Autotransplantation of premolars. A retrospective study.

The purpose of this clinical follow-up study was to evaluate the success rate of autotransplantation of human premolars. In the period between 1982 and 1994, 46 premolars in 31 patients were transplanted. A long-term success rate of 97.5% with a mean follow-up period of 3.5 years was achieved. It seems to be justified, therefore, in selected cases, to consider the possibility of autotransplantation as a good alternative to orthodontic or prosthetic treatment, including implant insertion, when closing a gap in the dental arch.

Oncologic aspects of vermilionectomy in microinvasive squamous cell carcinoma of the lower lip.

Twelve patients with microinvasive squamous cell carcinoma of the lower lip were studied in order to determine whether vermilionectomy was justifiable. The maximum tumor thickness (MTT) varied between 1.1 and 3.1 mm. In none of the patients was there local recurrence during a mean follow-up period of 4.4 years (range 4-8). In view of these results and of what has been reported in the literature, it seems warranted to conclude that vermilionectomy is an appropriate procedure in microinvasive carcinoma of the lower lip with a MTT of up to 3 mm.

T-cell lymphoblastic lymphoma of the lower jaw in a young child: a case report.

Among non-Hodgkin's lymphomas occurring in childhood two major histologic subgroups can be identified: (1) Burkitt's lymphoma and (2) T-cell lymphoblastic lymphoma, an uncommon high-grade malignant non-Hodgkin's lymphoma. Although Burkitt's lymphoma with maxillofacial involvement is a well-documented disease, T-cell lymphoblastic lymphoma in the perioral region is rare. An unusual case of T-cell lymphoblastic lymphoma with initial oral manifestation in an 18-month-old child is presented.

Computed tomography in evaluation of early secondary bone grafting.

Computed tomography (CT) was used to determine the fate of the bone graft in cleft lip and palate patients. Eight right-sided unilateral cleft lip and palate patients from the Free University Hospital in Amsterdam, treated with early, secondary bone grafting, were evaluated immediately after the operation and 1 year postoperatively. Approximately 70% of the volume of transplanted iliac crest bone was still present in the cleft area after 1 year.

Regional odontodysplasia: report of three cases.

Regional odontodysplasia is a developmental anomaly of dental tissues with characteristic clinical, radiographic, and histologic appearances. It most commonly affects the maxillary anterior teeth of both the primary and permanent dentition, and occurs in females twice as often as in males. The pathogenesis is unknown. The clinical and histopathologic findings of regional odontodysplasia in three patients are discussed.

Residual cementoblastoma of the mandible.

An unusual case of a residual cementoblastoma in a 24-yr-old man is described. The final diagnosis has been based on the history and the clinical, radiographic and histologic features. The lesion has been removed in toto. During the ten-year follow-up no signs of recurrence have been noticed.

[Pain and painkillers. 1. Neurophysiologic component].

A pain impulse is transmitted in the thin nerves from nerve cell to nerve cell, by means of neurotransmitters and -modulators, shed in the synapses. In the dorsal horn of the spinal medulla the impulse is either or not allowed to travel to the brain by a 'gate control' mechanism, and pain may be felt. The gate is influenced by cells in the nearby situated substantia gelatinosa, thick nerves and efferent nerves from higher situated centra.

[Pain and painkillers: 2. Biochemical components].

Besides the neurophysiological component of pain, subject of a previous article, biochemical components exist. Humorals, released in case of tissue damage and inflammation, cause among others vasodilatation, increased permeability of the blood vessels, itching and pain. Other endogenous biochemical products, among which endorphin beta, lessen the pain. It is also possible to further diminish the noxious (painful) action of the biochemical products with anti-biochemical medication.

A diversity of painkillers is available on the market. One may make an appropriate choice among them, based on the causative factors of the pain and recognizing both their therapeutic features and side-effects. This article tries to facilitate the choice, by describing the more important (groups of) painkillers.

[Infections along trans-mucosal fixation wires].

In a retrospective investigation the frequency and reasons for infections along internal fixation wires used in trauma patients was studied. A group of 628 trauma patients of the period 1985-1990 was reviewed. In 50 patients out of this group internal wires were used that perforate the oral mucosa. In 11 cases an infection was noted. The trauma group was than compared with 634 patients who underwent orthognathic surgery in the same period. In this group only 3 patients developed an infection along an internal fixation wire. After analysis of the date it seemed that the use of preventive antibiotics and a good oral hygiene prevents infections along intern wires.

[Haemangiomas. Review and case reports].

After reviewing the literature on haemangioma and vascular malformation, two cases of the latter condition are presented. A 40-year-old father shows a facial lesion and his 15-year-old son one of the tongue, the latter seemingly decreasing in size over the years. Because a vascular malformation, surgically removed, was present in the neck of the father's sister, a genetically (not Y-linked) determined coincidence is questioned. In studying its inheritance, the expression and penetrance of the anomaly has to be taken into consideration.

Sudden open bite resulting from hemarthrosis: report of a case.

In contrast to chronic inability to open the mouth, inadequate closure of the jaws seldom occurs. The present case describes a young women with hereditary hyperlipoproteinism on an anticoagulant who suddenly developed an unilateral open bite due to a hemarthrosis of the left TMJ. After aspiration of the accumulated blood in the temporomandibular joint, the open bite disappeared and the occlusion became normal.

Van der Woude syndrome--recognition of lesser expressions: case report.

A family with Van der Woude syndrome is described. The proband, whose mother and brother had facial clefting, showed inconspicuous abnormalities of the lower lip and a bifid uvula. Lesser expressions of Van der Woude syndrome are common and should be actively looked for in counseling families about cleft lip or cleft lip and palate.
