[Practical application of proliferation markers' (MIB-1, PCNA, AgNOR) expression analysis for differential diagnostics of nodular thyroid lesions].

In the study authors tried to estimate the diagnostic value ofAgNOR number and MIB-1 and PCNA expression in thyreocytes from various pathological lesions, with special attention to differential diagnosis of border-line lesions (nodule in adenomatus goiter--adenoma--follicular carcinoma). Fifty four thyroid glands were examined (10 adenomatous goiters, 20 follicular adenomas, 10 follicular carcinomas, 14 papillary carcinomas). Statistical analysis of obtained results allows in most cases to differentiate carcinomas from benign nodular lesions. The best differentiating factor for border-line lesions (especially differentiation between benign nodular lesions, neoplasms, nodules in adenomatous goiter adenomas and adenomas carcinomas) has turned out to be MIB-1. PCNA expression has differed significant between adenomatous goiter and follicular carcinoma and between adenoma and both carcinomas. AgNOR number has only helped in differentiation between nodules in adenomatous goiter and follicular carcinoma - correlations for other lesions have not been statistically significant.

Immunohistochemical expression of progesterone and estrogen receptors in meningiomas.

In this study we present results of investigations of progesterone and estrogen receptors in most frequent, WHO grade I histological types of meningiomas (meningothelial, fibrous, and transitional) and in atypical--WHO grade II variant of these tumors. Samples from 64 tumors were examined. The cohort consisted of 46 WHO grade I (21 transitional, 13 fibrous and 12 meningothelial histologic variants) and of 18 atypical meningiomas. Apart from immunohistochemical examination of progesterone and estrogen receptors, MIB 1 labeling index was estimated. Positive immunoreaction for progesterone receptors was found in 100% meningothelial, 95% transitional, 46% fibrous and 78% atypical variant of meningiomas. Intensity of immunoreaction was stronger in grade I than in grade II tumors. Immunoexpression of estrogen receptors was found in 48% of the investigated meningiomas. No correlation was stated between WHO grade I and grade II tumors, and between meningothelial, transitional and fibrous variants of the neoplasms.

Suppression of contact hypersensitivity after repeated exposures of humans to low doses of solar simulated radiation.

Although it is generally recognised that UV radiation (UVR) can induce suppression of contact hypersensitivity (CHS) in human subjects, most protocols to date have not tested the effect of low daily doses of solar simulated radiation (SSR). In the present study, healthy individuals, divided into four groups each consisting of approximately 34 subjects, were whole-body irradiated with 1.2 standard erythema doses of SSR for 2, 10 or 30 consecutive days, or were unirradiated. They were sensitised with diphenylocyclopropenone (DPCP) on one exposed body site 24 h after the final UVR. The occurrence and severity of the primary allergic response were noted, and both parameters were shown to be significantly lowered in the group irradiated for 30 days compared with the unirradiated group. Elicitation of CHS was undertaken 3 weeks after the sensitisation, using a range of concentrations of DPCP on a UV-protected body site. The extent of the CHS at 48 h was assessed by the clinical score, by an erythema meter and by histological examination of a biopsy taken from the site challenged with one selected concentration of DPCP. Although erythema and pigmentation did not differ between the groups, a significant negative correlation was found between the clinical CHS score and the number of days of UV exposure, at the lowest challenge dose of DPCP. In addition a significant negative correlation was revealed between the intensity of spongiosis (intraepidermal oedema and vesicles, as evaluated by histology) and the number of days of UV exposure. Thus small daily doses of SSR induce suppression of CHS in human subjects and the effect is cumulative, indicating that there is no adaptation to the immunomodulating effects of UVR, at least over the test period of 30 days.

Expression of vascular endothelial growth factor (VEGF) in vulvar squamous cancer and VIN.

Angiogenesis plays an important role both in progression of solid tumors and in metastasizing. An invasive growth of a neoplasm is mainly connected with appearing of blood vessels within a tumor. Inhibition of angiogenesis in solid neoplasms may deter both tumor growth and metastases. New treatment strategies based on suppressing of angiogenesis and selective damaging of neoplastic blood vessels may prove to be as efficient as those based on direct destruction of neoplastic cells. One of important angiogenic factors is vascular endothelial growth factor (VEGF), which is produced by neoplastic cells and shows high promitotic activity almost entirely for endothelial cells (paracrine activity). We decided to investigate VEGF expression in precancerous lesions as well as in squamous cancers of vulva. Our material included 31 cases of vulvar squamous cancer, 28 cases of VIN (vulvar intraepithelial neoplasia) III, 10 VIN II cases and 12 VIN I cases. A diagnosis was established according to WHO criteria on the ground of post-operative histopathological examination complemented with proliferation index estimated by the use of MIB-1 antibody. Immunohistochemical examinations were performed on paraffin-embedded material, using MIB-1 antibody (Immunotech), VEGF antibody (Santa Cruz), Goat serum Normal (DAKO), DAKO StreptAB-Complex/HRP Duet, Mouse/Rabbit DAKO DAB Chromogen Tablets, TBS (Sigma). Positive cytoplasmic expression of anti-VEGF polyclonal antibody (diffuse and/or focal and of various intensity) was observed in almost all samples from precancerous and cancerous lesions. The expression was especially strong and diffuse in all cancer cases; in cases of VIN it was mainly focal and weak.

[Prognostic significance of depth and mode of invasion of the neoplasm in the presence of micrometastases in laryngeal carcinoma--cytokeratin antigens expression in lymph nodes].

According to the recent reports analyses of characteristic of neoplasm's invasion is one of the most reliable methods of estimation the progress of the changes of tumor and prediction the outcome in patients with laryngeal cancer. Depth and mode of invasion are features which are able to assess the dynamics of the tumor growth quite precisely. In this study it was presented direct relation between morphological features of tumor front as a depth and mode of carcinoma's growth and the probability of micrometastases. The authors have analysed 22 cases of patients who were operated on the laryngeal squamous cell carcinoma in ENT Department Medical University of Lodz in the period of 1998-1999. Features of the TFG and its dependence on lymph node micrometastases, considered in immunohistochemistry with using of panel of cytokeratins (CKs) were analysed. Our study showed that feature such as depth and mode of invasion is very useful in prediction of micrometastases in patients with laryngeal squamous cell carcinoma.

[Prognostic significance of clinical and morphological features of the lymph nodes in the presence of micrometastases--cytokeratin antigens expression in lymph nodes in laryngeal carcinoma].

In patients treated surgically due to neoplasm, in lymph nodes can be presented micrometastases, focuses 1-2 mm of carcinoma's cells. Lymph node micrometastases in patients with squamous cell carcinoma of the larynx found node-negative by conventional histology may be discovered by immunohistochemistry, particularly by using antibodies which are reactive with epithelial cells eg. cytokeratin antigens. Then tumors classified as pN0 by routine methods could be reclassified more correctly as pN1. Authors investigated the incidence of micrometastases in the neck dissection specimens originally staged as pN0 from 22 patients with laryngeal cancer treated surgically at ENT Department Medical University of Lodz between 1998-1999, according to the survival, using immunostaining with panel of polyclonal antibodies to cytokeratins. The relationship between micrometastases and clinical (pre- and intraoperative) features of lymph nodes were estimated.

Histological examination in assessment of ultraviolet-induced suppression of contact hypersensitivity response.

The evaluation of contact hypersensitivity (CHS) reaction is one of the methods used in the assessment of the immune status of an organism after UV radiation. The aim of the study was to compare usefulness of visual scoring system and histological morphometry in the assessment of CHS response after exposure of humans to solar simulated radiation (SSR). The study included 140 healthy volunteers, 33 people were irradiated for 2 days, 34 - for 10 days and 33 - for 30 days with SSR. Forty non-irradiated individuals served as controls. All the volunteers were sensitized with diphenylocyclopropenone (DPCP) 24 h after final exposure. Statistical analysis comparing intensity of CHS reaction based on visual score between irradiated groups and non-irradiated group revealed no differences (p>0.05). We found a significant difference in epidermal thickness between healthy skin and irradiated groups (p<0.05) and a positive correlation between intensity of spongiosis and clinical score for CHS response at 3.2 DPCP site (p<0.000001). A negative correlation between time of irradiation and spongiosis score was revealed (R=-0.28; p<0.001). We conclude that histological examination of biopsies taken from one of the series of elicitation sites is a reliable and sensitive method in the evaluation of CHS response after UVR.

[Prognostic significance of morphological features of the tumor front in the presence of micrometastases in laryngeal carcinoma--cytokeration antigens expression in lymph nodes].

Despite innumerous both therapeutic and histopathologic studies that have been performed no morphologic markers are currently available in order to predict the outcome in patients with laryngeal cancer. According to the recent reports tumor front grading (TFG) is one of the most reliable methods of estimation of the progress of the changes in the peripheral part of tumor. TFG seems to be one of the technics, which is able to assess the dynamics of the tumor growth quite precisely. In this study it was presented direct relation between morphological features of tumor front and the probability of micrometastases. The authors have analysed 22 cases of patients who were operated on the laryngeal squamous cell carcinoma in ENT Department Medical University of Lódź in the period of 1998-1999. Features of the TFG and its dependence on lymph node micrometastases, considered in immunohistochemistry with using of panel of cytokeratins (CKs) were analysed. Our study showed that feature such as TFG is very useful in prediction of micrometastases in patients with laryngeal squamous cell carcinoma. The statistical analysis showed significant correlation between TFG score and mode of invasion and depth of invasion. The presented study emphasizes that it is necessary to extend the traditional histopathological diagnosis by TFG, which might influence decisions regarding therapeutic management and could eventually lead to more appropriate and individualized therapy.

CD44 expression in human meningiomas: an immunohistochemical analysis.

CD44 molecules play an important role in cell adhesion, interaction between cells and extracellular matrix and cellular invasive growth potential. The aim of the study was to evaluate CD44 expression in atypical and benign meningiomas. The tumor specimens were obtained from 61 patients (27 males and 34 females; aged 19 - 78). The analysed material included 10 atypical meningiomas (G2), and following subtypes of benign meningiomas (G1): 21 transitional, 17 meningothelial and 13 fibroplastic. Paraffin tissue sections were immunostained using CD44 antibody (DAKO). The expression of CD44 was multifocal and diffuse. The strong expression was observed in atypical meningiomas, no expression in fibroplastic meningiomas and moderate expression in the other subtypes of benign meningiomas. Statistically significant differences were revealed between the expression of CD44 in G2 and G1 tumors. The results of the study support the thesis on the role of mutated CD44 molecule in the invasive growth potential of neoplastic cells.

The morphological analysis of vasculature and angiogenic potential in meningiomas: immunoexpression of CD31 and VEGF antibodies.

Meningiomas, despite being the mostly well-differentiated tumours, are quite often characterised by invasive growth and recurrences. As this fact may be caused by proangiogenic factors, we have investigated immunoexpression of CD31 and vascular endothelial growth factor (VEGF) in these tumours. The materials included 10 atypical meningiomas (G2), and the following subtypes of benign meningiomas (G1): 21 transitional, 17 meningothelial and 13 fibrous. Paraffin sections were immunostained with CD31 (DAKO) and VEGF (Santa Cruz) antibodies. The highest number of blood vessels was revealed in atypical meningiomas and much lower (statistically significant difference) in benign ones. Diffuse or focal expression of VEGF of various intensity was shown both in atypical and benign types of tumours with no statistically significant differences between both groups of tumours and between subtypes of benign meningiomas.

Estrogen and progesterone receptors in neoplastic and non-neoplastic thyroid lesions.

The pathogenesis of thyroid cancer is complex. Iodium deficiency, genetic factors, sex, older age, irradiation in childhood, thyroid growth stimulating antibodies and epithelial growth factor possibly affect its development. Recent reports, also epidemiological, have shown that differentiated thyroid cancer (papillary, follicular, oxyphillic) is dependent on sex hormones, especially estrogens. This has prompted research into the presence of estrogens and progesterone, as well as androgen receptors (ER, PR and AR) in normal and neoplastic thyreocytes and estradiol content in thyroid tissue. The results of these investigations imply that thyroid cancers are estrogen-dependent. There is, however, no agreement in reports about correlation between tumor malignancy and ER, PR and AR expression. In this study 72 thyroid glands have been examined using immunohistochemical assays with ERs and PRs antibodies. Positive staining was demonstrated mainly in differentiated thyroid cancer nuclei (17 ERs positive and 2 PRs positive). There was no expression of antibodies in non-neoplastic cells or in adjacent tissues. The nuclei of 3 oxyphillic and 4 follicular adenomas were also ERs positive. On the other hand nuclei of 5 follicular adenomas were PRs positive. These results are in agreement with most available data. It seems, however, that the material so far examined by the investigators is too small. Furthermore, because of using various methods the results cannot be compared. Further studies are necessary to reveal if there is any true influence of sex hormones on the development of thyroid lesions and if the detection of sex hormone receptors may help in choosing adequate therapy.

[Neoplasms and angiogenesis].

Angiogenesis is a process of new blood vessels formation in many physiological and pathological conditions such as wound healing, cancer, ischaemic diseases (of heart or limb) and chronic inflammations. Neoplastic angiogenesis plays an important role in progression of the disease and development of metastases. The angiogenic process may be initiated by hypoxia in cancerous cells, mutations in suppressor genes or oncogenes. Cancer cells activate specific angiogenic factors mainly vasoendothelial growth factor (VEGF) that stimulate migration and division of vascular endothelial cells. The studies of several pro- and antiangiogenic molecules and factors, and mechanism of vessel growth, have revealed that therapeutic interferences with these processes offers a tool for clinical applications in various pathologies. Inhibition of angiogenesis can prevent diseases with excessive vessel growth such as, particularly, cancers. Stimulation of angiogenesis can be beneficial in the treatment of diseases such as coronary disease and limb ischaemia. In this article some aspects are presented of current knowledge of angiogenesis and its regulation.

Expression of vascular endothelial growth factor (VEGF) in human thyroid tumors.

Vascular endothelial growth factor (VEGF) is one of the factors related to angiogenesis of tumors and physiological and pathological proliferative processes. Location of VEGF in human thyroid tissue with various thyroid disorders was studied in order to explore its possible involvement in proliferative processes. Immunohistochemical examination was performed on 69 formalin-fixed, paraffin-embedded thyroid tissue specimens using the labelled streptavidin biotin peroxidase complex detection system. VEGF was not identified in normal thyroid follicular cells. Some but not all tumor thyreocytes expressed VEGF in cytoplasm. VEGF positive expression was found in 6/15 patients with papillary carcinoma, 4/9 with follicular carcinoma, 14/28 with follicular adenoma and 2/17 with adenomatous goiter. In benign follicular adenoma and adenomatous goiter the weak expression of VEGF was found in small areas of the tumor tissues, whereas in malignant tumors it was found strongly in many cells. VEGF probably functions as a hypoxia-inducible angiogenic factor, and the expression of VEGF is stronger in malignant tumors, which need more oxygen supply to proliferate. An intensive VEGF production by differentiated thyroid carcinomas could be a promising marker of tumor aggressiveness and may also be useful as a predictor of metastatic potential and extension of tumor mass.
