Ser217Leu polymorphism of the HPC2/ELAC2 gene associated with prostatic cancer risk in Japanese men.

The HPC2/ELAC2 gene may be associated with hereditary/familial prostate cancer (PCa). Two common missense variants (Ser217Leu and Ala541Thr) have been reported in the gene. We performed mutational, allelotyping and expression analyses and a molecular epidemiological study to clarify the relations between this gene and prostatic diseases, including PCa and benign prostatic hyperplasia (BPH) in Japanese men. We screened for mutations in 109 patients with PCa including 11 patients from 1 hereditary and 9 familial PCa. Loss of heterozygosity and expression were analyzed. An epidemiological study was done in sporadic PCa (n=98) and BPH (n=143) using 1 novel (Ser627Leu) and 2 previously described polymorphisms of the HPC2/ELAC2 gene. Somatic or germline mutations were not confirmed in any cases of PCa. Loss of heterozygosity at 2 microsatellites, D17S1289 and D17S520, was detected in 1 of 38 and 1 of 35 cases, respectively. Expression analysis revealed decreased or absent mRNA expression in 6 of 38 tumors. Epidemiologic analysis showed that a Leu allele at codon 217 was significantly more frequent in patients with PCa than in controls (10.2% vs. 3.5%, odds ratio = 3.11; 95% confidence interval, 1.22-7.90). At codon 541, all patients with PCa or BPH and all control subjects had the Ala/Ala genotype. At codon 627, the incidence of the Leu variant was slightly, but not significantly, higher in patients with BPH than in controls (7.0% vs. 2.8%, odds ratio = 2.59, 95% confidence interval, 0.94-7.13, not statistically significant). We concluded that germline/somatic mutations of HPC2/ELAC2 are uncommon in PCa. Similarly, allelic imbalances at the gene locus and changes in expression are rare. Although no difference in allele frequency at Ser217Leu between patients with PCa and controls has been reported in a Western population, this polymorphism is a potential indicator of PCa risk in Japanese men and it should be examined in other ethnic groups.

A new organic superconductor, (DODHT)2BF4 x H2O.

In addition to two organic superconductors (DODHT)2X [DODHT = (1,4-dioxane-2,3-diyldithio)dihydrotetrathiafulvalene; X = PF6 and AsF6] previously reported by us, the BF4 salt of DODHT containing one water molecule [(DODHT)2BF4 x H2O] has been found to undergo a superconducting transition at 3.2 K under a hydrostatic pressure of 15.5 kbar.

Design of novel inorganic-organic hybrid materials based on iron-chloranilate mononuclear complexes: characteristics of hydrogen-bond-supported layers toward the intercalation of guests.

Novel intercalation compounds constructed from the common two-dimensional hydrogen-bond-supported layers and functional guests [(H(0.5)phz)(2)[Fe(CA)(2)(H(2)O)(2)].2H(2)O](n)(1), ([Fe(Cp)(2)][Fe(CA)(2)(H(2)O)(2)])(n)(2), ([Fe(Cp*)(2)][Fe(CA)(2)(H(2)O)(2)])(n)(3), and [(TTF)(2)[Fe(CA)(2)(H(2)O)(2)]](n)(4) (H(2)CA = chloranilic acid, phz = phenazine, [Fe(Cp)(2)] = ferrocene, [Fe(Cp*)(2)] = decamethylferrocene, TTF = tetrathiafulvalene) are described. The guest cations are introduced between the ([Fe(CA)(2)(H(2)O)(2)](m-))(l) layers by electrostatic (1-4) and pi-pi stacking (3, 4) interactions. [Fe(Cp*)(2)](+) cations in 3 are stacked on each other making tilted columns which are included in the channel created by the chlorine atoms of CA(2-) dianions. TTF cations in 4 are stacked face to face with two types of S...S distances (type A; 3.579(3) A, and type B; 3.618(3) A) making a columnar structure. The TTF cations in the stacked column have a head-to-tail arrangement with respect to the iron-chloranilate layer. Mössbauer spectroscopy suggests that [Fe(CA)(2)(H(2)O)(2)](m-) anion in 3 is consistent with high-spin (S = 5/2) iron(III) ions and [Fe(Cp*)(2)](+) in the low-spin (S = 1/2) iron(III) ions. In 4, Mössbauer spectroscopy shows high-spin iron(II) ions (IS = 1.10 mm.s(-1) and QS = 1.66 mm.s(-1) at 297 K) and high-spin iron(III) ions (IS = 0.42 mm.s(-1) and QS = 1.27 mm.s(-1) at 297 K), suggesting that the anionic layer of iron-chloranilate has a valence-trapped mixed-valence state. At the temperature range of 77-300 K, the compounds 1, 2, and 3 are EPR silent, whereas the EPR spectrum of 4 shows two types of signals with g = 2.008 indicating the radical form of TTF.

[Decrease in anticoagulant factors in patients with prostate cancer treated with diethylstilbestrol diphosphate].

PURPOSE: Estrogen has been highly evaluated as one of the most potent endocrine agents for the treatment of prostate cancer. Unfortunately, a high risk of cardiovascular complications is a clinically important adverse effect of estrogen therapy, and occasionally the complications are fatal. In recent years a high incidence (55%) of thrombotic events has been reported in patients with congenital protein S (PS) deficiency. The aim of this study is to determine the relationship between cardiovascular complications of estrogen therapy and anticoagulant factor levels in the serum of patients with prostate cancer. MATERIALS AND METHODS: Study 1 employed 99 patients with prostate cancer: 39 were untreated, 25 were treated with LH-RH agonist therapy alone, and 35 were treated with oral diethylstilbestrol diphosphate (DESdP) 300 mg per day. We measured the serum levels of anticoagulant factors, parameters antithrombin III (ATIII), protein C (PC), PS, coagulant and fibrinolytic factors in all patients. In study 2, the adverse effects of DESdP therapy on the serum levels of anticoagulant factors were examined in 8 patients with advanced prostate cancer. RESULTS: In study 1, the ATIII and PS levels of the patients treated with estrogen therapy were significantly lower than those in either the untreated patients or the patients treated with an LHRH agonist alone. Especially, both PS antigen (51.5 +/- 16.0%) and PS activity (42.9 +/- 16.0%) were markedly lower in estrogen-treated patients than in the untreated patients (102 +/- 20.8%, 100.6 +/- 20.7%, respectively) or the patients treated with an LH-RH agonist alone (97.9 +/- 16.8%, 91.5 +/- 17.7%, respectively, both p < 0.0001). PS was decreased to below the normal lower limit of normal in 82% (24/35) of the patients on estrogen therapy. In study 2, all 8 cases showed a significant decrease in PS after DESdP therapy. CONCLUSIONS: Our results showed that the PS levels in the oral DESdP group were almost the same as in patients with congenital PS deficiency. We conclude that decreased PS may play a role in the development of cardiovascular complications in prostate cancer patients on estrogen therapy.

[Mucinous adenocarcinoma of the prostate: a case report and review of 32 cases on immunohistochemical study of both PSA and CEA].

Mucinous adenocarcinoma of the prostate is extremely rare and its biological behavior is not well known. We report a case of mucinous adenocarcinoma of the prostate which stained positively for prostate specific antigen (PSA) and negatively for carcinoembryonic antigen (CEA) on immunohistochemical study. Our case contained conventional adenocarcinomas and no signet-ring cells. Thirty two cases of mucinous adenocarcinoma of the prostate which performed on immunohistochemical study of both PSA and CEA, including our case, were reviewed. 17 of the 23 cases of immunoreactive to PSA contained conventional adenocarcinomas, and 3 of the 10 cases of immunoreactive to CEA contained them, respectively. The 6 cases of immunoreactive to CEA only contained signet-ring cells. It indicated that there seemed to be the two types of mucinous adenocarcinoma of the prostate, the one which stained positively for PSA was the subtype of conventional adenocarcinomas, and the other which stained positively for CEA and negatively for PSA was derived from the intestinal metaplasia with atypia of the prostatic urethra.

[Papillary adenocarcinoma of the prostate: report of 4 cases].

Papillary adenocarcinoma of the prostate, previously referred to as endometrioid carcinoma, is a variant of prostatic adenocarcinoma. Clinical and pathological evidence of involvement of the periurethral prostatic duct or verumontanum is usually required for definitive diagnosis of papillary adenocarcinoma. However, significant histologic and clinical features of papillary adenocarcinoma overlap with typical acinar carcinoma. Four cases of papillary adenocarcinoma were studied for the clinical features, histologic characteristics and immunohistochemical nature of prostatic specific antigen. In two cases, there were papillary regions, near the verumontanum, but in the other two cases, there were no papillary regions in the urethra. In two cases, acinar adenocarcinoma coexisted with papillary adenocarcinoma. All cases displayed positive immunohistochemical staining for prostatic specific antigen. In accordance with the observations of others, we suggest that papillary adenocarcinoma is one aspect of growth pattern of acinar adenocarcinoma, not a concept of a unique clinical and pathological entity.

[Retrospective analysis of chief complaints of patients with urogenital malignancies over the past decade at the Jikei University Hospital].

We analyzed the chief complaints of patients with four major urogenital malignancies (renal cancer, renal pelvis and ureter cancer, bladder cancer and prostatic cancer) over the past decade (1990-1999) at the Jikei University Hospital. Over the last 10 years, a high percentage of renal cancers were detected incidentally. By contrast, prostatic cancers were more likely (10.5%) than other cancers to be detected on the basis of symptoms of metastasis. However, since 1995 more prostatic cancers are being detected with prostatic-specific antigen screening at the health checkups. Gross hematuria is the chief complaint of most patients with uroepithelial cancers (cancers of the renal pelvis, ureter and bladder cancer). Additionally, renal pelvis and ureter cancers were diagnosed with screening in a few patients in the past five years.

2-(1,3-Dithiolan-2-ylidene)-5-(1,3-dithian-2-ylidene)-1,3,4,6- tetrathiapentalene(DHDA-TTP), a hybrid of BDH-TTP and BDA-TTP, and its metallic cation-radical salts.

The synthesis and electrochemical properties of the DHDA-TTP donor, a hybrid of 2,5-bis(1,3-dithiolan-2-ylidene)-1,3,4,6-tetrathiapentalene (BDH-TTP) and 2,5-bis(1,3-dithian-2-ylidene)-1,3,4,6-tetrathiapentalene (BDA-TTP), has been investigated, and its ability to form metallic cation-radical salts is elucidated.

Structural study of three isomers of Tm@C(82) by (13)C NMR spectroscopy.

The (13)C NMR spectra were measured for three isomers of Tm@C(82), which is one of the divalent metallofullerenes. The molecular symmetries were determined for each isomer: isomer I has C(s) symmetry, isomer II has C(2) symmetry, and isomer III has C(2v) symmetry. Moreover the cage structure of Tm@C(82)(III) was found to be C(82)(9). As a result, it was revealed that Tm@C(82)(III) has a cage identical to that of La@C(82), which is one of the trivalent metallofullerenes.

New organic superconductors consisting of an unprecedented pi-electron donor.

The synthesis and electrochemical properties of a new organic donor, (1,4-dioxan-2,3-diyldithio)dihydrotetrathiafulvalene (DODHT), and the crystal structures and physical properties of its superconducting salts are described. The DODHT molecule has the bulky dioxane ring condensed by cis fusion, and the reduced pi-electron system of DODHT, as compared with conventional tetrachalcogenafulvalene donors, leads to both the weak electron-donating ability and the large on-site Coulomb energy. Nevertheless, DODHT produces the superconducting AsF6 and PF6 salts, which are isostructural and exhibit superconducting transitions at 3.3 and 3.1 K, respectively, under 16.5 kbar.

[Fatal septic shock caused by transrectal needle biopsy of the prostate; a case report].

A 46-year-old man refer to us because of hemospermia. The prostatic gland was normal in size and consistency at rectal examination. Serum prostate specific antigen was 7.04 ng/ml. Magnetic resonance imaging showed an area of low signal intensity on T2-weighted images in the left peripheral gland, possibly indicative of carcinoma. Transrectal prostate biopsy was performed after intravenous administration of piperacillin. He developed chills and fever (39 degrees C) the next morning following biopsy. He was taken unconscious into the hospital where a diagnosis of septic shock caused by Escherichia coli was made. Five days later, he died. His general condition deteriorated notwithstanding intensive treatment. Postmortem blood cultures were positive for a piperacillin resistant Escherichia coli. Histological examination of the biopsies showed a benign prostatic hyperplasia. Autopsy showed diffuse tissue damage in the heart, lung, liver and kidneys. The prostate had numerous microabscesses. Currently, transrectal prostate biopsy is considered a generally reliable procedure to detect adenocarcinoma of the prostate. Our case seems to the sixth case report of fatal complications.

[Clinical study of bladder injury].

Since bladder injury has no specific clinical symptoms, accurate diagnosis at first consultation is relatively difficult. To elucidate the clinical characters type of injury, clinical symptoms, laboratory findings, methods of therapy and diagnosis, we reviewed 15 patients with bladder injury over a 9-year-period 1990-1998 (10 were traumatic injuries and 5 spontaneous injuries). We found no specific clinical symptom of bladder injury. Bladder injury may occur anywhere in the bladder wall, but most commonly occurred at the dome of the bladder (60.0%). Gross hematuria was not seen in 40.0% of the cases. The accuracy of diagnosis at first consultation was relatively low (46.7%) and the tendency to make a misdiagnosis as acute abdomen on digestive organs was found. Of the traumatic injuries 60% were afflicted in the drunken state, so alcohol intoxication was considered as an important enviromental factor of bladder injury. Surgical repair of injury sites was employed in 11 cases (73.3%: 7 were intraperitoneal injuries, 4 were extraperitoneal injuries), 4 cases were managed with indwelling urethral catheter. With appropriate treatment, the prognosis is excellent.

[A case of complete self-mutilation of penis].

Self-mutilation of the penis is extremely rare. A 69-year-old man was admitted after having amputated his own penis completely from its root. He had no history of psychiatric illness, but his physical condition on admission was abnormal. We performed urethrocutaneostomy, rather than replantation of the penis, because of the danger that he would reinjure himself. The patient was treated by a psychiatrist under a diagnosis of alcoholic dementia. To our knowledge, this is the 24th case of self-mutilation of the penis reported in the Japanese literature.

[Schwannoma with purely cystic form originating from the adrenal area: a case report].

A 62-year-old woman was hospitalized with complaints of right upper abdominal discomfort. Various imaging studies showed an extremely large suprarenal mass with solitary cystic formation. Partial adrenalectomy was successfully performed through the transperitoneal approach. The resected mass measured 12 x 10 x 10 cm and weighed 600 g. A pathological examination showed an Antoni-B predominant-type benign schwannoma containing a large volume of degenerative fluid. Our search of literature yielded few reports of solitary cystic schwannomas in the retroperitoneal cavity or throughout the body. This unusual cystic manifestation is thought to be a terminal stage of degeneration of a long-standing schwannoma.

[Malignant fibrous histiocytoma of perirenal tissue: a case report].

A 75-year-old woman was admitted to our hospital with an extremely large retroperitoneal tumor that had been detected with ultrasound on a routine health check. She had no complaint except lumbar pain. Computed tomography revealed a heterogenous tumor located outside the right kidney which was enhanced gradually. Doppler ultrasound showed mild vascularity in the tumor. We performed tumorectomy and right nephrectomy because the tumor was adherent to the right kidney. The tumor was 16 x 11 x 7 cm in size and weighed 621 g. The histopathological diagnosis was malignant fibrous histiocytoma. The tumor was considered to have arisen from perirenal tissue.

[A case of lung collapse caused by endobronchial metastasis from renal cell carcinoma reinflated with laser and electrosurgical snaring].

A 62-year-old woman, who had undergone left nephrectomy for renal cell carcinoma and had received interferon-alpha for metastasis to the lung, was hospitalized because of cough, dyspnea and anorexia 16 months after nephrectomy. Chest radiography showed collapse of the right lung. We performed bronchoscopy and found a red polypoid tumor completely obstructing the right bronchus. Biopsy specimens showed clear cell carcinoma, similar to previous specimens of renal cell carcinoma. We removed the endobronchial tumor with laser and electrosurgical snaring, after which the right lung reinflated.

Facile Dehydrogenation of alpha-Amino Acids Chelated to a Ruthenium(II) Ion: (alpha-Imino acidato)ruthenium(II) Complexes.

(alpha-Imino acidato)ruthenium(II) complexes, [Ru(II){N(R(1))=C(R(2))CO(2)}L(2)](+) (R(1) = R(2) = Me or R(1) = R(2) = -(CH(2))(3)-; L = 2,2'-bipyridine (=bpy) or 1,10-phenanthroline (=phen)), were obtained by anodic oxidation at a constant potential of the corresponding (alpha-amino acidato)ruthenium(II) complexes, N-methylalaninato or prolinato complexes, in good to excellent yields. (alpha-Imino acidato)ruthenium(II) complexes are stable in neutral or acidic aqueous solution. The half-wave potentials of alpha-imino acidato complexes are 0.73-0.78 V (vs SCE), which are more positive than those of the corresponding alpha-amino acidato complexes, 0.55-0.59 V. The crystal structure of [Ru(pro-H(2))(bpy)(2)]ClO(4).3H(2)O (pro-H(2) = 1,2-didehydroprolinato) has been determined by single-crystal X-ray analysis. Crystallographic data: space group C2/c, a = 21.73(1) Å, b = 19.33(1) Å, c = 14.58(1) Å, beta = 114.91(5) degrees, Z = 8, R = 0.0352. The length of the C=N double bond of the alpha-imino acidate moiety is 1.294(5) Å, and Ru-N(imino nitrogen) = 2.042(3) Å. The chelate ring of the alpha-imino acidato ligand is planar.
