[Several aspects of the pathogenesis of chronic circulatory failure in ischemic heart disease: role of atrial natriuretic peptide].

Alpha-atrial natriuretic peptide (ANP) levels, plasma renin activity, and plasma aldosterone levels were measured in 103 patients with coronary heart disease (CHD) having signs of various circulatory failures. The parameters were studied in relation to central hemodynamic values, and their dependence on the stage of chronic circulatory failure. Alpha-ANP was found to be one of the factors of the humoral systems implicated in the pathogenesis of chronic circulatory failure in CHD patients. The plasma level of alpha-ANP was found to be related to the state of the renin-angiotensin-aldosterone system both in healthy subjects and CHD patients.

[The effect of calcium antagonists on the hemodynamic function in patients with heart failure].

The therapeutic effect of calcium antagonists corinfar and phinoptin was studied in 48 patients with ischemic heart disease with chronic circulatory insufficient (CCI). The hemodynamic effects of these calcium antagonists were characterized by an increase of indices of cardiac contraction and essential reduction of the general peripheral resistance with simultaneous reduction of the left ventricle volumes during diastole and systole. It is concluded that corinfar and phinoptin are indicated in patients with CCI.

[Echo-controlled puncture biopsy of the liver].

The method of echo-controlled percutaneous puncture biopsy of the liver is described. Use of the method extends indications to diagnostic biopsy of the hepatic tissue, allows to realize aimed removal of material from the given portion of the organ and increases safety of the procedure. Therapists, gastroenterologists and other medical specialists are advised to use this method.

[Captopril in the treatment of refractory cardiac insufficiency].

The therapeutic efficacy of captopril, the inhibitor of the angiotensin-converting enzyme was studied in 18 patients with noticeable chronic cardiac insufficiency, who were refractory to foxglove drugs and diuretics. The use of captopril improved the patients' clinical status, significantly decreased the left ventricle volume and elevated the cardiac discharge. A simultaneous increase in diuresis during the day, favorable shifts in the electrolytic blood composition, a decrease in the concentration of blood plasma aldosterone and the level of carboxycatepsin in the blood serum were observed. Favorable changes in the hemodynamic and clinical-biochemical parameters during captopril therapy made it possible to achieve a positive therapeutic effect in 15 (83.3%) patients with refractory cardiac insufficiency. Subsequent recommendation of diuretics and maintenance doses of cardiac glycosides stabilized the therapeutic effect of captopril producing no complications. Thus, the incorporation of captopril in the management of severe refractory cardiac insufficiency was substantiated pathogenetically and found appropriate.

[The Berlipril and Retalzem treatment of hypertension patients: the dynamics of the clinico-hemodynamic indices and of the vasodilating humoral factors].

Submitted in the paper are data on clinical, hemodynamic, and neurohumoral effects of monotherapy with an inhibitor of the angiotensin-converting enzyme berlipril (enalapril maleate) and a calcium antagonist of the benzothiazepine series retalzem (diltiazem resinate) versus their combination therapy. The combination therapy offers a number of important advantages over monotherapy with the above-named agents, consisting in apparent hypotensive action and a greater activation of endothelium-derived vasodilating and antiaggregation substances.

[Central hemodynamic status in young patients with myocardial ischemia].

Results are submitted of investigations designed to study central hemodynamics and intracardiac kinetics in young patients with ischemic heart disease (IHD). In young coronary patients the course of the disease has been shown to be more often accompanied by the development of cardiac aneurism than in the elder ones in spite of the presence in the latter of more significant changes in their echocardiograms. Concomitant with aggravation of clinical manifestations of IHD (higher functional class of stable angina) there is an increase in the size of the left ventricle and decrease in the ejection fraction. It has been shown that hemodynamic parameters are closely related to both veloergometry indices and levels of cholesterol of high density lipoproteins APO-A-I.
