[Diagnosis and importance of the opportunistic herpes-virus infection in etiology and pathogenesis of different eye diseases].

A total of 439 patients (adults--184, children--255) with herpetic keratitis as well as with uveitis and choriorenitis of different geneses and localizations were examined. An aggravation of HSV-infection was determined by detecting, in the blood), anti-bodies to the unstructured early viral antigens by using the immune enzyme analysis (IEA). The efficiency of the test was demonstrated during the examination of patients with different ophthalmic pathologies. An active infection was detected in 16-39% of patients with various ocular diseases of non-herpetic etiology. The pathogenetic value of reactivated HSV infection is diverse: it can function as an etiological or trigger factor and it can aggravate the main disease through causing, postoperatively, complications.

[The state-of-the art and prospects for the pharmacological therapy of infectious and allergic eye diseases].

The inflammatory, infectious and allergic diseases of the eye still remain an acute problem in practical ophthalmology due to their high prevalence, relapsing clinical course, a growing frequency rate of mycotic and acanthoamebic keratitis and a growing number of resistant causative agents of bacterial keratitis. 55.3% of corneal pathologies are related with the herpes simplex virus, 37.3%--with secondary bacterial infection, 33.2%--with primary bacterial infection, 6.7%--with mycotic infection and 5.6%--with acathoamebic infection. Algorithms of pharmacological therapy are suggested for a majority of inflammatory lesions of the eye, i.e. for herpetic keratitis, ulcerous keratitis caused by blue pus bacillus, gonococcus, staphylococcus, fungi, acanthomebas, as well as for adenoviral and Chlamidia conjunctivitis and allergy of the eye. The author points out that the treatment must be complex, including specific therapy--antiviral one (zovirax ointment, interferon locally, and zovirax or valtrex perorally), antibacterial one (okacin, tobrex, vitabact, fucithamic locally, quinine antibiotics perorally, aminoglycosides or cephalosporins subconjunctivally or intramuscularly) and antimycotic one (locally and perorally). Another part of the complex therapy is equally important--pathogenetic one (carnosine, taufon, vitacic, corneregel ointment), antiallergic one (spersallerg, allergophtal, alomide, lecrolin), antinflammatory one (naclof, maxidex, cyclolip) tears replacing one (natural tears, ophthagel) and immunomodeling one (affinoleicin, lycopid, tactivin).

[Levofloxacin (Tavanic) in the treatment of corneal ulcers].

The main clinical forms of cornea ulcers were systematized and high efficacy of levofloxacin in the treatment of bacterial ulcer of the cornea was shown. In the treatment of several bacterial affections of the eye the systemic use of levofloxacin should be combined with local antibiotic therapy. Combination of levofloxacin with ophthalmic drops of lomefloxacin (okacin) or ofloxacin (floxal) proved to be efficient.

[Lomefloxacin in treatment of infectious eye diseases].

The data on the use of the difluoroquinolone lomefloxacin in the treatment of ocular infections such as bacterial conjunctivitis and keratitis, ulcer of the cornea, uveitis, gonococcal and chlamydial conjunctivitis are presented.

[The use of ocular drug films of propolis in the sequelae of ophthalmic herpes].

There was studied the therapeutic efficiency of ocular medical propolis films (OMF) in 35 patients with postherpetic trophic keratitis and in 20 with postherpetic nebula. OMF were applied behind the lower eyelid at bedtime during 10-15 days. All the patients endured the propolis films well. OMF accelerated the cornea epithelization. Epitheliopathy and micropoint edema of cornea epithelium rapidly disappeared. Time of patients recovery reduced nearly twice (P,001) in comparison with the control group--from 14.1 to 7.6 days. On the average their visual acuity increased in two times--from 0.12 to 0.27 (P > 0.001).

[Itraconazole in the treatment of ophthalmomycoses].

The method and results of the use of itrakonazol, a new antifungal agent of the imidazol class, in the treatment of fungal diseases of the eye are described. The clinical trials involved 50 patients with various clinical forms of ophthalmomycosis: 27 patients with fungal affection of the cornea, 3 patients with fungal endophthalmitis, 8 patients with fungal canaliculitis, 3 patients with fungal blepharitis and 9 patients with fungal conjunctivitis. The clinical trials showed that itraconazole had a sufficiently high antifungal activity in the treatment of the ophthalmomycoses. The drug is low toxic and well tolerated.

[Sodium sulfapyridazine in new delayed-action bases for the treatment of bacterial conjunctivitis].

Possible preparation of 10 per cent sodium sulfapyridazine ophthalmic drops containing aubazidan, a microbial polysaccharide, providing prolongation of the drops action and stability on the storage was studied. The pharmacokinetic studies showed that aubazidan which is a natural polymer provided high prolongation of the sulfapyridazine effect in the tissues of the anterior part of the eye in rabbits. The prolonged effect was similar to the previously observed effect of the solutions containing synthetic polymers such as 1 per cent polyacrylamide and polyvinyl . Satisfactory tolerance by the rabbit eye tissues of 6-fold daily instillations of the 10 per cent sodium sulfapyridazine solution with aubazidan for the observation period of 1 month was stated. When stored in vials the 10 per cent sodium sulfapyridazine ophthalmic drops with 0.5 per cent of aubazidan preserved the stability for 24 months with respect to the colour, transparency, viscosity, pH and drug content. It was demonstrated with using the agar diffusion method and Staphylococcus aureus, Proteus vulgaris, Escherichia coli and Pseudomonas aeruginosa as the test cultures that sodium sulfapyridazine completely preserved its antimicrobial activity in the presence of aubazidan. The data suggested that clinical trials of the 10 per cent sodium sulfapyridazine solution with 0.2-0.5 per cent of aubazidan were promising in prevention and therapy of bacterial conjunctivitis.

[Imidazoles in the treatment of ocular mycoses].

The use of a new class of broad spectrum antifungal drugs i.e. imidazoles and in particular ketoconazole in treatment of severe affections of the eye such as mycoses is described. The clinical trials included 40 patients with various forms of mycosis: mycotic canaliculitis (6 patients), mycotic blepharitis (4 patients), mycotic conjunctivitis (7 patients), keratomycosis (17 patients) and mycotic endophthalmitis (6 patients). Ketoconazole was used in the form of tablets and instillations. The combined treatment included nonsteroid antiinflammatory drugs and antibacterial agents. The clinical trials showed that ketoconazole had pronounced antifungal activity and was rather efficient in treatment of ocular mycoses. Its broad spectrum and low toxicity were recorded.

[Rengasil (pirprofen) in the treatment of inflammatory eye diseases].

The paper describes the method and results of using a nonsteroid anti-inflammatory preparation RENGAZYL in a complex treatment of severe forms of ophthalmic herpes-keratitis with ulcerations. Comparative clinical studies included 81 patients divided into 3 groups. The complex treatment used preparations of antiviral, anti-inflammatory, antiedematous, hypotensive, desensitizing and vitamin therapy. In group 1 (28 patients) rengazyl in capsules was used, in group 2 (24 patients)--intramuscular injections of rengazyl. In group 3 (29 patients) indometacine in pills was used. The clinical results obtained have shown that rengazyl in a form of intramuscular injections possesses expressed anti-inflammatory action in case of the most severe forms of ophthalmic herpes. Besides this, rengazyl possesses analgetic action that makes it more preferable in treatment of inflammations of the choroid accompanied by a pronounced algesic syndrome.

[Epicorneal coatings with kerakol in the combined treatment of keratitis].

The paper analyses effectiveness of treatment of keratitis with cornea ulceration in 114 patients with and without epicorneal covering by keracol. The preparation is a dehydrated and powdered cattle cornea. It easily hydrates by the own corneal fluid to a jelly-like consistency in its epicorneal application. It is shown that epicorneal medicinal covering by keracol allows to have a necessary keratoplastic effect in diseases of the cornea accompanied by its ulceration. Microsurgical interventions, such as scarification of epithelium, dosed local keratectomy of superficial pathologically changed portions of the cornea with subsequent epicorneal covering by keracol, allowed to arrest the pathologic process within 3-10 days in 85% of patients; in the control group--the same results were achieved in 68.4% of cases.

[Effect of cordarone on the eye (results of long-term follow-up of the treatment and prevention of arrhythmia)].

Thirty-one patients who received a prolonged maintenance therapy with cordarone to treat and prevent cardiac arrhythmias underwent an ophthalmologic examination. The treatment lasted 1.5 to 51 months. The signs of keratopathy were detected in 24 (77.4%) patients using a slit lamp biomicroscopy. There is a relationship between the severity of keratopathy and a daily dose of cordarone: in most cases signs of stage I keratopathy were detectable when the daily dose was up to 200 mg. Keratopathy appears to be reversible after the drug has been discontinued.
