Reduction of circulating HIV antigens in seropositive patients after treatment with AL-721.

Sixteen patients seropositive for human immunodeficiency virus (HIV) at different stages participated in an open trial using the lipid mixture AL-721. Seven patients were antigen negative and asymptomatic and the other nine antigen positive, three of whom were virtually asymptomatic. AL-721 was given per os once a day at a dose of 10 g in a fat-free breakfast for up to 16 months. The presence of HIV antigens in the serum was monitored by enzyme immunoassay (EIA) using two commercial kits. In five of the nine antigen-positive patients the concentration of HIV antigens was reduced to basal level after about 3 months of AL-721 treatment; three of these responders were asymptomatic, one had persistent generalized lymphadenopathy, and the other had Kaposi's sarcoma. In one of the antigen-positive patients who did not respond to AL-721, the addition of AZT to the treatment resulted in a marked decrease in antigen level. Another AIDS patient receiving this combination therapy from onset of the trial showed a similar decrease. The other two nonresponding patients were at a stage of full-blown AIDS. These results suggest that AL-721, an innocuous compound, may be effective in reducing the serum HIV level in patients at the preclinical stage.

Blurred vision: an overlooked initial presenting symptom of insulin-dependent diabetes mellitus.

Blurred vision, a well-known complication of uncontrolled diabetes mellitus, is occasionally overlooked as the first and only presenting symptom of type I diabetes mellitus. In this report we describe six type I diabetic patients whose first and only symptom of the disease was blurred vision, as documented by increased glycosylated hemoglobin. Institution of therapy was delayed until the classical symptoms of diabetes mellitus appeared.

Allergic blepharoconjunctivitis due to phenylephrine.

Acute allergic blepharoconjunctivitis with mucopurulent discharge due to phenylephrine eye drops is reported. The reaction begins 3-4 hours after drug application, persists for 12 hours and regresses gradually within 72 hours. This clinical picture occurs with a phenylephrine concentration as low as 0.1%. Conjunctival biopsy of the inflamed eyes reveals marked infiltration by lymphocytes, plasma cells and eosinophiles, without predominance of any of these cells.

Bullous eruption in a case of bullous pediculid.

We present a case of a bullous eruption appearing in a patient with pediculosis pubis. The eruption was apparently a reaction to the pediculi, possibly to their salivary secretions. To our knowledge, this is the first report of such a case. We have called this condition "bullous pediculid."

Computed tomographic air enema technique to demonstrate colonic neoplasms.

The use of intravenous glucagon and the judicious introduction of air into a clean colon offer additional help in the confirmation of the larger intracolonic tumors on computed tomographic examination. The procedure is simple and may aid in the differentiation of an intrinsic colonic tumor from other pathologic conditions, particularly those that are extracolonic in origin.

Demonstration of antispermatozoal antibodies in varicocele-related infertility with an enzyme-linked immunosorbent assay (ELISA).

To assess the existence of a possible immunologic factor in varicocele-associated infertility, we searched for antispermatozoal antibodies in serum, seminal plasma, and bound to spermatozoa in 32 infertile men with varicocele and 22 infertile patients without palpable varicocele, with the use of an enzyme-linked immunosorbent assay. In addition, we performed morphologic and microbiologic analyses of the semen and urethral smears for isolation of Chlamydia trachomatis. Twenty-nine men from the varicocele group (90.6%) demonstrated antispermatozoal antibodies, compared with only 9 men (40.9%) in the control group. The antibodies in both groups, when present, were mainly serum and seminal plasma immunoglobulins IgA and IgM. A significant quantitative difference between the varicocele and control groups was also observed for serum IgA, seminal plasma IgA and IgM, and sperm-bound IgG, IgA, and IgM. Oligozoospermia and asthenozoospermia were significantly more prevalent in the varicocele men. An asymptomatic genital tract infection with C. trachomatis, Ureaplasma urealyticum, and Escherichia coli was traced in 40.6% of the varicocele men and in 45.5% of the control group. No interaction could be demonstrated between the infection and antispermatozoal antibody formation. These data suggest that an immunologic factor may play a role in varicocele-associated infertility; however, its impact on reproduction has yet to be assessed.

Antibody-dependent cell-mediated cytotoxicity and phagocytosis of autologous red blood cells in alphamethyldopa-induced haemolysis.

7 alphamethyldopa (AMD)-treated patients with positive direct antiglobulin test (DAT) were investigated. Peripheral blood mononuclear phagocytes of 2 patients suffering from haemolysis caused lysis and phagocytosis of autologous DAT-positive red blood cells (RBC). Eluates from RBC of both patients contained antibodies of IgG1 subclass and supported antibody-dependent cell-mediated cytotoxicity (ADCC) and antibody-dependent phagocytosis (ADPh) of the patients' RBC after remission. Both cytotoxic activities were proportional to the serum concentration and to the number of attacking cells. The 5 patients without overt haemolysis did not show in vitro lysis or phagocytosis of autologous RBC. These results suggest that ADCC as well as ADPh participate in the destruction of RBC in AMD-induced haemolysis in vivo.

Transmission of acquired immune deficiency syndrome (AIDS) by a blood transfusion given in 1979 in Israel.

AIDS developed in a blood donor-recipient pair in Israel. Transmission of HTLV III/LAV probably occurred via a blood transfusion in November 1979. The donor was asymptomatic at the time of blood donation, but initial symptoms of AIDS-related complex developed in the donor 3 months later and in the recipient 6 months following the blood transfusion.

Segregation of peripheral blood lymphocytes in sarcoidosis according to their affinity to insolubilized histamine.

Using a method based on the differential affinity of mononuclear cells to insolubilized histamine, we were able to distinguish several differences in the composition of T cell subpopulations between healthy control subjects and sarcoidosis patients. In 5 of 10 patients with sarcoidosis, the ability of T cells to respond to three mitogens was severely depressed, while in the remaining 5 it was similar to the response of normal subjects. After the elimination of a subpopulation of peripheral blood mononuclear cells (PBMC) that adhered to the histamine column, the response to mitogens of the unbound cells of normal control subjects was significantly lower than values obtained before separation. The histamine-unbound lymphocytes of the five patients with active sarcoidosis, whose unseparated PBMC responded poorly to mitogens, reacted significantly better to the mitogens than before the separation. This pattern of responsiveness has been previously only in PBMC of patients with autoimmune diseases. It seems that some patients with sarcoidosis possess a subpopulation of T-lymphocytes with suppressor activity and a specific affinity to insolubilized histamine, the elimination of which restores the level of response to almost normal.

Lichen myxedematosus (scleromyxedema) serum stimulates hyaluronic acid and prostaglandin E production by human fibroblasts.

Serum from a 45-year-old patient with scleromyxedema (withdrawn prior to therapy) was added to synovial and foreskin human fibroblast cultures and its effect compared to that of normal human serum. The patient's serum stimulated production of both hyaluronic acid and prostaglandin E by fibroblast cultures. This stimulation was inhibited by hydrocortisone and indomethacin and was not accompanied by stimulation of cell proliferation.

Assessment of immunological mechanism in infertility of the rat after experimental testicular torsion.

A necrotic torted testis left in the scrotum affects the contralateral testis, resulting in depressed fertility. It was suggested that testicular torsion provoked a systemic response of an immunologic nature. In order to prove this theory, 56 rats were operated. Ten had a sham operation, in 17 a testis was torted and excised after 24 h, and in 29 a testis was torted and excised after 24 h, and in 29 a testis was torted but left in situ till the sacrifice. Antisperm-antibodies were not found in any phase of the experiment in the sham group. Using indirect immunofluorescence, antisperm-antibodies were found in the sera of 8/17 rats of the orchiectomy group and in 24/29 of the detorted group. In the direct immunofluorescence study, all the antibodies were localized in the tail of the spermatozoid. The antisperm-antibodies disappeared after 3 to 6 months.
