[The pathology service in medical insurance].

Stabilization of economy is since qua non of the efficient medical insurance the transition to which should be gradual. One of the most important conditions of successful functioning of the insurance companies is a control of treatment and diagnosis in which a leading role belongs to the pathologist. Hence, the pathology service should be independent (creation of bureaus of pathology and institutes of pathology). Qualified experts in pathology should be trained by 4-year specialization.

[Analysis of training pathologists on the basis of an educational-research-practicum complex].

The experience of the work of the educational-research-practical association (chair of pathology-pathologo-anatomical bureau) is analyzed. It is believed that organization of such complexes aimed at integration of educational, scientific and practical activity is one of the most effective ways of reorganization of the system of higher medical education and health service. Working within the complex permits a rapid improvement in the quality of education, effectiveness of scientific work and practical activity in pathological anatomy. A conclusion is made that one of the priorities of reorganization of the system of health care is realization of the principle of centralized management and technical supply of these complexes.

[The microcirculatory system and smooth muscle cells of major arteries in various rheumatic diseases].

The microcirculatory bed (MCB) and smooth muscle cells (SMC) of the main arteries were examined in subjects who had died from rheumatic disease (33 cases), systemic lupus erythematosus (n = 28), and systemic sclerosis (n = 8) at the age of 12 to 69 years. Adventitial film preparations impregnated with silver nitrate by the method of Kupriyanov and other techniques were applied to comparatively characterize pathological and adaptive changes in MCB of vasa vasorum. In each disease, there were impaired smooth cells along with formed SMC deficit that was mostly pronounced in lupus. The medial SMC were shown to contribute to the degradation of the basic substance and reparative and synthetic processes. It was found that the immune complexes might be accepted both by the vasa vasorum system and the luminal surface of large arteries.

[Erdheim's disease: morphogenetic problems].

Structural studies of resistive vessels in a 21-year-old woman who died in the postpartum period from rupture of the aorta in the presence of Erdheim's disease are described. Histological, histochemical, and immunomorphological methods were used, and microcirculation was examined in film preparations of adventitia treated with silver nitrate by Kupriianov's method. In the media of the aorta and the proximal parts of its large branches, nonspecific necrobiotic processes were in evidence, believed to be due to lesions in the microcirculatory bed of the vascular wall.

[Nodular fibrohistiocytosis of the gallbladder].

A case of nodular fibrohistiocytosis with tissue eosinophilia located in the submucous layer of the gallbladder in a woman of 42 is presented. Similar cases have been described so far only in the stomach. Clinically there existed conditions for the immune system disturbances (bronchial asthma).

[Lesions of the central and peripheral nervous systems in rheumatoid arthritis].

Using morhological, neurohistological and histochemical methods the author studied different areas and anatomical structures of the central and peripheral somatic and vegetative nervous system in 4 patients who had died during different periods of rheumatoid arthritis at the age of 27, 48, 51, and 60. The studies demonstrated symptoms of disorganization of the connective tissue in the sheaths and walls of vessels (mainly in the microcirculatory bed), lymphoid histocyte infiltrates, rheumatoid nodules and nonspecific changes of neurocytes and conductors. The paper contains a description of the main form of lesions in rheumatoid, arthritis of the nervous system.

[Affection of the central and peripheral nervous systems in systemic lupus erythematosus].

Using the complex methods of morphological, neurohistological and histochemical methods in the study of the central and peripheral nervous system in systemic lupus erythematosus, the authors observed 20 patients who died at the age of 12-56. The studies detected phenomena of disorganization of the connective tissue and vascular walls which are accompanied by a significant accumulation of muccopolysaccharides, thrombovasculites, endovasculites, lymphoid-plasmocellular infiltration and degenerative changes in the nervous elements.
