[Pharmacological cost-efficacy of various schemes of combined antihypertensive therapy in elderly patients].

AIM: To validate optimal cost-effective pharmacological policy in combined therapy of elderly hypertensive patients. MATERIAL AND METHODS: In 150 patients of a day hospital aged 55-74 years with at least stage II arterial hypertension of 4-40 years of duration a study was made of the cost efficacy of their combined treatment. The patients received lisinopril, metoprolol, nifedipin, enalapril, hypothiaside, indapamid. Clinical efficacy was assessed by the following criteria: a fall in systolic arterial pressure to at least 139 mm Hg or at least by 20% of the baseline, a fall in diastolic pressure to at least 89 mm Hg or 10% of the baseline. Economic parameters included cost of the disease, cost/effect values. RESULTS: Combination egilok+hypothiaside was the cheapest but the least effective. Diroton+indap combination was cheaper than less effective combination cordipin-retard+hypothiaside and renitek-hypothiaside. Pharmacological and cost efficacy of diroton+indap combination was the highest. CONCLUSION: Combination of lisinopril with indapamid was most adequate as to its cost and effect and can be used as a medication of the first line in elderly hypertensive patients.

[A morphological assessment of periodontal tissue status in the postshock traumatic period in an experiment].

Shockogenic sublethal mechanical multiple injuries were induced in outbred albino rats according to the method developed by Noble-Collip and periodontal tissues were examined histomorphologically in various periods after shock. Atrophic changes in periodontal tissues and dental pulp progressed starting from day 1 and reached the maximum on day 28. The greatest shifts were observed in the gingivodental epithelium (hyperkeratosis), periodontium (lymphocyte and macrophage infiltration), alveolar bone (lacunar resorption), and pulp (odontoblast reduction and decreased pulpocyte count). Phasic reactive changes in the periodontium after shock and trauma require timely correction.

[The involvement of the components of the immune system in iron deficiency].

As exemplified by iron deficiency anemia, the study was made of effect of iron deficiency on interdependence of immunity parameters. It is noted that patients with definite forms of the anemia showed less numerous correlations than in controls. The emergence of complications increased the number of persistent correlation pleiad compared to controls. Moreover, correlation and variance analyses suggested a correlation between immunity and iron deficiency parameters.

[Immunologic effect of hemosorption in patients with acute peritonitis].

An examination of 65 patients with acute diffuse peritonitis has shown that pronounced T-immune deficiency is observed in the postoperative period of acute peritonitis. Hemosorption was shown to be efficient in patients with high content of toxin which allows considering the method of extracorporeal detoxication as effective means of immune-correcting therapy in peritonitis patients with high level of toxinemia who make up a group of risk.

[Adaptation reactions in acute peritonitis and their correction using sorption detoxification].

Examined were 233 patients with acute peritonitis. In patients, who underwent the conventional intensive infusion therapy alone, in complicated course of the postoperative period, within 28 days, the high level of pathologic stress reactions was observed, which correlated with changes in the content of middle molecular mass peptides (MMMP). In uncomplicated course of the postoperative period, the MMMP level and adaptation reactions didn't differ from the control findings. Hemosorption in patients with high toxin level was accompanied by the decline in MMMP concentration below the diagnostic level, restoration of physiologic adaptation reactions.

[Importance of leukograms for the evaluation of nonspecific body resistance in patients with iron-deficiency anemia and prognosis of the course of the disease].

The possibility of stage evaluation of body resistance in patients with iron-deficiency anemia (IDA) was studied by isolation of adaptation reactions basing on the leukogram analysis. The results of the studies have permitted suggestion that changes in leukograms of IDA patients can be used for evaluation of non-specific body resistance. A detailed analysis of the leucocytic formula enables prognosing the course of the disease and can be used as an additional criterion of the evaluation of the conducted therapy effectiveness.

[Immunologic reactivity and sorption detoxication of patients with acute peritonitis].

Follow-up using immunological monitoring of 233 acute peritonitis patients revealed various immunological shifts occurring at all levels postoperatively, with the therapeutic methods being common. Depression of specific immune response mechanisms (T- and B-systems) along with disinhibited phylogenetically old forms of the common biological phenomenon non-specific immunity (phagocytosis) is believed to reflect the common adaptive mechanism activated in emergencies in patients with peritonitis. With high levels, the toxin median molecular mass polypeptide in the complicated postoperative course, the immunological impairment is more pronounced, while restoration of immunological responsiveness of the organism becomes protracted, resulting in reduction of the cellular homeostasis and metabolism reserved. Application of the extracorporeal detoxication-hemosorption promotes adaptation buildup and makes immunity functioning optimal.

[Mitotic activity of rat bone marrow cells during injury].

The mitotic regimen analysis in rat bone marrow cells was conducted 1, 3, 7, 14, 21 and 35 days after shock trauma. A sharp impairment of myelocyte reproductive function was registered against an increase in the mitotic index. It confirms a universal character of normal mitosis impairment in strong stresses, which was earlier established for epithelial tissues. Cell division disturbances in the bone marrow may be considered an pathogenic factor of a number of pathological processes occurring in the blood system in traumatic disease (anemia, immunodepression). A complex of drugs (sodium oxybutyrate, sodium oxiferriscarbon, Laevamizolum) is offered for the correction of proliferative processes in the bone marrow. This complex has no significant influence on mitotic index and causes relative reduction of pathological mitosis level, ensuring its earlier normalization.

[Clinico-immunological parallels in patients with iron-deficiency anemia].

The main parameters of humoral and cellular immunity as well as indices of nonspecific body resistance were studied in 70 patients with iron deficiency anemia. A decrease in the content of the main classes of immunoglobulins and an increase in the CIC level were noted. Routine therapy brought about a tendency toward the restoration of indices of humoral immunity. With respect to cellular immunity, a relative increase in the level of B-lymphocytes and a decrease in the number of 0-lymphocytes were observed. A relative and absolute decrease in the level of T-helpers was revealed. Certain correlation between some clinical and immunological indices was observed.

[Effect of hemosorption on the level of medium molecular mass polypeptides and humoral immunity in patients with peritonitis].

An examination of 43 patients with acute diffuse peritonitis of various etiology has revealed different direction of the dynamics of indices of humoral immunity (M, G, A immunoglobulins, general complement activity, circulating immune complexes) depending on the level of middle mass polypeptides (MMP) in the patient's blood. Hemosorption in peritonitis was established to allow rapid and effective elimination of MMP in patients with high content and thus control critical endotoxicosis.

[Insulin-depositing function of peripheral blood erythrocytes in rats that have sustained severe craniocerebral injury].

Rats with a grave craniocerebral injury were examined over time for the content of insulin in red cells (1, 3, 7, 14, 21 and 35 days after injury). Alterations were found in the affinity of circulating red cells for paraaldehyde fuchsin (PAF), linked primarily with a decrease in the proportion of the PAF-resistant forms of the cells. It is suggested that in general the data obtained reflect the intensification of the receptor function of the red cell membrane.

[Prognostic significance of changes in blood morphology in severe trauma].

The examination of 121 patients with traumas of adaptation reactions by morphological parameters of the white blood has shown them to be indices of non-specific resistance of the organism elucidating the degree of the compensatory-adaptative processes. The parameters in question may be used as subsidiary criteria for prognosis of the course of the traumatic disease.
