[The debatable aspects of the adhesive syndrome].

The commissural intestinal obstruction and commissural disease are two variants of the commissural syndrome characterized by a complete or partial disturbance of the intestinal transit. The data of 390 clinical observations have shown that choice of methods of treatment must be based on the topical diagnosis of the syndrome such as the establishment of the place of obstruction and assessment of the degree of the intestinal transit disturbance. In the both cases a new roentgenological technique of investigation can be used with contrast labels. The new approaches to medical tactics allowed the time of diagnosing the commissural syndrome to become shorter and to considerably improve the direct results of the operative procedures.

[Study of "contrast labels"--a solution of the problems of early diagnosis of ileus caused by adhesions?].

The expediency of using a new radiocontrast "contrast label" method of examination was studied in 82 patients with suspected ileus caused by adhesions. The method is based on ascertaining the joining of the contrast labels at the site of the obstruction after their oral administration at an interval of one hour. The method is substantiated and described, and the indications are clearly formulated, the main one is suspected ileus. It is shown that the "contrast label" method is simple in techniques, informative, and has some advantages over X-ray examination with the prescription of a barium meal.

[Surgical treatment of lung cancer in elderly patients].

The immediate results of surgical treatment of pulmonary cancer in 217 elderly patients were studied. The dependence of the incidence of postoperative complications and lethality on the age, presence of concomitant pathology and extent of operative intervention was revealed. By means of the multifactor mathematical analysis, the decisive principles for individual prognostication of the course of postoperative period have been developed.

[X-ray contrast examination in surgery of adhesive intestinal obstruction].

The nature, frequency, and informative value of contrast X-ray examination of the gastrointestinal tract after oral administration of liquid barium in the adhesive disease and adhesive ileus were studied in two groups of patients (123 and 162 persons). Signs such as prolonged retention of the contrast medium in the small intestine and "sagging" of the intestine with the appearance of an X-ray contrast level in the absence of gases above it are of most diagnostic significance. Goal-oriented X-ray contrast examination made it possible to reduce the time needed for preoperative diagnosis, reduce the number of operative interventions in which the intestinal lumen is opened, and improve the immediate outcomes of treatment; mortality reduced from 19.8% to 10.5%.

[Regional microcirculatory disorders in peritonitis].

Investigations performed under experimental and clinical conditions have shown that regional microcirculatory alterations in peritonitis consist in the arterial spasm, arteriovenous shunt of blood, hypoperfusion of the intestine, local metabolic disorders. The application of alpha-adrenoblockaders is recommended for directed action on microcirculation with an obligatory correction of deficit of the circulating blood volume.

[Differential approach to restoring motor function of the intestines in peritonitis].

The authors have made a comparative estimation of the effectiveness of different methods of treatment of 330 patients with peritonitis with special reference to subdivision of motor disorders into 4 stages (compensated, subcompensated, decompensated stages and enteroplegia). It was established that sympatholytic drugs and continuous peridural anesthesia were effective in the compensated stage. Continuous intubation of the gastro-intestinal tract with the help of a rigid probe ensuring decompression in the postoperative period was the operation of choice in decompensated disturbances of the motor-evacuatory function.

[Pathogenetic therapy of motor disorders of the gastrointestinal tract in the postoperative period and in peritonitis].

The authors have established the relationship between disorders of the motor function of the gastro-intestinal tract and the reduced volume of the mesenterial blood flow under peritonitis and after laparotomy in experiment and clinic. The regional blood flow was studied angiographically, the intestinal motor activity was followed by recording the intestinal potential. Similar changes in the inhibition of motor functions were found after creation of a graded stenosis of the mesenterial artery and reduced volume of the blood flow. The employment of a number of vasoactive drugs is believed to be expedient for the purposeful correction of the impaired regional blood flow as a cause of the inhibition of motor activity of the gastro-intestinal tract.

[Method of intestinal decompression in treating functional intestinal obstruction].

The authors present an analysis of 149 cases of total decompression of the gastrointestinal tract in grave forms of functional intestinal obstruction accompanying peritonitis and acute intestinal obstruction. Special intestinal sounds made of polychlorovinyl were employed, that facilitates considerably the technic of intubation of the bowel as a whole. The authors data evidence that each method of intubation (transnasal, through gastro-or ileostome, etc) has its positive and negative aspects, therefore a selection of the site for introducing the sound should be conditioned by patient's age, the type of pathology and duration of the intubation procedure.

[Disorder of regional mesenteric circulation as a cause of functional intestinal obstruction].

It is shown experimentally that an impaired mesenterial blood circulation is of primary importance for development of dynamic intestinal obstruction, that is in contrast with the existing opinion on the primary importance of disorders in the bowel motility. The data obtained were utilized to explain the mechanisms of a stimulating effect of different procedures in combating against dynamic intestinal obstruction, namely an elimination of the suppressive sympathetic effect on abdominal vessels and an increase of the volume blood flow.

[Experimental study of selective mesentericography in acute intestinal obstruction].

Experimentally on 76 animals (dogs) the authors have studied some diagnostic opportunities of angiography in various forms of acute intestinal obstruction: strangulation, obturation and paralytic (in peritonitis) types. Angiographic symptoms of an organic origin proved to be mainly typical for strangulation obstruction. Symptoms of a functional character reflecting disorders in mesenteric hemocirculation were found to be equally pronounced in all kinds of intestinal obstruction, these consisted in a retarded linear rate of arterial and venous blood flow, in particular.
