INTRODUCTION: Although CT and MR are sensitive techniques for the detection of cerebral tumours, both have limitations in distinguishing between tumour relapse (TR) and post-treatment radionecrosis (RN). PATIENTS AND METHODS: In this study we have determined the usefulness of metabolic imaging with PET-FDG in such situations. We assessed 70 patients with CNS tumours (22 low grade astrocytomas, 25 high grade astrocytomas, 3 oligodendrogliomas, 13 metastatic tumours and 7 other tumours. All had been treated with radiotherapy and other treatments such as radiosurgery, chemotherapy or different types of surgery, and presented clinical pictures which made it necessary to decide the differential diagnosis of relapse or radionecrosis. RESULTS: In the PET-FDG study visual and semiquantitative analysis was done by SUV (Standardized Update Value). Confirmation of the findings was obtained in 44 cases (24 TR and 20 RN). MR was doubtful or inconclusive in most cases, whilst with PET correct diagnosis was made in all cases. CONCLUSIONS: Metabolic imaging with PET-FGD is better than anatomostructural imaging techniques for differential diagnosis between tumour relapse and radionecrosis in CNS tumours which have been treated. Prospective studies are necessary for evaluation of SUV as a factor for prognosis of survival.
Fungal contamination in a greenhouse for production of cucumbers (Cucumis sativus, L.) was studied. 158 samples of water, sand, air, leaves and fruits were analyzed. 25 different genera were isolated, the most frequent ones being Pullularia, Aspergillus, Paecilomyces, Alternaria, Fusarium, Rhizopus and Penicillium. Pullularia, Aspergillus, Paecilomyces and Penicillium were present in all types of samples studied. Twenty-one strains of A. flavus were identified and only eight of those produced aflatoxins "in vitro".

OBJECTIVES: To assess the utility of single photon emission computed tomography (SPECT) with Thallium-201 after anterior myocardial infarction at predischarge time in the detection of; a) the prevalence of isotopic redistribution in the infarct area or at a distance; b) the correlation between peri-infarction ischemia and the angiographic state of the infarct-related artery, and c) the correlation between ischemia at a distance and the presence of multivessel disease. MATERIAL AND METHOD: Seventy-three survivors of an uncomplicated anterior myocardial infarction, 67 men and 7 women, with a mean age of 56 +/- 9 years (34-70 range), underwent T1-SPECT after stress test (62 exercise test and 11 pharmacological Dipyridamole test) as well as coronarographic studies before discharge. Peri-infarction ischemia was defined as redistribution presence in the distribution territory of the left anterior descending artery (LAD) and was assessed in a semi-quantitative way scoring both stress and rest images that allowed the calculation of a redistribution index R (Stress Score-Rest Score/Stress Score). Ischemia at a distance was defined as redistribution presence in the territories of circumflex and right coronary arteries as assessed both by visual analysis (VA) and bull's eye polar maps (BE) or by washout imaging (WO). RESULTS: Peri-infarction ischemia (R > 0) was found in 48 (65.7%) patients; 43 with and 5 without significant residual lesions in the LAD and the absence of peri-infarction ischemia (R = 0) was found in 25 (34.3%) patients, 19 with and 6 without significant residual lesions in the LAD. Sensitivity and specificity for multivessel disease detection by ischemia at a distance was 64% and 85% respectively, for VA; 60% and 77% for BE; and 95% and 65% for WO. Combinations of different analytical methods (multiparametric approach) showed a decrease in sensitivity but improved specificity and positive predictive value: 60%, 90% and 75%, respectively, for BE & WO; 44%, 90% and 69% for VA & BE and 60%, 92% and 79% for VA & WO. CONCLUSIONS: TI-SPECT imaging is a useful diagnostic method to detect both peri-infarction ischemia and ischemia at a distance at predischarge time following anterior myocardial infarction. Redistribution presence in the non infarct-area shows a fair sensitivity and a high positive predictive value to detect residual multivessel disease.
OBJECTIVES: To follow the left ventricular systolic function changes as assessed by global and regional ejection fraction during the first year of evolution after anterior myocardial infarction, as well as to analyse how the most relevant clinical and angio-graphic parameters influence them. MATERIALS AND METHODS: Sixty-six consecutive patients with a first infarction of anterior location, completed 1 year of follow-up with radionuclide ventriculography, T1-SPECT after exercise or dypiridamole and cardiac catheterization before discharge; radionuclide ventriculography was repeated 6 and 12 months later. Twenty-five patients underwent revascularization procedures at the time of predischarge, 16 using successful percutaneous angioplasty of the left anterior descending artery and 9 using aorto-coronary graft surgery. Dilated patients were controlled, from an angiographic point, of view 6 months after PTCA. RESULTS: Predischarge global EF (42.9 +/- 12.6) increased significantly between the 6-month (46.9 +/- 12.8; p < 0.001) and 12-month (47.6 +/- 12; p < 0.001) studies. Differences in these two last studies were minor and had no statistical significance. A significant increase was present in revascularized or medically treated patients; i.e. those who underwent thrombolytic therapy or conventional treatment at admittance as well as mono or multivessel disease patients. No significant differences were detected in the mean ejection fraction in patients without residual stenosis in the infarct-related artery (48.1 +/- 13.3 vs 50 +/- 12.6; NS) or those presenting a pre-discharge ejection fraction > or = 45% (53.5 +/- 7.5 vs 55.1 +/- 3.3; NS). Among different variables tested, predischarge global ejection fraction (negative coefficient) and, to a lesser extent, percutaneous angioplasty were independent predictors of a significant increase of ejection fraction during follow-up. Changes from predischarge to 12 month study were significant in the regional ejection fraction in the anteroseptal (33.1 +/- 14.7 vs 40.1 +/- 13.3; p < 0.001), inferoseptal (34.6 +/- 15.8 vs 42 +/- 16.9; p < 0.001), apical (46.3 +/- 19.1 vs 50.8 +/- 19.7; p < 0.01), antero-medial (50.6 +/- 22.4 vs 56.6 +/- 24.3; p < 0.01) and anteroapical (51.6 +/- 23.5 vs 58 +/- 27.5; p < 0.01) segments. In the anterobasal segment, regional ejection fraction only showed a statistical tendency to increase during follow-up and changes in inferior and lateral segments, distal to the infarcted area, were minor and without statistical significance. The most distinct infarct-related segments, antero- and inferoseptal, showed significant increases in the main group of patients, revascularized or nonrevascularized, undergoing thrombolitic or conventional therapy, with single or multivessel disease. Only patients without residual stenosis of the infarct-related artery (the left descending anterior) disclosed negligible or negative changes without statistical significance. Percutaneously dilated patients showed the highest increase of the infarct-related segments regional ejection fraction. In multivessel disease, the infarct-related segments regional ejection fraction did not increase significantly in surgically treated patients, conversely to those who underwent medical therapy. CONCLUSIONS: Global and regional ejection fraction of the infarcted area increased significantly during the first year after anterior myocardial infarction, mainly prior to six months patients with significant residual stenosis of the artery related to the infarct. Recovery is present both in patients who underwent thrombolysis or conventional treatment at admittance and revascularization or medical treatment before discharge and is inversely correlated to the predischarge global ejection fraction value. This evolution suggests that a significant amount of stunned myocardium is still present before discharge.
We describe the delayed occurrence of a frontal sinus mucocele 14 years after the original trauma. The patient presented with a laterally displaced, enlarging mass that encroached on the dura. The sterile mucocele was removed, and the cranial defect was reconstructed with methyl methacrylate and wire mesh. Our experience confirms the known but rarely observed late development of a mucocele after pediatric facial trauma. To prevent the sequela of mucocele development, the mucosa of the rudimentary frontal sinus in the pediatric patient must be carefully sought and ablated during reconstructive procedures of the forehead when traumatic injury significantly disrupts the normal bony anatomy.
The kinetic parameters of almond beta-glucosidase (beta-D-glucoside glucohydrolase; EC 3.2.1.21), using pNGP as substrate were kM = 2.24 +/- 0.11 mM and Vmax 588 +/- 25.1 U/mg protein. Only Hg(II) and Cu(II) showed irreversible inactivation of the enzyme. However, when these metals were present in the reaction system the inhibition effects were consistent with a mixed-type inhibition pattern (Cu(II) ki: 5.08 mM and Hg(II) ki: 0.07 mM). The glucose kinetic effect was also consistent with a mixed-type inhibition (ki = 406 mM) pattern with pNGP as varied substrate. Ethanol displayed the kinetic pattern of competitive inhibition (ki = 640 mM).
A 49-year-old woman developed acute renal failure secondary to interstitial nephritis. Her clinical history, complementary studies and two renal biopsies could not establish the etiology. She showed signs of incomplete Fanconi syndrome. Treatment with corticosteroids was very effective, though she tended to relapse. We comment briefly on some aspects of idiopathic acute interstitial nephritis.
Chronic intestinal pseudo-obstruction is the term applied to a heterogeneous group of functional motility disorders sharing a common clinical expression: signs and symptoms of bowel obstruction in absence of mechanical occlusion. It is caused by ineffective intestinal propulsion. The chronic form of intestinal pseudo-obstruction may be primary or secondary. Primary pseudo-obstruction or chronic idiopathic pseudo-obstruction (CIIP) defines a group of propulsive disorders having no recognized underlying diseases. This study presents four female patients, aged between 4 months to 7 years, and makes a review of the literature. The symptoms, very similar in three of them, were bilious vomiting, abdominal distention and constipation, alternating with diarrhea and malnutrition. The fourth patient, different from the others in the age of onset and evolution, only had severe constipation and abdominal bloating. The diagnostic was made by full thickness biopsies during laparotomy, getting specimens by mapping, at different heights of intestine and stomach. Samples were studied by optic and electronic microscopy and visceral myopathies were found. None of them had urinary disorders. Medical treatment consisted of total parental nutrition and/or enteral nutrition. Cisapride was not effective in the two patients who received it.
The aim of this work was to establish the diagnostic and follow up value of IgA-class antiendomysium (IgA-EmA) and IgA-class antigliadin (IgA-AGA) antibodies in celiac disease. Correlation with the intestinal histology at the different stages of the disease was evaluated, as well as its therapeutic monitoring ability. Fifty six children, twenty seven girls and twenty nine boys, aged six months to twelve years old, were studied. Thirty nine celiac children were al different diagnostic stages of the disease. Seventeen children with malabsorption symptoms and with normal intestinal histology were used as controls. Sixty blood samples were obtained simultaneously with the small intestinal biopsy. IgA-AGA (ELISA method) and IgA-EmA (immunofluorescent test performed over lower third Rhesus monkey esophagus) were determined in every blood sample. In 34 serum samples from patients with total or subtotal villous atrophy, two were negative for IgA-AGA and only one was negative for IgA-EmA. In 26 samples from patients with normal intestinal histology, two were positive for IgA-AGA and four were positive for IgA-EmA. The results for IgA-EmA had sensitivity 97%, specificity 84.6%, positive predictive value 89.2% and negative predictive value 95%. In the case of IgA-AGA were: sensitivity 94.1%, specificity 92.3%, positive predictive value 94.1%, negative predictive value 92.3%. IgA-AGA and IgA-EmA showed a high correlation with intestinal histology and are in combination powerful tools for the diagnosis and follow up of celiac patients. Besides, they provide a useful aid in the indication of a jejunal biopsy and in close monitoring of the dietary treatment compliance.(ABSTRACT TRUNCATED AT 250 WORDS)
OBJECTIVE: To investigate the repercussion of experimentally induced endometriosis on the fertility of Wistar rats and the effects of pregnancy and lactation on endometrial ectopic implants. DESIGN: Eight-five animals were used for the experiment. Experimental endometriosis was induced in 44 and the remaining 41 were sham-operated (control group). After evaluating the growth of the implants in a second laparotomy, all the animals were mated until finding sperm in vaginal smears. In third and fourth laparotomies during pregnancy and lactation, pregnancy rate was determined, and the implants were re-evaluated, being resected for histological study. Serum estradiol (E2) and progesterone (P) levels were also measured. RESULTS: The pregnancy rate in animals with endometriosis was 65.7% versus 100% in control group. No statistical differences in days of gestation and number of pups were found between the two groups. Ectopic implants underwent a significant regression during lactation period. Serum E2 and P levels and histological features sustained these findings. CONCLUSION: Experimentally induced endometriosis diminishes pregnancy rate in rats. It is not pregnancy, but the anestrus condition created by lactation that exerts a beneficial effect on experimentally induced endometriosis in rats.

Four cases of renal agenesis with an ipsilateral blind vagina and two cases with agenesis of all organs derived from one urogenital ridge are presented. These cases confirm the association of renal agenesis with these genital malformations and support the hypothesis that the embryologic development of the vagina is derived from the fused mesonephric ducts.
This is the first case report of a documented pregnancy initiated with a new assisted reproductive technique based on direct insemination of a preovulatory follicle with ultrasonographic guidance using a transvaginal probe in women with at least one patent fallopian tube and controlled ovarian stimulation.
A new technique for vaginal intratubal insemination (VITI) and transvaginal gamete intra-Fallopian transfer (TV-GIFT) using endosonography has been developed. A total of 47 infertile couples were treated using a K-JITS 1000-1100 catheter (William Cook, Australia; Jansen Anderson Intratubal Transfer Set) introduced by Jansen and Anderson in Sydney, Australia, and endosonographic control with a Kretz Combison 310 echograph and a 5/7.5 MHz vaginal sectorial transducer. In 40 couples, the VITI technique was used, and in the other seven couples, TV-GIFT was used. Fifteen pregnancies (37.5%) were obtained in 63 VITI cycles (23.8% per treatment cycle) and three with TV-GIFT (42% per treatment cycle). A review is also presented of the migration--sedimentation technique used for management of male factor in an artificial insemination programme.
Activation of different forms of adenylate cyclases (AC) by forskolin and displacement of [14,15-3H]dihydroforskolin binding from membranes by forskolin in the absence or presence of specific stimulatory hormone and beta, gamma-imidoguanosine 5'-triphosphate (Gpp(NH)p) have been studied. These conditions have been used to generate forskolin dose-response curves of AC activation. A plot of enzyme activation versus apparent forskolin-binding showed a linear and a nonlinear relationship, respectively, in the absence or presence of the other two stimulators. The latter relationship can be fitted by two linear regression lines with a defined intercept, the slopes of which represent two distinct binding-activation (B-A) effects. The B-A effects of forskolin for rat adipocyte and liver membranes in the absence of stimulatory hormone and Gpp(NH)p were 10 and 8 (pmol X min-1) X (pmol)-1, respectively. The B-A effects for the same membranes in the presence of the other two stimulators were 69 (high) and 13 (low) (pmol X min-1) X (pmol)-1 for adipocyte membrane, and 83 (high) and 9 (low) (pmol X min-1) X (pmol)-1 for liver membrane. The ratio of potentiation of forskolin-activated enzyme activity to the unmodified forskolin-stimulated activity (P-A ratio) was determined without the binding data. At 3 microM forskolin, with and without 230 epinephrine and 10 microM Gpp(NH)p, the P-A ratio was 3.7, decreasing to 1.1 with the addition 100 microM forskolin. The line representing a high B-A effect and a resulting high P-A ratio appears to describe the interactions between forskolin and the AC stimulated by epinephrine and Gpp(NH)p. The line of low B-A effect may represent the interaction between forskolin and the basal AC. Two peaks of AC activity were eluted from forskolin-Sepharose column. They have apparent differences in sensitivity to Gpp(NH)p and affinity for forskolin. Based on the results available thus far, with consideration for known limitations of the methodology, a working model has been proposed for forskolin activation of AC.
Further modification of Neville's method of preparation of rat liver plasma membrane has been made in order to study glucagon-sensitive adenylate cyclase. This modified method introduces two additional steps to the Neville procedures. One involves saving an intermediate layer above the pellet following 1500g centrifugation. The other adds a centrifugation step at 20,000g. The improved method increases the yield of membrane protein by 5-fold and increases forskolin, glucagon, and 5'-guanyl imidodiphosphate stimulated activity of adenylate cyclase by 2.8-fold. A 13-fold increase in the yield of total adenylate cyclase activity above the current method was obtained. The membrane adenylate cyclase and its hormone sensitivity was stable in liquid nitrogen for at least 8 months. This modified method appears to be useful in preparing a better yield and quality of rat liver plasma membrane from given starting hepatic tissue for studies of glucagon-sensitive adenylate cyclase.
