[Emergency heart surgery in the practice of the flight surgeon].

The authors analyze the materials from departments of emergency and advisory aid (sanitary aviation) over the past 20 years. Emergency care was rendered to cardiosurgical patients with cardiac and pericardial traumatic injuries in the surgical department of central district hospitals. They consider the reasons for a consultant's call, diagnostic aspects and board surgeon's organizational policy. The organizational policy in rendering cardiovascular patients an emergency surgical care consists in compulsory surgical interventions at the site of an accident. Patients with early postoperative complications should be evacuated to specialized departments. When the complications can occur in the absence of adequate management in the postoperative period in the central district hospitals the patients should be also evacuated to the specialized departments. Improving the qualification of a general surgeon is a basis for successful surgical aid rendered to patients with cardiac injuries.

[Results of gastrectomy with termino-lateral transverse gastroenteroanastomosis in peptic ulcer in patients over 60 years of age].

The authors analyse the material of a specialized gastroenterologic department of a regional clinical hospital and a department of sanitary aviation on rendering emergency and planned surgical aid to elderly and senile patients with gastric and duodenal ulcer. The immediate and late-term results of surgery were much better in resection of the stomach after Eck-Billroth-II with terminolateral transverse anastomosis in K.G. Komarchev's modification than in Billroth-II resection of the stomach in the Hofmeister-Finsterer modification. The peculiarities of the operative techniques leading to improvement of the immediate and late-term results of surgery for gastric and duodenal ulcer in elderly and senile patients allow us to recommend Eck-Billroth-II gastrectomy with terminolateral transverse anastomosis in K.G. Komarchev's modification for wider practical use.

[Surgical tactics in ulcer-related gastroduodenal hemorrhage in middle-aged and elderly patients].

Under analysis were immediate results of surgical treatment of ulcer disease of the stomach and duodenum complicated by profuse hemorrhage in 229 patients aged from 60 to 79 who were treated in a specialized gastroenterological department of a regional hospital and district hospital. It has been proved that better results of the surgical treatment of gastroduodenal hemorrhages of ulcerous etiology in elderly and senile patients are dependent on more active and earlier performing operative interventions.

[Organizational experience with polyclinic service for cancer patients].

The three organizational patterns of cancer patients' follow-up available within the framework of the Oncological Dispensary of the Ivanovo Region--the polyclinic of the Dispensary, the amalgamated oncological polyclinic and the "specialized" oncological polyclinic--are discussed. The rationale for maintaining a "specialized" oncological polyclinical service is presented.

[Multiple relaparotomies].

Under analysis was the material of the department of urgent and consultation medical aid (sanitary aviation) for the last 20 years. It was found that under conditions of surgical departments of central regional hospitals multiple relaparotomies made up 12.5% of all reoperations. It was shown that results of the multiple relaparotomies were dependent on urgent coming of a consultant and active surgical treatment on the spot. Reconstructive operations for intestinal fistulas must be performed under conditions of a specialized proctological department.
