Positron emission tomography as a multi-institutional effort.

The consortium relationship at State University of New York at Buffalo School of Medicine and Biomedical Sciences have made it possible to establish in this community a multi-institutional effort to develop positron emission tomography (PET). The type of approach used and the modalities employed to generate community, university, and federal support are presented. The development of PET technology has led to a greater understanding between the various disciplines, and joint efforts have been made to establish pilot projects in a number of research protocols in the clinical applications that are currently available today. The consortium concept has led to early consideration by third-party carriers to consider reimbursement for those established PET procedures that can be applied clinically. Hopefully, with the passage of time and the maturation of a regional PET center, the concepts proposed might be used as models for other installations throughout the country.

Radionuclide imaging in osteomyelitis.

The utility of bone imaging has remained significantly high despite the proven suboptimal specificity of the conventional three-phase bone scan. The quantitative four-phase study may play a role in maintaining its usefulness in the future. However, due to its extremely high sensitivity and excellent anatomical information, it remains as the first diagnostic study in most suspected cases of acute osteomyelitis. The future would see development of new imaging modalities aimed at enhancing the specificity for diagnosis of active osteomyelitis. The indium leucocyte study has been the single most important development in this respect in the last decade. NMR has been another important addition to complement the anatomical information for defining the extent and nature of infectious process--acute or chronic. It remains to be seen if the other investigative work on antigranulocyte antibodies or other imaging agents or techniques would make the future diagnosis of osteomyelitis more reliable and accurate using the nuclear medicine techniques.

Early diagnosis of testicular tumor using Tc-99m pertechnetate scrotal imaging.

Two patients who originally presented with clinical symptoms of epididymo-orchitis, with compatible findings on the Tc-99m pertechnetate scrotal images, had temporary remission of their symptoms with antibiotic therapy. After two months, both patients had repeat scrotal images, because of persistent testicular enlargement. Both now demonstrated a "cold spot" in the otherwise increased activity on the affected side. Both patients underwent orchiectomy; patient 1 had a tissue diagnosis of malignant mixed germ cell tumor and patient 2 showed teratocarcinoma.

Visualization of pancreatic pseudocyst.

In a 44-year-old woman, a pancreatic pseudocyst was demonstrated on delayed images obtained up to 8 days after the intravenous administration of 0.250 mCi 75Se-selenomethionine. The initial routine pancreas image study failed to visualize both the pancreas and the pancreatic pseudocyst. The diagnosis was confirmed at surgery and the fluid of the pancreatic pseudocyst contained 0.73% of the injected dose of the radioselenium.
