Gene expression in mixed type liposarcoma.

AIMS: Mixed type liposarcomas are rare. Here, we analysed the characteristics of an unusual case of mixed type liposarcoma, which consisted of a well-differentiated liposarcoma (WDL) and a pleomorphic liposarcoma (PL), with a special emphasis on molecular alterations. METHODS: Microscopic and immunohistochemical approaches were used to investigate this case of mixed type liposarcoma, and to identify molecular alterations in this tumour, gene expression was examined in PL, WDL, and normal adipose tissue (NA) samples using a 17,000 cDNA microarray. RESULTS: The tumour mass, 9 x 5 x 5 cm, was located in the left upper arm of a 76-year-old man. Grossly, the proximal portion of the tumour was composed of a yellowish fatty lesion, whereas the distal portion of the tumour was whitish and necrotic in nature. Histologically, the tumour was composed of two distinct components. The proximal component of the tumour was a WDL and the distal component was a PL. Immunohistochemically, S100 protein immunoreactivity highlighted lipoblasts in both tumour portions. The Ki-67 proliferation index was <1% in the WDL and 20% in the PL. MDM2 was positive in the WDL, but negative in the PL. p53 was negative in both areas. Numerous differentially expressed genes were found, which included genes coding for signal transduction, transcription, cell cycle, enzyme, structural protein, immune system and others. CONCLUSIONS: Our data demonstrate that multiple genes are differentially expressed in mixed type liposarcoma and suggest that these genes are associated with the differences in the morphological characteristics and pathogenesis of mixed type liposarcoma.

Reliability analysis of 16-MDCT in preoperative evaluation of total knee arthroplasty and comparison with intraoperative measurements.

OBJECTIVE: The aim of our study was to determine how consistent measurements of the distal femoral condyle on 16-MDCT are with intraoperative measurements and the reliability of 16-MDCT for the preoperative planning of total knee arthroplasty. MATERIALS AND METHODS: Between August 2003 and March 2004, 33 consecutive patients (two men and 31 women; age range, 53-89 years; mean age, 71 years) presenting with osteoarthritis underwent 16-MDCT (Mx 8000 IDT) of a total of 53 knees before total knee arthroplasty. The prospective analysis included measurements of transepicondylar distance, maximum anteroposterior dimension of medial and lateral femoral condyles, and trochlear width on a Rapidia workstation. To increase reliability, we repeated the measurements on a CT workstation after 2 months and compared them with the previous values. The values measured on the CT workstation were compared with the measurements obtained in the intraoperative field. Statistical analysis was performed using kappa statistics. A p value of less than 0.05 was considered statistically significant. RESULTS: The mean values of transepicondylar distance, maximum anteroposterior dimension of medial and lateral femoral condyles, and trochlear width were 75, 57, 58, and 38 mm at first measurement; 76, 58, 59, and 39 mm at second measurement on the CT workstation; and 79, 57, 60, and 42 mm at intraoperative measurement, respectively. At reliability analysis between the first measurements on the CT workstation and the intraoperative measurements, kappa values were 0.84 for the transepicondylar distance, 0.81 for the maximum anteroposterior dimension of the medial femoral condyle, 0.89 for the maximum anteroposterior dimension of the lateral femoral condyle, and 0.62 for the trochlear width. The kappa values for the second measurements were 0.86, 0.77, 0.84, and 0.61, respectively. Intraoperative measurements and measured values on the CT workstation showed excellent and almost perfect agreement, and intraobserver agreement was almost perfect. CONCLUSION: Femoral sizing on a CT workstation had excellent or almost perfect correlation with intraoperative measurements. Therefore, reliable preoperative planning for total knee arthroplasty may be done with 16-MDCT and an advanced workstation.

Therapeutic effect and outcome predictors of sciatica treated using transforaminal epidural steroid injection.

OBJECTIVE: The objectives of this retrospective study were to assess the therapeutic effect of transforaminal epidural steroid for sciatica and to identify outcome predictors. MATERIALS AND METHODS: Transforaminal epidural steroid injections were performed in 248 patients from June 2003 to May 2004. Fifty-six patients (33 women, 23 men; mean age, 53.3 years; age range, 30-83 years) were included. Therapeutic effects were evaluated 2 weeks after injection. The possible outcome predictors were as follows: intraepineural or extraepineural injection, saddle-type distribution pattern (contrast material distributed rostrally to the epidural portion of the preganglionic nerve root) or not saddle type, cause of sciatica (spinal stenosis vs herniated disk), patient age, patient sex, and duration of sciatica (acute or subacute [< 6 months] vs chronic [> 6 months]). The relationships between possible outcome predictors and therapeutic effects were analyzed. Statistical analysis was performed using Fisher's exact test, the chi-square test, and multiple logistic regression analysis. RESULTS: Forty-three (76.8%) of the 56 patients achieved a satisfactory result 2 weeks after transforaminal epidural steroid injection. Nineteen (65.5%) of the 29 patients treated by intraepineural injection and 24 (88.9%) of the 27 patients treated by extraepineural injection achieved a satisfactory result, and this difference was significantly different (p < 0.05). Other possible predictors of a better outcome were identified--that is, saddle-type pattern of contrast distribution, a herniated disk, and sciatica of less than 6 months' duration. Multiple regression analysis showed that the only factor significantly associated with outcome was the type of injection (p = 0.04, odds ratio: 5.01). CONCLUSION: Transforaminal epidural steroid is an effective tool for managing sciatica, and an extraepineural injection may be a predictor of a better outcome for sciatica treated using transforaminal epidural steroid.

[Analysis of the nursing interventions performed by public health nurses in health centers using the NIC].

PURPOSE: The purpose of this study was to identify nursing interventions performed by public health nurses in health centers. METHOD: Data was collected by the taxonomy of Nursing Intervention Classification (NIC 3rd: 486 nursing interventions) from 131 public health nurses in health centers and analyzed using descriptive statistics. RESULT: As its result, more than 50% of public health nurses performed 137 nursing interventions at least monthly. The most frequently used intervention class was "activity and exercise management", followed by "physical comfort promotion", "community health promotion", "life span care", "coping assistance", "self care facilitation", "information management", "nutrition support", "community risk management" and "patient education". One hundred twenty nursing interventions were rarely performed by 90% or more of the nurses. Most of them were in the physical complex domain. CONCLUSION: In conclusion, 137 interventions were performed by public health nurses at least monthly. NIC is helpful to build a standardized language for public health nursing.

Pelvic floor muscle exercise by biofeedback and electrical stimulation to reinforce the pelvic floor muscle after normal delivery.

PURPOSE: This study was conducted to investigate the effectiveness of pelvic floor muscle exercise using biofeedback and electrical stimulation after normal delivery. METHODS: The subjects of this study were 49 (experimental group: 25, control group: 24) postpartum women who passed 6 weeks after normal delivery without complication of pregnancy, delivery and postpartum. The experimental group was applied to the pelvic muscle enforcement program by biofeedback and electrical stimulation for 30 minutes per session, twice a week for 6 weeks, after then self-exercise of pelvic floor muscle was done 50-60 repetition per session, 3 times a day for 6 weeks. Maximum pressure of pelvic floor muscle contraction (MPPFMC), average pressure of pelvic floor muscle contraction (APPFMC), duration time of pelvic floor muscle contraction (DTPFMC) and the subjective lower urinary symptoms were measured by digital perineometer and Bristol Female Urinary Symptom Questionnaire and compared between two groups prior to trial, at the end of treatment and 6 weeks after treatment. RESULTS: The results of this study indicated that MPPFMC, APPFMC, DTPFMC were significantly increased and subjective lower urinary symptoms were significantly decreased after treatment in the experimental group than in the control group. CONCLUSIONS: This study suggested that the pelvic floor muscle exercise using biofeedback and electrical stimulation might be a safer and more effective program for reinforcing pelvic floor muscle after normal delivery.

Horizontal and vertical distribution of PCBs and chlorinated pesticides in sediments from Masan Bay, Korea.

Horizontal and vertical distributions of organochlorine compounds (OCs) were determined in sediments from Masan Bay. The concentrations of polychlorinated biphenyls (PCBs), dichlorodiphenyltrichloroethanes (DDTs), HCB, hexachlorocyclohexanes (HCHs) and chlordane related compounds (CHLs) in sediments were in the range of 1.24-41.4, 0.28-89.2, 0.02-0.59, nd-1.03, and nd-2.56 ng/g, respectively. The spatial distribution of OCs showed a negative gradient from the inner of the bay to outer part of the bay, indicating that the source of OCs was probably located inside the bay. Compositional pattern of PCB congeners showed a relatively high concentration of high-chlorinated congeners in the inner part of the bay and a relatively low concentration of low-chlorinated congeners in the outer part. In sediment core from Masan Bay maximum concentrations of PCBs and DDTs are observed in the subsurface samples and correspond to an age of early 1980s and late 1960s. The concentration profiles of PCBs and DDTs in sediments of Masan Bay appear to correspond to use of PCBs and DDTs in Korea.

Family functioning in the context of chronic illness in women: a Korean study.

The aims of this study were to clarify the concept of family functioning in the context of a female family member's chronic illness, and to describe the processes by which the housewife copes with the situation. The study applied the hybrid model of concept development, which consists of three phases: theoretical, empirical, and analytic. In the theoretical phase, a working definition of family functioning was established and the dimensions of family functioning and subconcepts were identified through an extensive review of the literature. In the empirical phase, in-depth interviews with members of six normal families and seven families in which the housewife had a long-term diagnosis of rheumatoid arthritis were carried out in order to gain descriptions of family functioning in both contexts, and to assess how the housewife coped with her illness in relation to family functioning. The final analytic phase identified the differences and similarities in family functioning between the normal family and family with a chronically ill housewife. A refined definition of family functioning emerged that identified the concept in terms of a complex set of functional dimensions comprising affective, structural, control, cognitive, and external relationships. The data revealed that family functioning is dynamically changed when the housewife becomes ill with a chronic disease. Three types of adaptation process were identified by which the housewives adapted to family functioning in the context of their chronic illnesses: negotiated, self-accommodating, and separated-enduring. The implications of these findings for research and nursing practice are discussed.

Effect of solids concentration on bacterial leaching of heavy metals from sewage sludge.

Microbial metal leaching from sewage sludge (2-9% w/v) was carried out with the iron-oxidizing bacterium Thiobacillus ferrooxidans. Measurements of pH, oxidation-reduction potential, and concentration of Fe2+ indicated that T. ferrooxidans was effective in removing metals from an incubation bath containing less than 5% sludge solids concentration. Specifically, Cu leaching was completely suppressed at a high solids concentration of 9% (w/v). Results indicated that the deactivation of T. ferrooxidans at a high sludge content was mainly due to the presence of inhibiting materials such as organic matter. A mixed culture of sulfur-oxidizing bacteria was obtained by enrichment from anaerobically digested sewage sludge to enhance the efficiency of the microbial leaching process. These bacteria were much more effective in metal leaching than was iron-oxidizing T. ferrooxidans. At 9% (w/v) solids concentration, the leaching efficiencies of Zn and Cu were 78% (2.66 g/kg dry sludge) and 59% (1.36 g/kg dry sludge), respectively. Therefore, when removing heavy metals from the anaerobically digested sewage sludge, the indigenous sulfur-oxidizing bacteria isolated in the current study were more efficient than T. ferrooxidans, especially at high sludge solids concentrations.

Prediction of recurrent hemoptysis with MDCT angiography.

OBJECTIVE: To identify the computed tomography (CT) findings to predict recurrent bleeding in patients with massive hemoptysis. METHODS: Fifty-eight patients with massive hemoptysis underwent both CT and conventional bronchial angiography for bronchial artery embolization. Retrospective analysis was done to analyze the statistical differences in the initial CT findings between the nonrecurrent and recurrent hemoptysis groups using the Mann-Whitney U test and the chi or Fisher exact test. Stepwise logistic regression analysis was used to find predictive variables for the recurrence of hemoptysis. RESULTS: After embolization, recurrence of hemoptysis was observed in 15 of 58 patients (26%). The diameter of bronchial (P = 0.001) and nonbronchial systemic arteries (P < 0.05), total number of dilated bronchial and nonbronchial systemic arteries (P = 0.001), pleural thickening (P = 0.01), extrapleural fat thickening (P = 0.004), and enhancing vascular structures within the extrapleural fat layer (P = 0.04) were significantly different in the nonrecurrent and recurrent hemoptysis groups. Stepwise logistic regression analysis showed that the total number of dilated bronchial and nonbronchial systemic arteries (odds ratio, 3.540; 95% confidence interval: 1.175, 10.662; P = 0.025) was a significant CT variable associated with the recurrence of hemoptysis. CONCLUSIONS: CT bronchial angiography may help in the prediction of recurrent bleeding in patients with massive hemoptysis.
