[Strategies of treatment of acute obturative colonic ileus].

Results of surgical treatment of 326 patients with an acute obturation colonic ileus (AOCI) were analyzed. Tumor was localized in left half of colon and retrosygmoid part in 76.8% of observations. There were admitted to the hospital 49.1% of patients with AOCI in subcompensation and decompensation stages. Radical operation was done in 65.4% and palliative one--in 34.6% of patients. Decompression of upper and lower parts of the gut, using oxygenated solutions, was done in complex of preoperative preparation and during operation performance. The method of colonic intraoperative decompression was depicted. In 26.3% of patients for AOCI postoperative complications occurred. Mortality have constituted 11.7%: after operation for AOCI in compensation stage--2.4%, while in subcompensation and decompensation--21.3%.

[Combined surgical interventions on the organs of the abdominal cavity].

The results of treatment of 197 patients who underwent combined operations on the stomach, duodenum, small, large intestine and rectum, gallbladder and anterior abdominal wall are presented. The postoperative lethality was 1.6%. This didn't exceed its level in separate performance of the similar operations.

[Surgical treatment of chronic paraproctitis].

The experience with operative treatment of 974 patients with chronic paraproctitis has been summarized. After the operations for intrasphincter fistula, 1% of the patients developed a disease recurrence, 0.5%--incompetence of the anal sphincter; for transsphincteric fistula--2.6 and 1.9%, respectively, for extrasphincteric fistula--5.7 and 5.2%. A fistula excision into the intestinal lumen is an operation of choice in intra- and transsphincteric fistula. In extrasphincteric fistula, fistula excision, plastic transfer of mucosa for closure of its internal opening with preservation of the anal sphincter should be performed. In presence of the extensive scars and purulent leaks, a ligature should be added to fistula excision.

[The surgical treatment of patients with thyrotoxic goiter].

The results of operative treatment of 1247 patients with thyrotoxic goiter are presented. Thyrotoxic crisis occurred in 0.9%, paresis of the inferior laryngeal nerve--in 1.0%, parathyroid insufficiency--in 0.8%, hypothyrosis--in 2.3% of the patients operated on. The postoperative lethality was 0.2%.

[The diagnosis and surgical treatment of nodular euthyroid and toxic goiter].

The authors discuss their experience in treating 815 patients who underwent an operation for nodular euthyroid and toxic goiter. Thermography was used in the examination of 367 patients and the results were compared with those of radioisotope scanning of the thyroid, intraoperative findings, and the results of histological study. It was established that in complex with clinical methods of examination, thermography is very important in making the differential diagnosis of thyroid diseases. The specific features of surgical tactics in patients with nodular goiter are shown. Economical resection of the gland together with the node is recommended in nodular euthyroid goiter and subtotal subfascial resection of the involved lobe or the thyroid gland in nodular toxic goiter.

[Surgical treatment of hemorrhoids].

The results of operative treatment of hemorrhoids in 1437 patients are presented. After hemorrhoidectomy with restoration of the rectal mucosa, the decrease in incidence of complications after operation was noted.

[Surgical treatment of Meckel's diverticulum].

The author analysed the results of operations on 19 patients with emergency diseases of the abdominal organs caused by pathological processes in Meckel's diverticulum. The inflammatory process in the diverticulum led to acute obstruction of the small intestine in 12, acute diverticulitis in 5, and peptic ulcer complicated by perforation of the intestinal wall in 2 patients. Two patients died in the postoperative period from progressing peritonitis. It was established that Meckel's diverticulum may occur in any area of the small intestine. Meckel's diverticulum detected during planned laparotomy must be resected.
