Clinical covariates of abnormal heart rate turbulence in coronary patients.

BACKGROUND: The aim of this study is to evaluate the association between heart rate turbulence (HRT) parameters and clinical characteristics of coronary artery disease (CAD) patients. METHODS AND RESULTS: In 122 patients (mean age 62 +/- 9 years) with angiographically documented CAD, 24-hour Holter monitoring with HRT analysis was performed to evaluate turbulence onset (TO) and turbulence slope (TS). There was a significant correlation between TO and TS (P =-0.31; P < 0.001). According to quartile values, TO >or=-0.37% and TS 60 years (OR 1.27; P = 0.002) and EF < 40% (OR 1.39; P = 0.001) as independent clinical factors associated with abnormal HRT parameters. CONCLUSIONS: HRT parameters are influenced by clinical characteristics and pharmacotherapy of studied patients with TS more than abnormal TO depending on clinical characteristics of patients. Advanced age, prior myocardial infarction and left ventricular dysfunction are key factors influencing values of HRT parameters.

[Sex steroids and heart rate variability in men with coronary artery disease after myocardial infarction].

The role of sex steroids in coronary artery disease (CAD) has been studied for years. In patients after myocardial infarction autonomic nervous system dysfunction has been described. Heart rate variability (HRV) analysis is one of the method of autonomic nervous system evaluation. The aim of the study was to evaluate correlations between sex steroids and HRV in 88 men (aged 36-73, av. 53 yrs), with the history of MI, with angiographically documented CAD. In all the patients 24-hour Holter monitoring was performed to assess HRV time domain parameters: SDNN, SDNNI, SDANN, rMSSD, pNN50. Levels of testosterone and estradiol were measured in two subsequent blood samples, taken with 30 min interval. Free testosterone index as well as estradiol/testosterone ratio were assessed. For statistics r-Spearman test was used. Positive correlations were found between testosterone and SDNN (r = 0.38, p = 0.03), testosterone and rMSSD (r = 0.51, p = 0.002) and between testosterone and pNN50 (r = 0.45, p = 0.007). Since rMSSD and pNN50 are parameters describing parasympathetic activity, our results suggest that in CAD men with the history of myocardial infarction, testosterone may influences the function of autonomic nervous system promoting parasympathetic dominance. It can be favorable for the circulatory system function.

Early cardiovascular involvement in patients with systemic sclerosis (SSc).

BACKGROUND: Systemic sclerosis (SSc) is a connective tissue disease characterized by progressive fibrosis of the skin, and by distinctive forms of internal organ involvement. Symptoms usually occur relatively late and are nonspecific, thus often unrecognized. The purpose of our study was to evaluate cardiac function in patients with SSc by means of non-invasive methods in order to detect early dysfunction of the cardiovascular system. MATERIAL/METHODS: A group of 22 patients with SSc was compared with a group of 22 healthy volunteers. Standard EKG, 24-hour Holter monitoring (HM), and echocardiography were performed in all subjects. RESULTS: In patients with SSc, HM revealed a tendency to tachycardia. Conduction disturbances were observed in 3 patients. In 6 patients significant ventricular arrhythmia was found. Silent ischemia episodes were detected in 6 patients. In HRV analysis, significantly lower values were detected in patients with SSc than in controls. LPs were present in one patient with SSc, and none in the control group. The mean values for QT interval did not exceed the range of normal values. No signs of systolic cardiac dysfunction were detected, while in 6 patients left ventricle diastolic dysfunction was recognized. Valvular lesions were observed in 8 patients, but only in 2 patients were they hemodynamically important. CONCLUSIONS: 24-hour Holter monitoring and echocardiography enable the early detection of cardiovascular dysfunction in patients with systemic sclerosis presenting without apparent cardiac impairment symptoms.
