Dysthyroid optic neuropathy. The crowded orbital apex syndrome.

The authors have reviewed the clinical presentation, visual fields, color vision testing, visual-evoked potentials, and computed tomographic (CT) findings of 58 patients (95 eyes) with dysthyroid optic neuropathy. The authors compared these findings to a control group of 60 patients (119 eyes) with thyroid eye disease who underwent CT scanning and did not exhibit evidence of optic neuropathy. Clinically, dysthyroid optic neuropathy is an insidious disease; when compared with the usual Graves' orbitopathy patient, the optic neuropathy group presented at a later age and with a later onset of thyroid eye disease. The patients in this group were more likely to be male and/or diabetic, and often presented with desaturation of color vision. Asymmetrical extraocular muscle restriction and vertical tropias were more frequent in the optic neuropathy group. The most sensitive indicators of optic nerve dysfunction appeared to be visual-evoked potentials and color vision. Computed tomographic studies confirmed that apical orbital crowding was a characteristic feature of optic neuropathy. These findings should alert the clinician to a more aggressive approach to these patients.

Urea peroxide solution in the treatment of gingivitis in orthodontics.

A 3-month study was conducted to determine the effect of a 10 per cent urea peroxide solution on gingivitis and oral debris accumulation in orthodontic patients. Sixty fullybanded children (thirty-two girls, twenty-eight boys) 10 to 17 years of age were randomly placed into two groups of thirty members. The treatment group was assigned the use of the test solution and given normal oral hygiene instructions. The control group was given only the normal oral hygiene instructions. The subjects' gingival status was evaluated by means of the gingival index (GI) of O'Leary. The accumulation of oral debris was monitored by means of the oral debris index (ODI) of Greene and Vermillion as modified by Womack. Subjects were scored at the start of the study and approximately 1 and 3 months thereafter. Analysis of variance of the GI and ODI scores revealed significant treatment effects (p = 0.0013 and p = 0.0051, respectively), which substantiated the clinical observations. Rinsing with the test solution in conjunction with toothbrushing appears useful as an oral hygiene measure for orthodontic patients. No adverse reactions were experienced by any of the patients using the test solutions.

Phosphoric acid penetration during direct bonding.

This work has investigated the actual penetration of acid etching material into the enamel surface as it is used in preparation for the direct-bonding adhesive. Three important conclusions can be drawn from this investigation. (1) Rinsing does not necessarily completely arrest the acid etching process in the depth of the exposed enamel. (2) The effect of the acid etching material does seem to be deeper than 10 mu, which is contrary to the manufacturers' statements. There is (3) no evidence of actual penetration of the acid etching material at the dentinoenamel junction.
