[Reinnervation of a skeletal muscle after its directed injury].

The connection of the motor nerve stump with a muscle permits to achieve the definite standard of the muscle structures reinnervation, but this process is limited the nerve localization zone. Morphological substantiation of the muscle reinnervation method in experiment is proposed, including the fascicules preparation and accommodation of the nerve-neurotizer in a muscle depth, the neurotropismus stimulation using the directed damage of the muscle tissue. The method was tested in clinic in 9 patients.

[Restorative microsurgery in severe injury and traumatic amputation of the distal portions of the extremities].

Application of microsurgical methods and free grafting of the compound complexes of tissues with the vessels and nerves in particular with microanastomoses permitted to achieve high functional results in traumatic amputation of extremities in contiguity, severe destroying traumas and their consequences. Experience of 1000 urgent microsurgical operations, including 638 replantations of digits and their blocks for various kinds of amputation on the palm level was summarized. Classification of amputation, taking into account the injury mechanism and character, promoting the choice of optimal tactic and technique of restoration operation was elaborated. Indication for replantation are substantiated, the results of restoration operation, complications occurring after large segments of extremities replantation and measures for their treatment are analysed. Definite success of operation is determined by the extent of first operation and by subsequent rehabilitation measures conduction including the correcting operations and various conservative methods.

[Anatomical basis of optimal suturing on the cross-striated skeletal muscle].

Surgical procedure of the transversely cut skeletal muscle joining, taking into account the location of stump and the motor nerve stumps on the cut off, was proposed, based on the microsurgical anatomy of the motor nerves fragments of skeletal muscles data and on personal experience of the upper extremity large segments replantation and musculocutaneous flaps transplantation. The useful restoration of the muscle motor activity, estimated clinically and instrumentally, was obtained in more than 83% of observations.

[Radiodiagnosis of the thoracic outlet syndrome].

The results of roentgenological investigations in thoracic outlet syndrome are summarized. The method for defining the width of a costoclavicular cleft and its diagnostic importance in 72 patients operated on is described. A role of anomalies of the osteofibrous formations in the development of neurovascular compression is noted.

[Use of external brachial cutaneo-fascial flap for plastic microsurgery of defects of functionally important areas of the hand and foot].

On the basis of morphological investigations on 20 non-fixed cadavers, the microsurgical anatomy of the external fasciocutaneous brachial flap was detailed . A flap was successfully used for plasty of the tissue defects of the foot, forearm and hand in 14 patients. The recommendations concerning the technique of flap elevation and transplantation are given.

[Natural electrical activity of human sympathetic nerve fibers recorded from the body surface].

The electrical potentials recorded from human forearm were stored and averaged as synchronous with the cardiac rhythm. The R-wave of the ECG was used to synchronize the averaging system. The record obtained with this technique from human forearm during transient stop of blood flow to avoid mechanical artifacts was a sinusoid-like curve with peaks between the R-waves of the ECG. The nature of this electronegative wave was investigated by means of comparing electrograms and rheograms recorded from forearm of normal subjects and subjects after denervation or desympathization of their upper extremities. The data obtained suggest that the electronegative wave recorded with this technique is the averaged sum of tonic pulse-synchronous electrical activity of sympathetic nerve's fibers innervating vasoconstrictors of the skeletal muscle.
