[High time to make choice: preventive or curative appendectomy?].

The authors advocate prophylactic appendectomy in conditions of wide application of laparoscopic surgery, refer to literature data on the absence of life-important functions in appendix and any functional disturbances after appendectomy. Taking into account high rate of development and difficulties in the diagnosis of acute appendicitis, favourable clinical and economical results of laparoscopic appendectomy, the authors suggest wider use of preventive removal of the appendix, especially in definite groups of population.

[Laparoscopy in the differential diagnosis and treatment of acute appendicitis and acute gynecological diseases].

Laparoscopy was used in differential diagnostics of acute appendicitis in 524 female patients. Acute gynecological diseases were diagnosed in 123-of them, inflammatory processes in appendages being most frequent. The laparoscopic technology allowed not only to avoid diagnostic errors in all the cases, but also to take all necessary surgical measures. The effectiveness of laparoscopic operations for acute gynecological diseases was confirmed by the absence of complications and recovery within short terms.

[Laparoscopic appendectomy in acute appendicitis].

Laparoscopic technology was applied as diagnostic method in 608 patients, 50 patients have undergone laparoscopic appendectomy. There were no diagnostic errors in laparoscopy application. Laparoscopic technology allowed to perform appendectomy quite safely and without any complications. This method permitted considerably reduce terms of treatment of the acute appendicitis patients and quickly restore their ability to work.

[Use of regional perfusion in the treatment of arterial occlusive diseases of the lower extremities].

The work presents an analysis of 120 isolated perfusions performed in 103 patients with obliterating injuries of the peripheral vessels of lower extremities. In 17 patients the perfusion was performed twice. For prophylactics of the commissural process the area of connection of main vessels of the artificial circulation apparatus (ACA) in 56 patients was wrapped up by teflon in order to ease reoperation. Good and satisfactory results were obtained in 79 (76.7%) of the patients which allowed them to return to their working activity. The application of the teflon sleeve at the level of connection of the ACA vessels was found to prevent the commissural process and to facilitate the access to the vessel in a repeated isolated perfusion.

[Experimental and clinical endophleboscopy].

A method of endoscopic estimation of the inner walls of large veins has been developed in the experiment and used in clinic. Fibroscope "Olympus" was used. Under observation was the dynamics and ECG in occlusion of the vena cava posterior in experiments on dogs. Eleven endophleboscopies of the vena cava inferior were performed in patients with acquired valvular disease.
