[Effect of antibiotics on the absorption of basic nutrients].

The impact of tetracycline, gentamicin, streptomycin, erythromycin, oxacillin, benzylpenicillin and sulfanilamides on the absorption function of the small intestine was studied in 79 patients with chronic bronchitis. The drugs were shown to mainly inhibit fat absorption: on the average, by 87 per cent in all the patients treated with tetracycline, on the average, by 89 and 36 per cent in 80-70 per cent of the patients treated with gentamicin and streptomycin, respectively, and, on the average by 26 and 16 per cent in 20-25 per cent of the patients treated with erythromycin and oxacillin, respectively. The absorption of protein decreased, on the average, by 23-74 per cent in all the patients treated with tetracycline, gentamicin and streptomycin. As for the other drugs, they had, on the average no significant effect on protein absorption. The absorption of carbohydrates (d-xylose) significantly decreased under the effect of these three drugs (by 21, 34 and 36 per cent, respectively). The analysis revealed a relation of absorption function to the presence and degree of intestinal dysbacteriosis.

[Regional lung perfusion in patients with chronic nonspecific lung diseases based on scintigraphic study data].

Perfusion scintigraphy of the lungs with 99mTc-labeled microspheres was used for a study of changes of regional perfusion of lesser circulation in patients with chronic bronchitis of a varying degree of disturbed pulmonary hemodynamics. Even before a stable rise of pulmonary vascular resistance and development of pulmonary arterial hypertension, chronic bronchitis was characterized by redistribution of the blood flow from the lower parts of the lungs to the upper ones, which progressed with disturbance of pulmonary arterial circulation and the development of chronic cor pulmonale. This redistribution could be determined by an earlier and more important rise of the vascular tone and resistance to the blood flow in the lower parts of the lungs. The use of scintigraphic indices of the upper and lower parts (U/L) perfusion ratio led to adequate assessment of a degree of redistribution of the pulmonary blood flow in patients with chronic bronchitis.

[Chart-algorithm of early etiological diagnosis of acute pneumonia].

A proposed method of scheme-algorithm is aimed at facilitation of an early etiological diagnosis of acute pneumonia (AP) by initial clinical signs. A blind trial with 78 AP verified cases showed that the scheme can establish a correct etiological diagnosis basing on AP early clinical symptoms alone in 75% of cases. The sensitivity of the method is 75%, specificity 91%.

[Etiologic structure of exacerbations in chronic bronchitis].

The etiological appearance of exacerbations of chronic bronchitis was investigated in 117 patients. A combined study was aimed at the detection of viral, mycoplasmal, fungal and bacterial endobronchial infections. The etiological role of the latter was verified by a pathogen isolated from the sputum. Exacerbation of chronic bronchitis was determined by bacterial (79%) and/or viral (55%) infections, mostly by pneumococcal infection (45%), H. influenzae (30%), and influenza (31%). The role of pyogenic cocci was minimal (2%). Exacerbation of chronic purulent endobronchitis was more frequently caused by H. influenzae than in catarrhal bronchitis. In 9% of the patients the infectious etiology of an inflammatory bronchial process could not be established.

[Effect of intratracheal administration of gentamycin in exacerbations of suppurative bronchitis due to gram-negative bacteria and Staphylococcus].

The effect of intratracheal administration of gentamicin (G) was observed in 22 patients in exacerbations of chronic purulent bronchitis caused by gram-negative bacteria and staphylococcus. A high concentration of gentamicin quickly occurred in the sputum, it always exceeded a minimum concentration suppressing the growth of an agent isolated from the sputum but its absorption from the respiratory tracts was poor: in half of the patients it hardly penetrated the blood, in the rest of them its concentration in the blood was insignificant. In order to potentiate the therapeutic effect in some of the patients, penicillin or gentamicin administration should be continued intramuscularly. The duration and type of antibacterial therapy must be based on the results of bacteriological investigation of the sputum in the course of treatment.

[2-stage treatment of acute pneumonia patients with rehabilitation at the sanatorial stage].

A total of 73 patients with acute pneumonia received two-stage treatment: etiotropic treatment based on early etiological diagnosis (at hospital) and subsequent early rehabilitation at sanatorium with the use of peloid therapy. It made it possible to attain the best short-term results in the treatment of pneumonias (as compared with control group patients), to shorten 2-4-fold the times of the patients' stay at hospital and to raise bed capacity, to reduce the total doses and duration of antibacterial therapy, to decrease the possibility of the allergic reactions and side effects, and to reduce 2-fold the cost of antibacterial therapy.
