A new system of intraocular instruments: III. Guillotine scissors.
The development of a new manual 20-gauge intraocular scissors provides several distinct advantages over prior models. They are compatible with standard vitrectomy systems and useful in applications in both the anterior and posterior segments of the eye particularly for resection of epiretinal and pupillary membranes.
Paraneoplastic retinopathy.
Paraneoplastic retinopathy appears to represent a nonmetastatic remote effect of carcinoma and is characterized by rapid visual deterioration accompanied by narrow arterioles seen on ophthalmoscopic examination and an extinguished electroretinogram. This syndrome has been described in postmenopausal women, and it is possible that it represents an autoimmune disorder. Corticosteroid therapy may arrest the visual deterioration.
Endogenous Actinobacillus actinomycetemcomitans endophthalmitis.
A 56-year-old immunocompromised man had an acute decrease in vision that led to the detection of Actinobacillus actinomycetemcomitans endocarditis complicated by septic emboli to his eye and other organs. This case demonstrates that bacterial sepsis can mimic a fungal infection. To our knowledge, this is the first report of endophthalmitis resulting from infection with A actinomycetemcomitans.
A new infusion/aspiration coaxial cannulated handpiece.
A new lightweight, low flow infusion/aspiration coaxial cannulated handpiece incorporates novel features which facilitate extracapsular techniques. It functions well when used with either manual or mechanized systems.
Sodium hyaluronate use during lens expression in extracapsular cataract surgery in rabbits.
Albino rabbits were used to compare the effects of filling the anterior chamber with balanced salt solution or sodium hyaluronate prior to lens expression in an extracapsular surgical technique. No significant differences were found between the two utilizing periodic slit lamp examinations, measurements of intraocular pressure, corneal thickness or corneal endothelial cell density. Sodium hyaluronate maintained a deeper anterior chamber and simplified lens expression.
New system of intraocular instruments. I. Guillotine intraocular forceps.
A four-instrument system of 20-gauge, rustproof intraocular forceps can be used in conjunction with pars plana vitrectomy to (1) extract intraocular foreign bodies, (2) manipulate intraocular tissue and a dislocated pseudophakos, and (3) obtain biopsy specimens of vitreous gel and abnormal preretinal and epiretinal membranes. The forceps can also be used as an intraocular needleholder for transvitreal suturing of selected giant retinal tear flaps. These instruments are lightweight and sturdy and use a guillotine action and a new barrel-handle design. To facilitate introduction and use through a pars plana approach, the various jaw faces are angled 135 degrees from the long axis. Prototype models have been used successfully in numerous and varied clinical applications.
Effect of binocular variations of Snellen's visual acuity on Titmus stereoacuity.
Thirty normal subjects were tested with the Titmus stereotest varying the binocular Snellen's visual acuity from 20/20 to 20/200. From the 36 possible combinations, a nomogram describing the relationship of binocular variations of Snellen's visual acuity and stereoacuity was constructed. The nomogram provides a useful reference for comparing expected and achieved clinical results of the Titmus stereotest in normal subjects. The effects of various pathologic conditions of the eye on the relationship between Snellen's visual acuity and stereoacuity needs to be studied.
A new system of intraocular instruments. II. Intraocular needleholder.
The authors designed an intraocular needleholder to grasp the needle of 8-0 or 9-0 sutures during transvitreal procedures to suture the edge of certain giant retinal tears to the eyewall. The instrument uses a guillotine action and has a barrel handle design. There is 135 degrees of angulation between the jaws and the long axis of the instrument. With this device the needle is firmly grasped, and a locking mechanism permits release of finger pressure during intraocular manipulation.

Psychoesthetic environmental design for pediatric care facilities.
Since the emotional reactions of children have a direct effect on their health exclusive of their disease, health care facilities are obligated to serve the psychological and emotional needs of pediatric patients as well as their physiologic needs. Following guidelines of certain environmental and psychological studies, intelligent and creative decoration can be executed by use of elemental design principles to achieve the atmosphere in a facility capable of satisfying the demands of the patient, the family, and the working personnel.
