Teardrop-femoral head distance after shelf acetabuloplasty for Perthes' disease.

Shelf acetabuloplasty is an accepted treatment for Perthes' disease. Favourable outcomes have been reported. This study aimed at determining whether changes in femoral head subluxation after shelf acetabuloplasty occur and whether they predict outcome. We assumed that reduction of subluxation after shelf acetabuloplasty would improve the long-term outcome. During follow-up, growth presented as a source of error when comparing the amount of absolute subluxation. Correction for growth was achieved by calculating a subluxation index. This index showed a significant reduction during follow-up. There was no correlation between clinical and radiological outcome, but the fact that the subluxation index showed a decreasing trend could be promising regarding outcome. This might be one of the reasons why shelf acetabuloplasty produces its effect. Following shelf acetabuloplasty the enlarged acetabulum could possibly direct the growth of the femoral head to a better-contained joint. This could result in better joint congruity and lesser tendency to degenerative arthritis.

The jaw index: new guide defining micrognathia in newborns.

OBJECTIVE: Newborns with glossoptosis due to micrognathia can suffer from life-threatening respiratory distress and severe feeding difficulties. These characteristic features are found in newborns with Pierre Robin sequence. In the relevant literature, no concensus concerning diagnosis for these children can be found. A guide defining micrognathia can be helpful in the diagnosis of glossoptosis and possible airway obstruction. METHOD: The jaw index is obtained in newborns by measuring three facial dimensions. In a series of 100 healthy neonates the standard measures were defined. RESULTS: The average jaw index in children with Pierre Robin sequence differs significantly from the reference group and was found to be over 3.6 times the normal value. CONCLUSIONS: Micrognathia can be defined with the jaw index.

The plantar arch revisited.

In 20 random cadaveric feet, the arterial plantar arch, its anastomosis with the medial plantar artery, and the communication between the lateral plantar and the dorsal arteries of the foot were studied. The anatomic variations found in this material are described and illustrated. The presence of a "classic" anastomosis between the medial and lateral plantar arteries was observed in only 20 percent of cases. The "classic" conjunction of the dorsal artery with the lateral plantar artery was found in 95 percent of cases. The authors conclude that most variations in the contributions to the plantar arch have not been previously reported, and these may have clinical consequences.
