drugs induced a significant elastase release. LDL caused the most dramatic enzyme release from PMNLs of aged males. It was concluded that the release of PMNL-elastase after LDL incorporation as well as by CB and Ca ionophore stimulation may be an age-related process.

Age related variations of some polymorphonuclear leukocyte functions.

The age-related alterations of some biochemical processes in polymorphonuclear leukocytes (PMNL) obtained from 20 healthy aged male and 20 healthy aged female (age: 60-94 years) subjects were investigated. The basic level of luminol dependent chemiluminescence (LDCL) was increased, whereas the basal value of reduced/oxidized glutathione (GSH per GS-SG) was decreased in the aged groups. The enhancement of LDCL was more significant by PMNLs of aged males than of females. The change of both GSH and GS-SG levels during phagocytosis were diminished in the PMNLs of aged subjects. The L-alanine beta-naphthylamide specific elastase like protease (ELP) activity measured in living cell suspension was markedly increased with aging in male subjects. Therefore, it was concluded that the aging of PMNLs is partly a sex related process. The intensive release of both beta-glucuronidase (beta-G) and ELP by the PMNLs of aged subjects after in vitro treatment with calcium ionophore A23187, Cytochalasin B or low density lipoprotein (LDL) suggests that they could play an important role in some age-related disorders.

Body composition in elderly people. I. Determination of body composition by multiisotope method and the elimination kinetics of these isotopes in healthy elderly subjects.

The body composition of 40 healthy elderly and 20 healthy young subjects was determined and compared. It was found that the age-related changes were more marked in females than in males. Total body water was decreased as a consequence of diminished lean body mass. Extracellular fluid volume remained unchanged, while plasma volume increased. Thus, the liquid components of lean body mass were redistributed between the intracellular and extracellular spaces. The elimination kinetics of isotopes were not significantly different between young and aged subjects, except the elimination of radiosulfate.

Body composition in elderly people. II. Comparison of measured and predicted body composition in healthy elderly subjects.

The predictive values of the equations of Moore et al. and Hume and Weyers and the measured values of body composition were compared in healthy, carefully selected aged and young subjects of both sexes. In the case of young subjects, these prediction equations seem to be adequate, even if this could not be affirmed with certainty by our results. The equations of Moore et al. were found not to be adequate for elderly males, while they were only suitable for the determination of intracellular components in elderly females.

Age-dependent alterations of Fc gamma receptor-mediated effector functions of human polymorphonuclear leucocytes.

Changes in the effector functions in polymorphonuclear leucocytes (PMNL), harvested from blood of young and aged healthy subjects of both sexes, were studied. FC gamma-receptor (Fc gamma R)-mediated incorporation of IgG coated 51Cr-HRBC significantly increased in the aged male group, while the phagocytosis of pre-opsonized fungi (Saccharomyces cerevisiae and Candida albicans) was independent of both the age and sex. However, the intracellular killing capacity of neutrophils obtained from aged male subjects significantly decreased toward 51Cr-labelled c. albicans. The antibody-dependent cellular cytotoxicity (ADCC) was also impaired with ageing in both sexes. The age-dependent decrease in the effector functions of PMNL may be explained, among others, by the fact that during yeast cell incorporation the increased cAMP level does not return to the basic level in the old group. On the other hand, the cGMP level which increased in PMNL of aged subjects does not show any progressive increase as in the young subjects, but remains unchanged. The oxidative metabolism producing free radicals being necessary for the effective intracellular killing and ADCC diminished in PMNL of aged subjects of both sexes. The above findings indicate that the adaptation of cyclic nucleotide system and the oxidative burst to the cell activation becomes impaired with ageing.

Complex arrhythmia analysis in uraemic haemodialysed patients.

Impulse formation and conduction disturbances of the heart were investigated in 23 uraemic patients with transvenous and oesophageal electrodes. Utilizing regular atrial pacing the sinoatrial conduction time (SACT) was obtained and sinus automaticity was evaluated by measurement of the sinus node recovery time (SNRT). The atrial effective refractory period and the Wenckebach point were also determined. The effects of regular haemodialysis treatment on impulse formation and propagation disorders caused by uraemia were observed. In 16 patients transitory or permanent cardiac stimulation had to be carried out because of 2nd and 3rd degree AV block. No complication was observed during simultaneous haemodialysis and pacemaker treatment. It is concluded that the non-invasive, simple bedside oesophageal atrial stimulation method is a good alternative and should be used in the exact diagnosis of heart conduction disturbances of haemodialysed patients.

Interrelation between body composition and endocrine system in healthy elderly people.

The body composition of 40 healthy aged and 20 healthy young people was determined using a multi-isotope method. At the same time their hormonal status was assessed. A correlation between the body composition and the endocrine status was investigated. Our results concerning the body composition and the hormonal values correlate with the findings of the literature. A strong correlation was found between the plasma volume and the renin-angiotensin-aldosterone system. The interrelation could explain the reciprocal age related changes of plasma volume and the renin-angiotensin-aldosterone system. No or weak correlation could be found between the body composition and other measured hormones.

Age-dependent changes of the Fc gamma-receptor-mediated functions of human monocytes.

The age-dependent alterations of phagocytosis and age-dependent cell change (ADCC) activity through Fc gamma receptors of human monocyte monolayers using anti-D human IgG-coated 51Cr-labeled human red blood cells were determined. 40 healthy aged subjects of both sexes (age: 60-85 years) and 20 healthy young people of both sexes (age: 18-25 years) were studied. The phagocytosis increased significantly with aging in both sexes, whereas a decrease in ADCC activity was found.

Lithium carbonate in the management of intractable congestive heart failure.

Lithium carbonate was administered to 11 patients with advanced congestive heart failure resistant to the usual forms of therapy. Abundant diuresis, consecutive regression of oedema, loss of weight and clinical improvement ensued in response to lithium therapy. Doses ensuring a serum level of 0.5 to 1.0 mmol/l are regarded as optimal in heart disease. Within this range lithium produces no clinical or ECG signs of cardiotoxicity, provided the serum Na and K levels remain normal. The side-effects observed are reported in detail and attention is drawn to the necessity of follow-up and especially of checking the serum lithium level.
