[Comparative antihistamine and anti-allergic effects of various antihistamine preparations].

AIM: To compare antihistaminic and antiallergic activity of antihistaminic drugs of the latest generation (ebastin, cetirisine, fexofenadine, loratadine) and antihistaminic drugs of the first generation (clemastin) in the same patients with pollenosis. MATERIAL AND METHODS: Skin prick-titration with 10-dilution histamine and specific allergen, provocative nasal titration with 2-dilution histamine and allergen before and after a single intake of H1-antagonists were made in 30 patients in stable clinical remission of pollenosis during maximal antihistamine activity of the above drugs. RESULTS: Systemic administration of the known H1-antagonists suppresses histamine sensitivity of both skin and nasal mucosa in the same degree. Drugs with more potent antihistaminic activity (fexofenadin and cetirisin) inhibited allergen-induced reactions more effectively. The order of the tested drugs by suppression of allergen-provoked skin and nasal reactions (by lowering antiallergic activity) is the following: fexofenadin and cetirisin > ebastin and loratadin > clemastin. CONCLUSION: The above drugs of the latest generation seem to posses antiallergic activity not only due to antihistaminic effect but also due to other mechanisms. Different suppressive action of H1-antagonists reflects also individual sensitivity to different drugs. The factor of individual sensitivity of the patients to a pharmacological action of the drug may be crucial in the selection of the most effective medicine for each patient. This is confirmed by the data of individual sensitivity of the patient to antihistaminic and antiallergic action of H1-antagonists. The illustrated method may be helpful for individual selection of H1-antagonists for treatment of patients with allergic diseases.

[The in-vivo test of the inhibition of leukocyte natural migration with aspirin and analgin in the specific diagnosis of the asthmatic triad].

Asthmatic triad (AT) is a clinical syndrome incorporating bronchial asthma (BA), recurrent nasal and sinus polyps, intolerance of aspirin, derivatives of pirasolone and other nonsteroid antiinflammatory drugs. Specific diagnosis of aspirin and analgin intolerance in patients with AT was made using the test of natural leukocyte emigration inhibition in vivo in the oral cavity. The test was performed in 22 patients with AT and 13 patients with BA with pollenosis without aspirin and analgin intolerance. In all the patients the test provided positive results, in control patients these were negative. High significance, safety and simplicity of the test made it applicable for specific diagnosis of AT in allergological and pulmonological departments.

[A shock reaction from insect bites in patients with urticaria pigmentosa].

The examination of 11 urticaria pigmentosa (UP) patients included allergological history, skin prick, scarification tests and intracutaneous tests with noninfectious and bee poison allergens, total and specific serum IgE measurements, in vitro reaction of histamine release from peripheral blood basophils induced by bee poison. The response of mastocytosis patients to insect sting was characterized by a rapid (within 5 min) development of severe systemic reactions or shock. The skin reactions and serum antibodies to bee poison were not registered in 9 of 11 patients. They also had a negative reaction of allergen-specific histamine release from basophils. This gives evidence for nonimmunological, pseudoallergic mechanism of the shock reaction. The latter can be prevented by bee poison immunotherapy. IgE-antibody-mediated allergic reaction to bee sting could not be excluded in 2 patients. For them specific immunotherapy with bee poison, possibly with purified poison preparations containing the allergens alone, are indicated.

[A comparative study of the efficacy of Tilade and intal in atopic bronchial asthma].

The authors compared the responses of patients with atopic asthma to two cromolyn drugs of different generations, tilade and intal. As shown by clinical, allergological and immunological findings, tilade (sodium nedocromil) compared to intal is more active against atopic bronchial asthma complicated by obstructive bronchitis. It can more efficiently reduce specific and nonspecific bronchial hyperreactivity, is more potent against inflammation. Due to tilade, the need in glucocorticoid inhalation lowered, it became possible to induce a prolonged remission.

[A new Soviet allergen made from bee venom for the specific diagnosis of allergic reactions to bee stings].

Overall 46 patients with allergic reactions to bee stings were examined. As a result of making skin prick, scarification and intracutaneous tests with an allergen from bee venom, allergy to bee stings was revealed in all the 46 patients, whereas only 37 patients responded to an allergen from the bee body. Specific IgE-antibodies using RAST were detected in 29 out of 31 patients. All the 29 patients had positive skin tests with an allergen from bee venom and only 22 with an allergen from the bee body. Specific histamine release was detected in all 13 patients examined by means of the indicated test. 100% coincidence was recorded with the results of intracutaneous tests with an allergen from bee venom. Only 11 out of the 13 patients had positive intracutaneous tests with an allergen from the bee body. Thus, the new Soviet allergen obtained from bee venom is effective in the diagnosis of allergy in response to bee stings.

[The immunomodulating effect of sodium nucleinate in chronic infectious-allergic urticaria].

Based on clinical, laboratory, bacterial, allergologic and immunologic studies a group of patients with infectious allergic urticaria (IAU) was distinguished. The immunologic examination of 30 patients with IAU identified a decrease in phagocytosis. Sodium nucleinate (SN) was employed as an immunomodulator as compared to placebo. SN was noted to produce an optimal immunomodulating effect, provided it was combined with antibacterial drugs. The use of SN in patients with IAU produced a good clinical effect elucidated for 6 months. It is recommended that SN be used as an immunomodulator in the multimodality therapy of IAU and introduced into the practice of the allergological rooms.

[Clinico-immunologic characteristics of acute toxic-allergic reactions to drugs and current methods for their treatment].

The paper is concerned with clinicoimmunological and allergological characterization of 29 cases of acute toxic allergic reactions (ATAR) to drugs. Four degrees of severity of disease were identified; the most severe form was toxic epidermal necrolysis caused by the use of sulfanilamides and pyrazolones in patients with acute respiratory virus and bacterial infections. Blood analysis in patients with III-IV degree of severity showed a sharp decrease in Ctot and C3 up to 0, an increase in the level of circulating immunocomplexes and "average molecules". Cellular immunity in III degree of severity was decreased. The main principles of therapy using extracorporeal immunocorrective methods were worked out. A leukocyte natural migration inhibition test was used for specific diagnosis of intolerance to medication.

[Clinico-immunological and allergological characteristics of urticaria caused by pyrazolone derivatives].

The authors describe the results of clinical, allergological and immunological examination of 35 patients with urticaria caused by pyrazolone derivatives. Clinically, the patients with pyrazolone-induced urticaria were marked by chronic diseases requiring the prolonged and frequent intake of the analgesics, pyrazolone derivatives. The allergological examination of the 35 patients with pyrazolone-induced urticaria showed that only one of the patients had pollenosis, 6 patients had IgG-mediated reactions to egg protein and one patient to penicillin. For specific diagnosis of drug allergy use was made of the natural leukocyte migration test in vivo according to A. D. Ado. The test appeared positive with analgin in all the 35 patients suffering from pyrazolone-induced urticaria. It represents a simple and accessible method for specific diagnosis of drug allergy both in inpatients and in those visiting allergological rooms at the polyclinics. The immunological examination made with the aid of the histograms demonstrated an appreciable reduction in the content of D-phagocytosing neutrophils. The latter fact might explain the presence of multiple chronic foci of infection in patients with pyrazolone-induced urticaria. Such patients manifested a decrease in C3 that might be related to immediate activation of the alternative pathway of complement by pyrazolone derivatives.

[Allergen-free wards in the treatment of pollinoses and bronchial asthma].

Two allergen-free wards (AFW) fitted out with Academician I. V. Petryanov's filters have been in operation in the Department of General Allergy of the Institute of Immunology, USSR Ministry of Health, since 1981. A high AFW efficacy was shown during treatment without drugs of pollinosis patients with grave manifestations in the pollination season and patients with atopic bronchial asthma outside the season of plant pollination. Since 1981 treatment in AFW has been provided to 232 patients, of them in 104 with a good effect in the season of plant pollination.

[Allergic pollen-induced myocarditis].

The paper concerns two cases of allergic pollen myocarditis. The diagnosis was confirmed by an evident relationship of myocardial lesion to the period of pollen production by allergenic plants or parenteral injection of an allergen and with the respiratory manifestations of pollenosis. In addition to the clinical picture of myocarditis (heart pain, dyspnea, abnormal rhythm), there were also alterations in blood serum enzymes (AsG, ASL-O). Roentgenologically, there was an enlargement of the boundaries mainly on the part of the left ventricle. Flaccid pulsation, ECG abnormalities of the rhythm, conduction and repolarization common to myocarditis were established. Both patients received therapy in an allergen-free ward and completely recovered. It is believed that specific immunotherapy is contraindicated in such patients. The treatment should be carried out in an allergological or cardiological hospital and should consist in the exclusion of allergenic food, administration of glucocorticosteroids, antihistaminic drugs, cardiac glycosides and potassium drugs until full disappearance of the clinical manifestations, normalization of the ECG and laboratory characteristics.

[Correlation between the T, K and NK cell count and the IgE level in patients with atopy (pollinosis)].

In a homogenous group of patients with polyposis, IgE level was juxtaposed to the number of E-rosette-forming cells (E-RFC), K- and NK-cells. The effect of a parenterally introduced allergen (via an accelerated course hyposensibilization) was also determined upon those indices. The number of E-RFC was diminished in the patient group (53,1 +/- 3,3%) as compared with the results (67,2 +/- 2,1%) obtained from the control subjects, selected by the control method of couples. A dependence of a direct ratio was found between the number of E-RFC and IgE level in the patients. The number of K- and NK-cells in the patients did not significantly differ from their number in the control subjects; no relationship was found between the number of K-cells and the number of NK-cells, between those cells and IgE level or the number of E-RFC. Allergen-specific cells - kilers were present in the peripheral blood of the patients with polyposis. The course of specific hyposensibilization carried out was accompanied, in all patients by increased number of E-RFC (to 122 +/- 7% as compared with the initial level) and a reduction of the number of K-cells (to 36 +/- 5.4% as compared with the initial level). Both the changes in IgE level and the number of NK-cells were not of one type in all patients.
