[The role of endoscopy in diagnosis and treatment of gastroduodenal bleedings].

The experience in diagnosis and treatment of 469 patients with gastroduodenal ulcer bleedings is presented. Sensitivity of endoscopy in detection of bleeding source was 98.8%, in detection of ulcer size -- 93.4%, specificity -- 80.6%. It is demonstrated that the most frequently recurrence of bleeding occurs when ulcer size is more than 1 cm. Location of ulcer on posterior, posterior-superior and posterior-inferior walls of the duodenal bulb is an unfavorable prognostic symptom (rebleeding occurs in 17.1, 30.0 and 12.5% cases, respectively). Types Forrest-Ia and Forrest-IIa are especially dangerous for recurrence (21.4 and 15.6%, respectively). In types Forrest-Ib and Forrest-IIb rebleeding occurs less frequently (4 and 6%, respectively). In types Forrest-Ia and Forrest-Ib rebleeding occurs mainly on the first day, rebleeding in types Forrest-IIa occurs more often on the first-second day, however it is possible up to day 7. Rebleeding in types Forrest-IIb occurs also mainly during the first two days. Endoscopic hemostasis (electrocoagulation and injection therapy) was performed in 40 patients. In 6 (15%) cases hemostasis was not achieved. In 8 (20%) cases endoscopic hemostasis led to a temporary effect, i.e. rebleeding was seen within 24-72 hours after endoscopic procedure. Stable hemostasis with endoscopy was achieved in 26 (65%) patients.

[Nutrition of patients operated for complicated gastroduodenal ulcers at the early postoperative period].

The investigation is based on an analysis of results of treatment of 102 patients after an emergency resection of the stomach for complicated gastroduodenal ulcers. The probe was inserted into the jejunum intraoperatively. The period of enteral probe nutrition (EPN) was 5-7 days. At the early postoperative period in patients who had undergone the emergency resection of the stomach EPN is an effective method of nutrition maintenance as compared with the infusion-transfusion therapy. The method is simple and open to general use, and is economically advantageous under conditions of any hospital.

[Surgical tactics in bleeding duodenal ulcers].

The article gives a review of literature on the determination of surgical tactics in patients with duodenal ulcer complicated by bleeding. The authors are supporters of the actively-temporizing tactics. They claim that the period of the delay in the decision should not exceed 24 hours and depends on the intensity of the bleeding, the degree of blood loss, the location and nature of the ulcer, the patient's age, and the concomitant diseases. They believe resection of the stomach to be the operation of choice in bleeding duodenal ulcer. One of the authors suggested a modified method of gastric resection in "difficult" duodenal ulcers which excludes the occurrence of incompetence of the duodenal stump sutures.

[Electrocoagulation in the arrest of gastrointestinal hemorrhage].

The curative endoscopy with diathermocoagulation was performed in 100 patients with acute gastro-intestinal bleedings. Peptic ulcers were the cause of the bleedings in most patients (64). The complete arrest of hemorrhage was achieved in 75 patients. Recurrent bleedings at different terms were noted in 25 patients. Patients with critical degree of blood loss (in repeated bleedings) must be subjected to urgent operations without loosing time for a repeated attempt to stop the bleeding. Urgent operations were made on 19 patients.

[Incidence and characteristics of complicated forms of gastric ulcer].

The frequency and certain specific features of compound forms of the gastric ulcerous disease were studied in 762 operated patients. The most frequent causes of operations for the gastric ulcerous disease were stenosis (28, 3%) chronic ulcers (25%), perforation (21.7%), hemorrhages (14.4%), penetrations (10.6%). Malignization of ulcers was noted in 15% of patients. A complex investigation of the secretory function of the stomach allowed distinguishing two groups of gastric ulcers accompanied by hypo- or hypersecretory syndrome.

[Bleeding gastric ulcer].

Under study were 113 patients with hemorrhagic ulcer of the stomach. It was established that gastric ulcers were the causes of hemorrhage from the upper parts of the gastrointestinal tract in 9,3% of cases. Hemorrhagic ulcers of the stomach were the cause of hemorrhages of ulcerous etiology in 22% of cases. Predisposition of gastric ulcers to profuse hemorrhage, high percentage of their degeneration makes such radical surgical intervention as stomach resection very expedient.
