[Disorders of sleep structure and autonomic regulation in sleep of patients with stroke].

Peculiarities of both the structure of night sleep and autonomic regulation in the sleep were studied in 30 patients with ischemic stroke of different location. Clinical, polysomnographic (including autonomic indices) and psychologic methods were used. Considerable alterations in the structure of night sleep were observed, namely: lengthening of the period of falling asleep, increase of the time of awakening inside the sleep, impossibility to support necessary duration of both stages and phases of sleep. The indices of cardiovascular system's activity such as heart rate (HR) and arterial pressure (AP) were altered most of all. The most dramatic disorders of both the structure of sleep and autonomic regulation in the sleep were found in right hemispheric and medial location of ischemic stroke. High values of HR and AP in the sleep (especially in phase of the fast sleep), high HR and AP variability as well as the decrease of these indices from the first cycle of the sleep to the third one were prognostically unfavourable factors.

[Treatment of fibromyalgia].

The paper reports the results of therapy of 23 patients with fibromyalgia (FM). Tetracyclic antidepressant lerivon, was administered to group 1, nonsteroid antiinflammatory (NSAI) preparation nurofen to group 2 and phototherapy (exposure to bright white light) was used in group 3. Clinical effect in the form of a decrease of both the intensivity of algesic syndrome and autonomic manifestations as well as improvement of night sleep were clearly seen in group 1. Manifestations of both anxious and depressive disorders were less pronounced. Treatment by Nurofen resulted in slight decrease of intensivity of pains but didn't lead to pronounced alterations of emotional sphere. Administration of either Lerivon or Nurofen promoted the increase of pain thresholds (according to the data of nociceptive flexory reflex). The data obtained testified the necessity of complex therapy of FM patients including administration of antidepressants and analgetic drugs of NSAI group. Dynamic polysomnographic examination of patients from group 3 revealed the increase of total sleep duration, decrease of the time of falling asleep, the latent period of the phase of the fast sleep, activated movement index, intensivity of movements and the time of being awake in the sleep. The conclusion was made that it was worth while to use phototherapy as alternative, nonmedicine method of phothotherapy.

[The characteristics of the structure of night sleep in patients who have had a stroke].

The aim of the study was investigation of the structure of the night sleep in relation to the disease stage and location of the destructive focus. 18 patients with ischemic stroke (in the right hemisphere-8 cases, in the left one-6 persons, with brain stem location-4 patients) were observed as well as 5 practically healthy individuals. The diagnosis was verified by computer tomography in all cases. Clinical neurological investigation and polygraphy of night sleep (electroencephalography, electrooculography, electromyography) were performed. Analysis of sleep parameters was carried out according to program elaborated by the Center of Sleep Research of Sechenov Moscow Medical Academy. This program includes the analysis of segmental sleep structure in addition to standard parameters. The sleep quality was estimated by Sleep Index (Slind) according to the degree of intactness or destruction of its structure. Rude disorganization of night sleep structure was revealed in the patients, including disturbances of mechanisms of the whole sleep organisation, as well as generation and maintenance of its separate stages. The most rude disorders of sleep in patients with ischemic stroke were connected with location of ischemic focus in right hemisphere and in medial, deep structures.

["Music of the Brain" in the treatment of insomnia patients].

The effects of "Music of the Brain" which is the new nonpharmacological method of treatment of insomnia were studied. The method is based upon the transformation of EEG signals into the music by using a special algorithm developed by the author. Sleep polygramme was registered and analysed. EEG sites of sleep stages and phases which most satisfied usual criteria were selected and transformed into music. The patient listened to the recorded audio cassette just before sleep. 58 patients with insomnia were divided into 2 groups according clinical, formal, psychological and electrophysiological (polysomnography, EEG) methods which were applied before and after 15-day course of treatment. 1st group (44 individuals) was the main group and the 2nd group (14 patients) was the "placebo" group ("Music of the Brain" of other individual was used in this case). The positive effects of "Music of the Brain" were evaluated in patients with insomnia in more than 80% cases. These effects were marked on the basis of both subjective feeling and objective (psychological and neurophysiological) results. The high effectivity of "Music of the Brain" for patients with insomnia combines with the lack of side effects and complications.

[The effect of Imovane on sleep structure and respiratory indices during sleep in insomnia patients].

Night sleep was examined in 120 patients with different forms of insomnia. The positive effect of Imovane was shown both on presomnic and intrasomnic disorders. Transitory side effects (dryness, bitter taste in the mouth) were in some cases that didn't need cessation of therapy. Polysomnographic studies confirmed effectivity of Imovane: there were observed either lengthening of the duration of sleep, its 2 stage, delta-sleep, or decrease of the duration of falling asleep. There were no respiratory changes after course of Imovane, meanwhile some indices positive dynamics was revealed in this case.

[Features of the sleep/wakefulness cycle in long-term isolation].

The sleep/wakefulness cycle was the subject of polysomonographic and actographic studies in two experiments with isolation and confinement of six volunteer operators for 135 and 90 days. The dynamics of sleep structure was the target parameter before, during, and after the experiments. The sleep structure dynamics and changes in the motor activity differed in their directionality. At all the stages of experiments the operators' sleep structure had some unique peculiarities. Discussion is centered around the cases of shortened duration of delta-sleep and altered latent periods of specific stages. During the off-nominal situation (ONS) with 45 hour sleep deprivation polysomnographs recorded episodes of latent sleep which became more multiple during the second ONS and were combined with signs of "slow" sleep, and increases in the index of motor activity during night sleep post ONS. Hence, adaptation to changed physical and social environments is manifested by a specific functional restructurization of the sleep/wakefulness cycle.

[The effect of Lendormin (brotizolam) on the sleep structure and subjective assessment of sleep in insomnia patients].

The influence of Lendormine (diazepine derivative) both on the sleep structure and subjective estimation of sleep was examined in 25 patients at the age of 19-64 years old with neurotic insomnia. The investigation was performed before, during and after 10 days of treatment (0.25 mg before the sleep time). The drug administration resulted in significant improvement of sleep structure that is in sleep duration and efficiency because the drug either promoted falling asleep or increased the second and the forth stages of sleep. Lendormine decreased also the wakening time inside the sleep. The polysomnographic results coincided completely with subjective estimation of sleep by patients themselves after the treatment. The optimal time of therapy didn't exceed 2 weeks. The drug was well tolerated without any complications. The conclusion is made about high effectiveness of Lendormine in the treatment of patients with insomnia.

[The phototherapy of parkinsonism patients].

40 patients with idiopathic parkinsonism were treated by artificial white light (intensity--3300 lux) together with total or partial "drugs holidays". The patients with newly made diagnosis "parkinsonism" were treated too. Both before and after light therapy (LT) (10 light exposures) we performed estimation of the main clinical manifestations of the disease (in rating scales), psychological assessment (Back's depression rating scale), examination of motility (authors' own special computer program "Hand's Movement Test for Windows"). It was observed that LT resulted in a decrease of the severity of either rigidity or bradykinesia (but not the tremor) as well as it also decreased the depression gravity and improved the motor functions. LT facilitated the patients' capacity to tolerate "the drugs holidays". LT permitted to combine it with drug therapy as well as LT decreased the complications of DOPA-containing drugs (on-off-effects and dystonic hyperkinesias). LT may be used as monotherapy in patients with initial manifestations of the disease. LT didn't cause any complications and was quite simple in carrying out. The conclusion is made about the perspectivity of LT application in parkinsonism treatment as important non-drug therapy.

[Effects of sleep deprivation in patients with parkinsonism].

The paper is concerned with a pilot detailed study of a single 24-h sleep deprivation (SD) in parkinsonism patients (n-35). The score estimation of the disease clinical signs demonstrated diminution of rigidity, bradykinesia, gait defects as well as of the total score of the patients condition severity. The clinical observations were verified by computer processing of motor tests. SD proved antidepressant in parkinsonism patients. The combination of drug treatment with SD permits a 2-fold and 50% decrease in the treatment duration and daily dose, respectively. A test is proposed for assessing tonicity of facial muscles.

[Sleep deprivation and brain pathology].

The paper presents a brief review of literature data on clinical effects of sleep deprivation (SD), short-term deprivation (up to 40 h), in particular. SD was studied for effects on patients with defected cerebral dopaminergic systems (parkinsonism, dystonic hyperkinesis, persistent lactorrhea-amenorrhea, acromegaly). The control was made using clinical, electrophysiological, psychological, motor and biochemical tests. SD was compared to L-Dopa, parlodel, lisenil action. SD is shown to have therapeutic and differential diagnostic significance in varying pathological conditions of cerebral dopaminergic systems.

[Autonomic disorders in Parkinson disease].

The authors provide the clinical and laboratory findings regarding the vegetative nervous system in 60 patients with parkinsonism and in 11 healthy volunteers. It has been found that parkinsonism is necessarily associated with vegetative disorders which are pronounced to a greater degree in right-handed patients and bilateral clinical manifestations of the syndrome.

[Lateralized forms of parkinsonism (clinical, electroencephalographic and psychomotor aspects)].

As many as 101 parkinsonism patients were examined. The mean patients' age was 50 years. Of these, 46 patients (12 with left-handed clinical manifestations of parkinsonism, 17 with right-handed, 17 with bilateral ones) were subjected to comprehensive examinations -- clinical, electrophysiological and psychomotor. Ten healthy persons of the respective age and sex made up the control group. The examination made it possible to reveal the clinical and electrophysiological characteristics of the different patterns of parkinsonism. At the same time it allowed the conclusion about the involvement into the pathological process of both hemispheres whatever the primary lateralization of the clinical manifestations; about varying roles played by the right and left hemispheres in the pathogenesis of parkinsonism.

[Psychosomatic and humoral correlations of persons in late middle age].

Comprehensive examination of psychoemotional personality characteristics, electropolygraphic examination during night sleep and in various states of day-time wakefulness, as well as determination of the circadian rhythm of urinary catecholamine excretion were carried out in elderly patients versus control subjects. A correlational analysis of the findings obtained revealed certain changes of the studied indicators in the elderly patients. Nocturnal sleep was associated with elevated functional activity of the ascending activating system of the brain and with increased noradrenaline excretion. In day-time wakefulness the dynamics of physiologic, psychologic and biochemical changes were of the unidirectional nature: a decrease in reactivity of alpha-index was logically combined with decrease in reactive anxiety and in noradrenaline and dopamine secretion.
