[Direct recording of the electrical activity of the sinoatrial node].

The electrical activity of the sinoatrial node was recorded simultaneously from the epicardial and endocardial surfaces of the sinoatrial node in 73 dogs. The electrical activity of sinoatrial node had the form of a slow deflection and a rapid preatrial deflection, the duration of the former being 81 +/- 28 msec and reflecting slow propagation of excitation within the sinus node. Duration of the rapid preatrial deflection was 17 +/- 11 msec. It reflects the rapid propagation of the sinus impulse (probably, the rapid spread of the excitation in corona of the sinus node and invasion in crista terminals). The form and duration of the slow and rapid deflections changed under the influence of drugs.

[Parameters of the bioelectrical activity of the heart conduction system in virtually healthy persons].

Intracardiac conduction was studied on the basis of the conception of the atrial spike potentials. Intracavitary electrograms were recorded from the right atrium, zone of the bundle of His, and the esophagus in 110 persons in 35 of whom no abnormalities were found in the heart and the results of the examination were accepted as control indices. On the grounds of the investigations conducted by them, the authors determined the shape, amplitude, and time of appearance of the spike potentials of the sinus rhythm, atria, and the bundle of His and also the duration of the intervals of intracardiac conduction. Study of the phenomena of bioelectric activization of the atria and the system of the bundle of His in a complex significantly widens the possibilities of selective assessment of disorders of intracardiac conduction at various levels of propagation of the stimulation.

[Relationship between the spike potentials of "local" intra-atrial and esophageal electrocardiograms of the atria and notched P wave of ECG from the body surface].

The interrelationship of spike potentials of "local" intraatrial and esophageal electrograms and notched P wave of an intensified ECG recorded from the body surface was studied. It was found that the process of excitation in the region of the sinus node is reflected in the slow deviation of the P wave of intensified ECG, preceding the appearance of the P wave of an ordinary ECG recorded from the body surface. The A1 spike of a "local" electrogram reflecting the penetration of excitation into the right atrium is manifested in the form of the first notch of the P wave on an intensified ECG. The A2 spike reflecting the arrival of the excitation to the atrioventricular node is manifested in the form of a notch--the first peak of the P wave. The A1(2)) spike reflecting the arrival of the excitation to the left atrium along Bachmann's bundle is manifested as the second peak of the P wave (A1(2)). The A3 spike of the "local" electrogram reflecting the arrival of the excitation from the left atrium to the orifice of the coronary sinus is manifested in the form of the second notch of the P wave (A3).

[Changes in sinoatrial node function in ischemic heart disease].

Seventy-nine patients with ischemic heart disease were examined. Weakness of the sinus node was suspected in 32 patients according to the clinical findings and the ECG; diagnostic atrial stimulation was conducted. Sinoatrial node potential was registered in the form of a spike in patients with ischemic heart disease. Direct stimulation from the region of the sinoatrial node was performed. The stimulator impulse was followed by a distinct potential of the sinoatrial node in the form of a spike which was continuous with the wave of atrial depolarization. The time of sinoatrial conduction (from the onset of the sinoatrial) node potential to the first atrial spike) was determined in 47 patients with ischemic heart disease. It was determined that under the effect of nitroglycerin the time of sinoatrial conduction becomes significantly shorter (reduced from 29 +/- 10.6 to 20 +/- 8.3 msec).

[Interpretation of a bundle of His electrogram (using the spike potentials of the sinus node and atrial conduction tracts in the analysis)].

Spike potentials of the sinus node and of the atrial conductivity pathways were recorded by way of heart microcatheterization via the subclavian vein in 40 canine experiments and in 307 patients, using the obtained data for a more precise interpretation of the bundle of His electrogramme taken in the course of these examinations. The importance of the density of the spikes for their successful recording was studied, and it was found that the onset of the atrial A--wave preceeded that of the P-wave by 10-30 ms on ECG tracings recorded from the body surface. The atrial A--complex recorded with the aid of an intracardiac unipolar electrode from the right atrium was found to consist of a slow depolarization wave of the atria and of three spike potentials (A1, A2 and A3). The A2 spike coinsides with the peak of the P-wave in the II lead and reflects the moment of the impulse's arrival at the atrioventricular node along the Bachman and Wenkebach's tract, while the A3 spike coinsides with the termination of the P-wave in the II lead being dependent on the depolarization of the Torel's tract. The P--A interval should be measured between the sinus spike and the A2 spike, while the A--H interval--between the A2 spike and the H-potential.
