Comparison of the effects of on-pump versus off-pump coronary artery bypass surgery on serum prostate-specific antigen levels.

AIM: To compare the effects of coronary artery bypass operation with or without extracorporeal circulation on serum total prostate-specific antigen levels. METHODS: Seventy-six men with a mean age of 57.04+/-9.27 years (range 44-77 years), who underwent coronary artery bypass surgery were enrolled to the study. In 50 patients (Group I), coronary revascularization was performed using extracorporeal circulation, and in 26 patients (Group II) coronary bypass grafting was performed on the beating heart without using extracorporeal circulation. All the patients had serum total prostate-specific antigen levels measured preoperatively and twice postoperatively in the first and fifth postoperative days. Differences in mean total prostate-specific antigen levels between the two groups in the postoperative period were analysed. RESULTS: The mean preoperative total prostate-specific antigen levels in Group I and Group II were 1.28+/-1.13 ng/mL and 1.11+/-0.93 ng/mL, respectively, and there was no significant difference in the preoperative total prostate-specific antigen values between the two groups (P=0.519). In Group I, postoperative means were 4.96+/-6.29 ng/mL and 5.86+/-9.09 ng/mL in the first and fifth days, respectively (P=0.0001, P=0.0001). Total prostate-specific antigen means in the same postoperative period for Group II were 2.13+/-2.72 ng/mL and 2.00+/-2.20 ng/mL, respectively (P=0.014, P=0.024). The comparison of total postoperative prostate-specific antigen levels between the groups showed significantly higher elevation in Group I (postoperative day 1: P=0.013; day 5: P=0.05). CONCLUSIONS: Coronary revascularization can cause a statistically significant rise in serum total prostate-specific antigen levels. This rise is more marked in patients undergoing conventional coronary revascularization.

Bacterial etiology of acute otitis media and clinical efficacy of amoxicillin-clavulanate versus azithromycin.

BACKGROUND: Acute otitis media (AOM) is one of the most common acute bacterial infection in childhood and also the most frequent reason for outpatient antibiotic therapy. Little recent information about susceptibility patterns of AOM bacterial pathogens in Turkish children has been reported. OBJECTIVE: To determine the bacterial etiology of acute otitis media in children and to compare the efficiency of 3 days course of azithromycin with a 10 days course of amoxicillin-clavulanate. METHODS: This prospective, single blind, randomised comparative study was carried out in 180 children with AOM. Paracentesis was performed for middle ear fluid culture before the first dose antibiotic therapy. Children with acute otitis media were randomised to receive either low dose amoxicillin-clavulanate (45/6.4 mg/kg/day in two divided doses for 10 days) or low dose azithromycin (10mg/kg/day for 3 days). Clinical response was assessed on days 2-4, 11-13, 26-28. RESULTS: Bacterial pathogens were isolated from 108 (60%) of 180 children. Streptococcus pneumoniae was the most common isolated pathogen (39.7%), followed by Haemophilus influenzae (20.7%), Moraxella catarrhalis (15.5%), Staphylococcus aureus (13.8%), Group A beta-hemolytic streptococcus (5.1%), Escherichia coli (3.4%) and Enterococcus faecalis (1.7%). This study demonstrated low resistance rates compared to studies of different countries. Although clinical response rates were better in patients treated with amoxicillin-clavulanate, this was not statistically significant [86.6% (78 of 90)] versus [95.2% (80 of 84)]. Success rates of amoxicillin-clavulanate were high for both S. pneumoniae and H. influenzae. Difference between success rates was not statistically significant (P=0.144 and 0.352). CONCLUSIONS: Bacteria were isolated in 60% of AOM cases. The clinical efficiency of amoxicillin-clavulanate was found to be equal compared to azithromycin in children with acute otitis media.

Prediction methods for babies' birth weight using linear and nonlinear regression analysis.

The aim of this study is to determine more accurate prediction methods between linear and non-linear methods for prediction of babies' birth weight among maternal demographic characteristics. Three hundred pregnant women were included in the study. Blood glucose level before and after ingestion of glucose load, age, body mass index, % of change in weight during pregnancy, height, gestational age, parity, and fetal sex were collected as independent variables and baby birth weight as dependent variable. In linear regression, least squares estimation method was used to estimate parameters. Non-linear regression method was performed using neural network model with multilayer perceptrons, back propagation method was preferred as learning algorithm. Coefficient of determination, R2, of the linear regression equation was found 59.8% and the standard error of the estimate was calculated as 325.69 gr. In non-linear regression method R2 value was also found 59.8% and standard error of estimate was calculated as 320.30 gr. According to the results of the present study, one method is not significantly better than the other. When "accuracy in prediction" is aimed, it is better to use the two methods and compare the results, and then decide on the selection of the favourable method.

Clinicopathologic and radiopharmacokinetic factors affecting gamma probe-guided parathyroidectomy.

HYPOTHESIS: The kinetics of technetium Tc 99m sestamibi (MIBI) in primary hyperparathyroidism are variable and affected by the cellular size of the abnormal glands, the parathyroid hormone levels, and the functional expression of P-glycoprotein (Pgp). The success of gamma probe-guided parathyroidectomy is closely related to the parathyroid-to-thyroid activity ratio at the time of surgery. Preoperative determination of maximum uptake ratio may improve the surgical outcome. DESIGN: Thirty-one patients with primary hyperparathyroidism attributed to a solitary parathyroid adenoma (27 patients) or multiglandular hyperplasia (4 patients) underwent dynamic MIBI imaging preoperatively. Maximum MIBI activity and activity elimination half-life in the abnormal parathyroid glands and thyroid glands were measured, and the maximum uptake ratio was calculated. After a second MIBI injection on the day of surgery, all patients underwent gamma probe-guided parathyroidectomy and cervical exploration. Timing of surgery after MIBI injection was individualized according to the optimal time to surgery (time to maximum uptake ratio), which was determined by preoperative scintigraphy. During surgery, the gamma probe was used to measure ex vivo counts of excised lesions and adjacent postexcision normal tissue (background). Image characteristics, MIBI kinetics, and gamma probe findings were correlated with gland volume, oxyphil cell content, Pgp expression, and serum parathyroid hormone levels. RESULTS: Probe localization of abnormal glands at maximum uptake ratio was successful in all patients. The volume of the parathyroid lesion ranged from 0.03 to 9.8 mL (median, 0.7 mL). Parathyroid maximum MIBI activity correlated with the volume of the gland (r = 0.54, P = .002) and serum parathyroid hormone level (r = 0.58, P = .001). No correlation between maximum MIBI activity and oxyphil cell content or Pgp expression could be demonstrated. Elimination half-life of MIBI from parathyroid inversely correlated with Pgp (r = -0.36, P = .05). The ex vivo lesion-background count ratio positively correlated with volume of the gland (r = 0.66, P = .001) and parathyroid hormone level (r = 0.48, P = .006). Ex vivo lesion counts and Pgp expression were negatively correlated (r = -0.37, P = .04). CONCLUSIONS: A strong relationship between volume of the parathyroid gland, serum parathyroid hormone levels, and MIBI uptake exists in primary hyperparathyroidism. Gamma probe-guided localization of abnormal gland(s) can be more successful if surgery is undertaken at maximum uptake ratio. High Pgp expression increases MIBI parathyroid clearance rate, decreases gamma probe counts, and may significantly alter the optimal time to surgery.

Evaluation of gloss changes of two denture acrylic resin materials in four different beverages.

OBJECTIVES: The primary disadvantages of the materials which are used in construction of complete and removable partial dentures is that their esthetic, physical and mechanical properties change rapidly with time in the oral environment. For esthetics, color stability is one of the criteria that needs careful attention. Color may provide important information on the serviceability of these materials. Color change affects the gloss of these materials. The objective of the present study was to determine the gloss changes resulting from the testing process in four different beverages in one heat-polymerized denture base resin and one cold-polymerized denture base repair resin. METHODS: Thirty-six samples were fabricated for each material. Each sample had a smooth polished and a rough unpolished surface. The gloss measurements were made with a glossmeter before testing. Four different beverages (tea, coffee, cola and cherry juice) were used for testing. Two angles of illumination (20 and 60 degrees) were used for the gloss measurements. The samples were immersed in water, tea, coffee, cola and cherry juice solutions. The gloss of the samples was measured again with the glossmeter at the end of the 45th day and 135th day of testing. The arithmetic mean and standard deviation of each of the samples were calculated and compared with each other statistically by using the Wilcoxon test (within times) (p < or = 0.05 significant), the Kruskal-Wallis analysis of variance (p < or = 0.05 significant) and the Mann-Whitney U-test with Bonforoni correction (when the difference between the samples was significant) (p < or = 0.05 significant). RESULTS: The results of this study revealed that gloss changes occurred after testing in heat-polymerized denture base resin and cold-polymerized denture base repair resin. The significance of the gloss changes exhibited by each sample, kept for different lengths of time in the same solution, were compared using the Wilcoxon test. The results were statistically significant (p < or = 0.05). According to the Kruskal-Wallis analysis of variance, the difference between measurements for angles of illumination was statistically significant (p < or = 0.05). Also according to the Mann-Whitney U-test, the difference between two polished surfaces or two unpolished surfaces was statistically insignificant (p > 0.05), but the difference between smooth polished and rough unpolished surfaces was statistically significant (p < or = 0.05). SIGNIFICANCE: It was found that either the gloss of heat-polymerized denture base resin or the gloss of cold-polymerized denture base repair resin was affected by tested agents, and the four beverages demonstrated noticeable gloss changes. Cherry juice demonstrated the least change, while tea exhibited the greatest change.

A comparative evaluation of the clinical effects of systemic and local doxycycline in the treatment of chronic periodontitis.

In this study, the clinical efficacies of systemic doxycycline (SD) and local doxycycline (LD) in the treatment of chronic periodontitis were compared. Forty-five patients were studied in 3 main groups with 5 treatments: SD alone, SD+scaling-root planing (SD+SRP), LD alone, LD+SRP and SRP alone. Antibiotic-treated patients were given doxycycline treatment alone in 1 quadrant of their upper jaws, and doxycycline+SRP was given in the contralateral quadrant. The areas included at least 4 teeth with > or = 5 mm pockets. Probing depth (PD), clinical attachment level, gingival index, sulcular bleeding index and plaque index values were recorded at baseline and the 7th week. The results were statistically analyzed. All of the clinical parameters were significantly reduced by all treatments (P < or = 0.05). The SD and LD treatments alone provided significant clinical healings. The significant differences among the groups were only in PD at the 7th week. The LD treatment provided significantly higher PD reduction than the SD treatment (P < or = 0.05). No significant difference was found between the SD+SRP and the LD+SRP treatments. There was no significant difference between SD+SRP and SRP alone treatment (P > 0.05). The SD group showed lower PD reduction than SRP group (P < or = 0.05), while no significant difference was found between LD and SRP treatments. The LD alone treatment seemed more effective than SD alone treatment on PD reduction, but no significant difference was found between them when combined with the SRP. LD may be more preferable than SD as an adjunct to mechanical treatment since LD seems more effective than SD on PD reduction and does not have the side effects of SD.

Analysis of the relationship between the severity of periodontal destruction and proteoglycan metabolism of gingiva and gingival crevicular flu.

BACKGROUND: Although it is well-described that proteoglycans (PGs) are among the major non-collagenous components of the matrix which are degraded during periodontal diseases, the relationship between PG metabolism and seventy of periodontal breakdown, the extent of degradation of PGs together with the resulting end-products, and the elimination pathways of these catabolic end-products is likely to need further clarification. OBJECTIVE: The main aim of the present study was to analyze the possible impact of severity of periodontal destruction on PG metabolism of gingiva and gingival crevicular fluid (GCF). MATERIAL AND METHODS: For this purpose, gingiva and GCF samples obtained from patients (n = 45) exhibiting sites (n = 57) with moderate periodontal breakdown (MP) or severe periodontal breakdown (SP) were analyzed for PG metabolism via spectrophotometric determination of uronic acid levels. Gingiva and GCF samples were obtained from the same sites in every patient to analyze the possible relationship between uronic acid content of gingival tissue and GCF. RESULTS: No significant differences were found in uronic acid levels between sites with MP and SP (p > 0.05). The uronic acid content of GCF and gingiva showed significant overlaps between MP and SP sites and uronic acid levels did not present any constant correlation with the clinical parameters (p > 0.05). In a similar manner, uronic acid content of GCF and gingival tissue was not correlated (p > 0.05). CONCLUSION: The lack of a significant correlation between the uronic acid content of gingival tissue and GCF may suggest that the passage of PG metabolites from gingiva to GCF is likely to be under the influence of multifactorial interactions rather than being linear. As a general measure of PG metabolism, uronic acid levels do not seem to be related with the severity of periodontal destruction and tend to act as different measures when compared to traditional clinical parameters.

Depression and anxiety in patients with pseudoexfoliative glaucoma.

OBJECTIVE: This study aimed to determine whether patients with pseudoexfoliative glaucoma (PXG), associated with vascular disorders, have more depressive and anxiety symptoms than patients with primary open-angle glaucoma (POAG) and controls without glaucoma. The vascular depression model was tested in this study. METHOD: The sample consisted of 41 PXG patients, 32 POAG patients and 40 controls. Each subject was diagnosed as having PXG or POAG or chosen as a control patient by an ophthalmologist and then evaluated by a psychiatrist. The Structured Clinical Interview for DSM-IV, Turkish version; Hamilton Depression Rating Scale (HDRS); Hamilton Anxiety Rating Scale (HARS); Mini Mental State Examination; and Montgomery-Asberg Depression Rating Scale (MADRS) were administered to all subjects. RESULTS: The HDRS and MADRS scores of the PXG patients were significantly higher than those of the POAG and the control group (chi(2)=9.858, p=.007 and chi(2)=11.618, p=.003, respectively). The HARS scores did not show a significant difference between the patients with PXG or POAG and the control subjects (chi(2)=1.615, p=.446). In each of the three groups, there was no correlation between the HDRS, HARS or MADRS scores and any of the following parameters: duration of glaucoma, medical treatment, visual acuity, intraocular pressure, perimetric stage, cup-disc ratio and number of glaucoma operations (p>.05). CONCLUSION: A relationship between PXG and severity of depressive symptoms was shown by our data. However, no significant difference could be found between the anxiety levels of the three groups.
