[Nonspecific brain systems and rehabilitation of children with cerebral palsy].

Basing on a long-term investigation of 157 children with spastic forms of cerebral palsy, regressive mathematical models of motor development have been worked out, taking into account a degree of severity and outcome of the disease. Mathematical patterns, reflecting dysneuroontogenesis of suprasegmental regulation system, are obtained. Complex electroencephalographic, electromyographic and vegetologic data demonstrated a role of limbic and reticular complex of intersystemic and intrasystemic disintegration in the reduction of adaptability and rehabilitation potential in patients with cerebral palsy. Basic concepts of adaptoregulatory approach to rehabilitation potential, basing on the necessity of nonspecific brain system correction for creation of neurodynamic suprasegmental and segmental adaptor energetic and adaptor immunoregulative premises for forming and providing specific psychomotor functions, are presented.

[Results of day polygraph study in children with spastic forms of cerebral palsy].

Integrative possibilities of non-specific brain systems of 30 patients with cerebral palsy (CP) aged 6-10 years were studied in the process of day polygraph examination including the record of EEG, skin-galvanic reflex and ECG both at physiological rest and at functional trials. 14 children had spastic diplegia, 8 children--hemiparetic form on the right side and 8 patients--on the left. The results obtained have indicated a systemic disneuroontogenesis of the middle structures mainly with the intrasystemic, intersystemic and interhemispheric manifestations of disintegration. It is concluded that regulatory conditions were created for forming a stable pathologic system maintaining disneuroontogenesis for strengthening of epileptic seizures during afferent stimulation and for considerable reduction of functional adaptation. That affects the compensatory abilities of the brain in patients with CP.

[The status of adaptive-compensatory mechanisms after the brain concussion in children].

80 children of preschool age were observed after concussion of the brain. In follow-up from 6 months 3 years clinical-anamnestic and neuropsychologic methods were used. The functions of autonomic nervous system were evaluated according to the results of estimation of initial autonomic tonus, autonomic reactivity, autonomic maintenance of the activity and the data of cardiointervalography. The state of nonspecific systems of the brain was studied with day time polygraphic research with a visual EEG evaluation, construction of histogramms by Fora method, estimation of speed and order of the fading of the components of a rough response. Psychovegetative stress was the ground for neurologic disorders in posttraumatic period. The disorders of adaptive regulation were manifested in exhausting of ergotropic systems with compensatory overstrain of trophotropic mechanisms, resulted in predominance of inhibiting influences with an inadequate response of synchronization on EEG during functional loads. Severity of clinical manifestations and disorders of adaptive regulation were more pronounced in children 1-3 year-old and with hypoxic-traumatic damages of the brain in anamnesis. Disintegration of cerebral activity correlated with the remote period. In the first 6-12 months after the concussion of the brain changes of vegetative reactivity prevailed, after 18 months the alterations in vegetative maintenance progressed.

[The adaptive systems of the brain in typical absence seizures in children during treatment].

Combined investigation of vegetative regulation in children with absences revealed overstrain of the sympathetic vegetative nervous system. This underlies the use of sinusoidal modulated currents (SMC) in multimodality treatment of the disease. SMC are applied on the cervical vegetative formations. In response to SMC treatment vegetative homeostasis noticeably improved as a result of a reduction of adaptive-compensatory overstrain.

[Night sleep in temporal-lobe epilepsy in children].

Polygraphy of night sleep performed in 31 children with temporal epilepsy showed enhanced nonspecific mesolimbic activation, probably of compensatory nature, in patients with common partial fits or complex partial fits. In wakeful patients with complex partial fits there were signs of growing influence of synchronizing inactivating non-specific structures.

[Comparative clinical characteristics of ischemic stroke with reversible and stable neurological deficit].

Fifty-four patients with minor and 54 with complete brain stroke were examined for the effect of the patients' age, nature of the underlying disease, its course, acuity of development, initial intensity, and successive dynamics of focal symptoms on the course of brain stroke. It has been discovered that the patients' sex or age, or the nature of the underlying disease do not produce any effect on the development of minor or complete brain stroke. The tendency towards decrease of the intensity of focal symptoms within the first 24 hours since the disease onset and early hospitalization of the patients was of paramount importance in cases with a complete regression of focal symptoms. The highest percentage of minor brain strokes was recorded if the patients were hospitalized within 12 hours since the disease manifestation.

[Epidemiology and risk factors of ischemic stroke in the Middle Ob region].

Based on the methods of the Brain Stroke Register, a study was made of epidemiology and risk factors (RF) of ischemic brain stroke (IBS) in one of the cities of the Middle Ob region, numbering 220 thousand population. Based on 615 cases of IBS, a noticeable rise of the disease incidence was revealed in a group of subjects aged 40-49 years as was a high level of the morbidity and mortality among persons over 60 years. Studies into RF discovered a high portion of arterial hypertension (75.1%), atherosclerosis (44.3%), aggravated heredity (28.6%), coronary heart disease (27.9%), alcohol abuse (17.9%). In 90.8% of cases, IBS developed in the presence of unfavorable fluctuations in atmospheric pressure, relative humidity, temperature, and the rate of air movement.

[Vertebrobasilar stroke].

As many as 112 residents of a town in the North of the Tyumen region were examined for the clinical course of vertebral basilar stroke (VBS). The latter ones occurred in 22.9% of patients with ischemic stroke, treated at the neurological hospital. The age of 66% of patients did not exceed 60 years. In 40% of cases, VBS developed within the first 5 years of the living in the Tyumen region and were dependent to a definite measure on the regional meteofactors.

[Autonomic regulation in the acute period of tick-borne encephalitis in children].

The clinico-epidemiological analysis and comparison of the data of a comprehensive study of suprasegmental vegetative regulation revealed an appreciable tension of the adaptation mechanisms in the acute period in 55 children with the meningeal form of tick-borne encephalitis (TE). Alterations in vegetative regulation in children suffering from TE were found to be coupled with impairment of the stem-hemispheric interrelations, reduction in the level of nonspecific activation, rise of convulsive preparedness of the brain. A correlation was established between alterations in the main parameters of variational cardiointervalography, character of brain bioelectric activity and the disease gravity. The data obtained can be employed as criteria for predicting the disease outcome and assessment of TE treatment in children.

[Autonomic innervation in newborn infants according to interval cardiography].

Altogether 48 full-term neonates were examined for vegetative supply with the aid of cardiointervalography performed at different stages of the consciousness-sleep cycle. It was established that the process of early adaptation was accompanied by the potentiation of the influence of the central contour of heart rhythm regulation. During feeding, a certain weakening of the adrenergic mechanisms was recorded. The cry was characterized by heart rhythm stabilization with an excess activation of the sympathetic part of the vegetative nervous system. Some characteristics of vegetative supply were defined on transition from sleep to being awake and during feeding in healthy children on days 1, 3 and 5 of life. The data obtained can be used for characterization of the vegetative homeostasis in full-term neonates.

[Mechanisms of the development of autonomic regulation disorders in neurogenic syncope in childhood].

Based on a clinico-anamnestic analysis and comparison of the data of comprehensive studies of vegetative nervous system functions it was discovered that in 85 children aged 4 to 14 years suffering from neurogenous syncopes, vegetovascular maintenance of the processes of activation and adaptation turned out insufficient. It is shown that in early childhood, the formation of vegetative dysregulation occurs under the influence of numerous factors, of which the most significant are hereditary predisposition to paroxysms, perinatal pathology, and psychogenic effects. It is established that there are two possible variants of the combination of pathophysiological alterations predetermining the clinical picture and phenomenology of neurogenous syncopes in children.

[Vegetative disorders in brain stem gliomas at different levels].

Sixty-six patients with gliomas of the cerebral trunk were examined clinically to reveal the features of vegetative disorders depending on tumor sites. Morphometric studies and mathematic simulation (made in 8 cases) of the time course of processes that occur in the cells of the paraventricular and supraoptic nuclei of the hypothalamus from their integral characteristics enabled one to notice the adaptation character of changes in the responsiveness of the hypothalamohypophyseal neurosecretory system.

[Vegetative disorders in the clinical picture of temporal lobe tumors].

Permanent and paroxysmal vegetative disorders were detected in 68,8% of the cases with temporal brain tumors. The intensity of the vegetative disorders was determined by intralobal localizations of the tumor, by the phase of the tumorous process and the histological structure of the neoplasm. Electrophysiological (rheographical) studies of cardiovascular changes detected in most cases marked to a varied extent regional cerebral and systemic disturbances of hemodynamics. Analysis of the cardiac activity demonstrated an increase in the physiological asynchronism of the systolic phases in the right and left ventricles, due to a pronounced disturbance of the right ventricle contractility.

[Regional-cerebral and systemic hemodynamic disorders in brain tumors].

The results of a combined electro-physiological study of cerebral circulation, peripheral circulation and cardiac activity in 100 patients with supratentorial tumors of the brain were examined. Regional-cerebral and systemic hemodynamic disorders increase with the development of intracranial hypertension and in a definite way depend on the histological structure of the new growth and the effect of the median brain structure.
