[Activity of 22 antibacterials against O1 and O139 serogroup Vibrio cholerae strains isolated from humans within 1927-2005 in various regions of the world].

Analysis of antibioticograms of 390 O1 and O139 serogroup Vibrio cholerae strains isolated from humans within 1927-2005 in various regions of the world showed that the strains of V. cholerae isolated within 1927-1966 were susceptible to 22 antibacterials, the strains isolated within 1938-1993 possessed 1-3 resistance markers and the strains isolated within 1994-2005 had 3-8 resistance markers including resistance to fluoroquinolones. All the strains of O139 serogroup V. cholerae isolated in 1993 and 1994 possessed 3 resistance markers. Studies on albino mice with generalized experimental cholera due to the V. cholerae eltor 1 strain (P-18826, 2005) isolated from a cholera patient, which was highly resistant to nalidixic acid, streptomycin, ampicillin and trimethoprim/sulfamethoxazole and showed cross resistance to fluoroquinolones (ciprofloxacin, ofloxacin, pefloxacin and norfloxacin) and moderate resistance to ceftriaxone and cefotaxime, revealed that the only efficient antibiotics were tetracyclines and aminoglycosides (except streptomycin). The investigation demonstrated an extension of the antibiotic resistance spectra of the epidemically significant strains of the cholera pathogen and the necessity of using antibacterial drugs in strict accordance with the antibioticograms in emergent prophylaxis and therapy of cholera and immediate replacement of the drug by a more active one.

[Retrospective VNTR-analysis of genotypes of Vibrio cholerae 01 strains isolated, during the 7th cholera pandemic, in Rostov Region].

Antiplague Research Institute, Rostov-on-Don, Russia Retrospective multi-locus VNTR-analysis was made for 166 Vibrio cholerae strains isolated, 1967-2001, in Rostov Region from clinical samples (82 strains) and from water samples (84 strains). On the basis of cluster analysis of heterogeneous identification strain genotypes, 45 variations of individual strains were shared between 11 separate clusters, among which the F cluster vibrios were predominant. Having emerged, 1970, in the region, they were widely spread during the 1973-1975 cholera pandemic and were registered, among the isolated strains, till 1992 indicating the possibility of long persistence of V. cholerae 01 in the natural aquatic environment. Presumably, the ecosystem specificity contributed to the long-term vibrio persistence.

[Experimental resistance of Vibrio cholerae el tor to nalidixic acid and fluoroquinolones].

It was shown that sensitivity of Vibrio cholerae eltor P-5879 to tetracycline, levomycetin, furazolidone, trimethoprim/sulfamethoxazole, aminoglycosides, beta-lactams, rifampicin, quinolones in vitro correlated with drugs efficacy in the treatment of experimental cholera of albino mice. Mutants of V. cholerae eltor P-5879 Nalr resistant to nalidixic acid (MIC 160-200 mg/l) formed with frequency 10(-9)-110(-8) had no cross resistance to fluoroquinolones. But the efficacy of ofloxacin, lomefloxacin, norfloxacin against these mutants in vivo reduced, though it was not changed in vitro. Mutants of V. cholerae eltor P-5879 resistant to fluoroquinolones and selected after culturing in the presence of the drugs had cross resistance to all quinolones studied. Infection caused by Cpfr mutant could not be treated with nalidixic acid and fluoroquinolones, therapeutic efficacy of rifampicin and beta-lactams, also reduced though sensitivity in vitro was not changed. The results of investigation proves the necessity of quinolones use for cholerae treatment as it is recommended for other severe enteric infections.

[Ultrastructural changes in the interstitial cells of the kidney medullary substance in suckling rabbits with experimental cholera].

A culture of virulent selection of cholera vibrios L-top 5879 was introduced through the probe to suckling rabbits-pups 10 to 12 days old. Ultrastructural changes of interstitial cells and capillaries of kidney medulla were studied. During vibrio adhesion (4 hrs after the inaction) interstitial cells acquire dystrophic changes, lipid granules content reduces, while vascular permeability grows higher which suggests the presence of prostaglandin precursors elimination into blood flow. Cholera development (1-2 days later) is accompanied with progressing of signs of prostaglandin synthesis activation and their precursors passage into the vascular bed.

[The effect of myelopid on the ultrastructural changes in the rat myocardium induced by an endotoxin].

Ultrastructural changes in the myocardial atria and ventricles have been studied in experiments on rats in the intermediate period of the endotoxin shock, on the stage of late endotoxemia and its correction with myelopid. It was established that 5 hours after endotoxin injection contractile disturbances and intracellular myocytolysis take place and growth of secretory activity is observed in atrial cardiomyocytes. Simultaneously vascular permeability increases. Only the endocrine heart function is restored 3 days later, while dystrophic changes and glycogen depletion are maintained. The myelopid administration beginning from the intermediate period of the endotoxin shock and then once during 3 days, removes most of ultrastructural damages or attenuates their expression.

[Ultrastructural study of transepithelial pinocytosis induced by endotoxin in the distal section of the nephron].

In initial and intermediate periods of endotoxin shock in rats and dogs significant micropinocytosis in the epithelial cells of distal tubules and collecting ducts are observed. On the stage of late endotoxemia in rats transepithelial pinocytosis disappears. Existence of correlative connections between ultrastructural changes in neurosecretory system and epithelial cells responsible for facultative reabsorption in different periods of experimental endotoxemia have been shown.

[Ultrastructural changes in interneuron connections in intramural ganglia of the small intestine in suckling rabbits with experimental cholera].

Data, received in investigation of the small intestine intramural ganglia of 34 suckling rabbits show, that development of experimental cholera induce in the neurons reactive shifts, changed by dystrophic alterations. Prevaluation of cholinergic influences in autonomic ganglia in small intestine promotes enterocytes hypersecretion and accelerating of motor activity.

[The 7th cholera pandemic in the world and the USSR].

1,713,057 cases of cholera were registered in the world during the seventh pandemic of the disease at the period of 1961-1989. The pandemic still continues, being the most prolonged pandemic in comparison with earlier ones. During the period of the seventh pandemic 10,723 cases of cholera were registered in the USSR. Great outbreaks occurred in 1965 and 1970-1974. At present sporadic cases of cholera can be registered, and wide circulation of mainly avirulent, nontoxigenic strains of cholera vibrios in environmental objects is characteristic of the epidemic situation.

[The practical performance of cholera control measures in the USSR during the 7th pandemic].

The critical analysis of anticholera measures carried out in the USSR since 1965 till the present time is presented. The grounds for the abolition or considerable reduction of a number of measures are considered from the viewpoint of their scientific substantiation, the adequacy of means and efforts spent for their realization and their antiepidemic effectiveness. Special attention is paid to the necessity of differentiated approach to these measures, depending on concrete local climatic and geographical, sanitary, hygienic and other factors which determine the epidemic potential of a given administrative territory.

[The types of the epidemic manifestations of cholera on the territory of the USSR].

On the basis of the analysis of cholera cases for the period of 1965-1989 three main main types of epidemic manifestations of this infection on the territory of the USSR were determined with due attention to the complex of data, characterizing the intensity and types of the epidemic process, the danger of the outbreak and spread of cholera. This made it possible to differentiate and decrease the complex of prophylactic measures, depending on the type of the territory.
