[Experience with orthotopic transplantation of the heart].

The improvement of cardiac transplantation outcomes has resulted, since early eighties, in a considerable annual increment in the number of the operations. Original experience with orthotopic heart transplantation is reported. Eleven transplantations were performed between 31.01.84 and 25.09.86. Two patients died in the early postoperative period, 9 patients were discharged from hospital. Two patients died during the late postoperative period. The 7 surviving patients are in a satisfactory condition. According to reports in the literature, supported by the authors' own experience, heart transplantations in carefully selected cases have become a method of treatment. However, they should only be performed at clinical departments with adequate skills in cardiosurgery and organ transplantation.

[Right ventricular function in occlusion of the right coronary artery].

Hemodynamic studies at rest and under volumetric loads were carried out in 10 dogs before and after the ligation of the right coronary artery with the closed pericardium. The ligation of the right coronary artery resulted in hemodynamic signs of depressed right-ventricular function: decreased systolic blood pressure and elevated end diastolic pressure, combined with the functional curve's downward shift to the right. Total cardiac performance (minute output and mean systemic flow pressure) was also reduced. It is suggested that depressed right-ventricular myocardial systolic function in infarction may be important for the development of the clinical low minute volume syndrome. Its pathophysiological mechanism and clinical implications are discussed.

[Emergency coronary surgery].

The authors report their experience with combined investigation and surgical treatment of patients with threatened myocardial infarction and those with postinfarction rupture of the interventricular septum. Direct reconstruction of coronary arteries was performed in 12 of 17 patients with threatened myocardial infarction (three patients died, two of cardiac complications). In the survivors, anginal signs either disappeared or became less pronounced. The correction of interventricular septum rupture was performed in 8 patients, three of whom died. The remaining five patients showed obvious improvement.
