[Complex evaluation of the action of inhibitors of hydrochloric acid secretion on gastric function in ulcer disease].

AIM: To characterize actions of inhibitors of hydrochloric acid secretion on gastric function and to formulate indications to using different antisecretory drugs in ulcer disease (UD). MATERIAL AND METHODS: Of 120 examinees with duodenal ulcer (DU), 40 received gastrocepin, 40--famotidin, 40--omeprasol. Follow-up study was made of motor, acid-, pepsin- and mucus-forming functions of the stomach. RESULTS: By reducing production of acid and pepsin, gastrocepin is inferior to famotidin and omeprasol. Gastrocepin is more effective than famotidin and omeprasol in motor-evacuatory dysfunction. Gastrocepin improves defensive properties of gastric mucus better. CONCLUSION: The results expand indications for a differential administration of antisecretory drugs in UD.

[Use of essentiale and cholagogum in prevention of cholelithiasis in fat hepatosis with concomitant cholecystitis].

AIM: To assess efficacy of essentiale in combination with cholagogum in prevention of cholelithogenesis in patients with fat hepatosis (FH) and concomitant cholecystitis (CC). MATERIAL AND METHODS: 100 patients aged 31-60 years with FH and CC were examined using clinical, device and biochemical tests. The diagnosis was verified by the results of ultrasonography, computed and MR tomography. Fractionated duodenal tubing followed by biochemical examination of bile was conducted in all the patients. RESULTS: Changes in physico-chemical properties of hepatic and vesical bile corresponding to stage I of cholelithiasis occurred in most of the examinees. Treatment with essentiale +cholagogum is clinically effective, improves functional condition of bile ducts and the liver, diminishes bile lithogenicity. CONCLUSION: In fat hepatosis and concomitant cholecystitis, essentiale and cholagogum improve biochemical composition of bile, resulting finally in decreased risk of cholelithogenesis.

[Study of the functional state of the hepatobiliary system in the dynamics of treatment of patients with peptic ulcer].

There was a complex study of the functional condition of the liver and biliary system in 130 patients with peptic ulcer. Thus, 84.6% of the patients had some disorders of the hepatobiliary system. When antihelicobacterial therapy is used, the deterioration of some initially impaired functional parameters of the hepatobiliary system--reduction of the contractive function of the gallbladder, increased cholestasis and increased ALT and AST levels in the blood--are observed.

[Age-related comparative characteristics of ulcer disease course].

AIM: To study pathogenetic factors underlying development of ulcer disease and its course in different age periods. MATERIAL AND METHODS: Clinical, psychoemotional, endoscopic device, biochemical and radioimmunological examinations were used in the study of 283 patients with gastroduodenal ulcer. By age, the patients were divided into four groups: 15-18, 19-35, 36-60 and 61 and older. RESULTS: Some age-specific clinical and endoscopic characteristics were found in the examinees. Risk factors (hereditary predisposition, high gastric juice acidity, the presence of Helicobacter pylori, etc.) occurred almost with the same frequency at different ages. Older patients had deficient defense-protection properties of gastric mucus. Most of the examinees's psycho-emotional state was characterized by moderate reactive and high personality anxiety. In all the age groups reactions of the hormonal system were of the same type--hypophyseo-adrenal hyperfunction and hypophyseo-thyroid hypofunction. CONCLUSION: The above findings may help in design of differentiated by age therapeutic and diagnostic measures for ulcer patients of different age.

[Use of tykveol in the treatment of impaired biliary tract function in patients with chronic non-calculous cholecystitis].

The therapeutic efficacy of tykveol was evaluated in 22 patients with chronic non-calculous cholecystitis and/or dyskinesia of the biliary tract (BT) concurrent with gallbladder deformity (GD). Supplementation of tykveol to the combined therapy in the patients with chronic non-calculous cholecystitis concurrent with GD was shown to exert a pronounced therapeutic effect. This caused positive changes in clinical symptoms and BT function, diminished the lithogenic propertes of bile. With the use of tykveol, recovery of neurohumoral regulation is an important factor that improves biliary tract function, as evidenced by decreased gastrin secretion.

[Chewing gum Dirol effect upon the status of oral cavity and gastric acid secretion in patients with hypersecretion of hydrochloric acid].

The purpose of our research was to learn the efficacy of the Dirol chewing gum upon the condition of the oral cavity and the stomach acidity in patients with hyperchlorhydratic type of secretion. It has been estimated that correct usage of chewing gum by patients with hyperacidity of the stomach in a month after its administration decreases the level of gastric acidity to a normal one. Thus, it has been proved that the Dirol chewing gum favorably changes the condition of oral cavity both from hygienic point of view, acido-alkaline balance and saliva condition.

[Erosion of the gastroduodenal zone: an independent nosological form or an ulcer phase?].

We examined 215 patients, 98 of them being with gastric and duodenal erosions, 52 being with erosions of the gastroduodenal zone combined with duodenal ulcers, and 65 being with duodenal ulcers. Endoscopic, radiological, biochemical, pH measurement studies and abdominal gastroduodenal manometry were used for the evaluation of the functional state of the gastroduodenal system. It was established that impaired pyloric and cardiac functions, duodenogastric reflux, dyskinesia of the stomach and duodenum, stomach deformation and gastroduodenoptosis are observed more frequently in cases of erosive lesions as compared to ulcers. The obtained data allow us to assume that gastric and duodenal erosions are an independent nosological form with their own pathogenic mechanisms, clinical presentation and complications including ulcers.

[Therapeutic efficiency of a combined use of low-intensity laser radiation and actovegin in gastroduodenal ulcers with inhibited cicatrix formation].

AIM: To validate use of intravenous laser blood irradiation (ILBI) combined with actovegin administration in indolent gastroduodenal ulcers. MATERIAL AND METHODS: Modern endoscopic, morphological, device, biochemical techniques and radioimmunoassay were used in examination of 92 patients with indolent gastroduodenal ulcers aged 24 to 69 years. ILBI plus actovegin was given in failure of standard medicinal therapy. RESULTS: ILBI plus actovegin combination produced marked analgetic, anti-inflammatory and detoxication effects. Favourable trends were observed in the composition of gastric mucus, detoxication, reparative and metabolic processes in the gastroduodenal mucosa, neurohumoral regulation. CONCLUSION: Combination of ILBI with actovegin proved highly effective in indolent gastroduodenal ulcers.

[Dynamics of general and primary morbidity of digestive organs in the Udmurt Republic].

We studied the dynamics of morbidity of the digestive apparatus including peptic ulcer, gastritis, duodenitis and mortality in the Udmurt Republic from 1992 to 2002. We revealed the decrease of morbidity of the digestive apparatus and stable data for peptic ulcer but the increase of mortality from the digestive apparatus pathologies on the whole and from complications of peptic ulcer. The mortality from the digestive apparatus diseases and peptic ulcer is forecasted to increase in the next years if the same social and economic conditions are preserved and there are no changes in the health-care system.

[Characteristics of aggressive and protective factors in erosive lesions of gastroduodenal mucosa].

AIM: To investigate aggressive-protective factors in patients with erosive lesions of gastroduodenal mucosa. MATERIAL AND METHODS: Intragastric pH-metry, tests for Helicobacter pylori, lysozyme activity, content of bile acid in gastric juice, components of gastric mucous gel were made in 106 patients. RESULTS: It is shown that there are two alternative paths of erosions development in the gastroduodenal zone: infectious and non-infectious. CONCLUSION: In patients infected with Helicobacter pylori, lowering of a protective ability of the mucous gel is caused by prevalence of synthesis of immature mucous components while in non-infected patients this happens as a result of high catabolism of mucous components.

[The gastric mucigenous function in the treatment of patients with ulcer with various antisecretory preparations].

The investigation of the gastric mucigenous function in dynamics in the treatment by antisecretory medications was carried out in 120 patients with peptic ulcer disease. The selective Mi-cholinolytic gastrocepine, H2-blockers--Ranitidin and Famotidin, the proton pump inhibitor--Omeprasol, were used in the complex therapy. The mucigenous stomach function was investigated by the biochemical method at the level of sialic acids fractions, glycoproteins, glycosaminoglycans and proteins. It was shown that at the exacerbation of the ulcerative disease the expressed changes in gastric mucus were observed. It was shown, that Mi-cholinolytic gastrocepine has the most effective recovering activity on the gastric mucigenous function. The obtained results expand the possibilities of differential use of antisecretory preparations in ulcerative disease in connection with the gastric mucigenous function and acidity disorders.

[Use of essentiale with cholagogue in fatty liver complicated by cholecystitis for lowering the lithogenic properties of bile].

The group of 50 patients with the fatty hepatosis and an accompanying cholecystitis in the age from 22 till 60 years were examined alongside by clinical and some modern laboratory ultrasonic, biochemical researches. In portions "B" and "C" of the bile the level of cholic acids and cholesterol were investigated with the subsequent account of the cholate-cholesterol coefficient. It was shown that in the most of the patients with fatty hepatosis and an accompanying cholecystitis observes infringement of physical and chemical bile properties what correspond to 1st stage of the disease. The use of Essentiale with Cholagogue in the complex therapy at the pathology of hepato-biliary system renders the expressed therapeutic effect. Thus, alongside with the positive dynamic of clinical disease symptoms the bile lithogenic features are lowering.
