Ureteroscopic manipulation of stones in the ureter: four years experience.

Our experience of 223 ureteroscopies and stone manipulations is reported. Stone removal was successful in 82% of the cases whereof the stones were pushed back into the renal pelvis and subsequently managed with percutaneous nephrolithotomy or extracorporeal shock wave lithotripsy in 24%. For distal stones the success rate was 88%. Reasons for failure were acute obstruction of the ureter and previous surgery or irradiation to the pelvis. In six cases (4%) strictures requiring treatment developed secondary to ureteroscopy. Ureteroscopy can be a tedious and difficult procedure with a considerable learning time. In experienced hands the rate of serious complications is low and the procedure is an alternative in the treatment of ureteric stones, especially in the distal part of the ureter.

Norfloxacin versus trimethoprim-sulfamethoxazole. A study in patients with known bacteriuria undergoing transurethral resection of the prostate.

Three hundred and forty-three patients with suspected bacteriuria undergoing transurethral resection of the prostate (TUR-P) were randomized to treatment with either trimethoprim-sulfamethoxazole (TMP-SMX) or norfloxacin (NF) for 5 1/2 days beginning the evening prior to operation. It was possible to analyse 165 patients for efficacy. Elimination of bacteria on days 10 to 20 was achieved in 78.1% and 78.3% in the TMP-SMX and NF group, respectively. The accumulated elimination rates for the follow up period (days 10-42) were 68.5% for the TMP-SMX group and 76.2% for the NF group. The differences were not statistically significant. No patient had any clinical signs of upper urinary tract infection or septicemia. Three hundred and twelve patients were analysed for safety. Twenty-six patients reported 32 adverse drug events (ADEs). Four reactions in the TMP-SMX group were considered severe while in the NF group all the ADEs were of mild or moderate intensity. In this study NF seems to be at least as effective and safe as TMP-SMX.

The natural course of prostatic carcinoma in relation to initial cytological grade.

We investigated retrospectively 91 patients with prostatic carcinoma diagnosed cytologically between 1978 and 1979. Of the patients 57 had no metastases (M0) at presentation. The majority of the patients without metastases had well or moderately well differentiated carcinoma. Of 18 patients with poorly differentiated carcinoma 17 had metastases at presentation. The patients without metastases were left untreated, while those with metastases received active antitumor treatment. Local progression and/or development of metastases during surveillance occurred in 24 patients (42 per cent) and antitumor treatment was initiated. Mean observation time in the group untreated throughout observation was 47 months. Mean interval to progression in the patients treated subsequently was 31 months. In the surveillance group no difference in mean interval to progression, frequency of local progression, development of metastases or death rate of prostatic carcinoma was found when the patients with initially well and moderately well differentiated carcinoma were compared. Therefore, in our study initial cytological grade failed to predict a difference in progression in patients with well and moderately well differentiated prostatic carcinoma. Since almost all patients with poorly differentiated carcinoma had metastases at presentation a poor differentiation seems to predict a worse prognosis compared to well and moderately well differentiated tumors.

The time dependence of training-induced increase in skeletal muscle capillarization and the spatial capillary to fibre relationship in normal and neovascularized skeletal muscle of rats.

One hundred and eighty-four rats of different ages were investigated regarding the basic capillary-fibre array and the array resulting from capillary proliferation induced by swimming training and dipyridamole treatment. The time dependence of the training-induced capillary increase in the muscle was also investigated. Sections of the medial portion of the quadriceps femoris muscle were evaluated in light microscope for total capillarization, fibre area and fibre type composition. After 4 and 8 weeks of swimming training, increases in capillary density, capillary to fibre ratio and mean number of capillaries in contact with each fibre were recorded. No changes in muscle fibre area or fibre composition were seen. In all groups the majority of the capillaries was in contact with three muscle fibres. The capillary proliferation induced by training and dipyridamole treatment was confined to the capillaries in contact with two and three fibres. The results indicate a basic hexagonal capillary-fibre array with a possible position for a capillary in each corner of the hexagon. This pattern is retained in the increased vascularity induced by swimming training and dipyridamole treatment.

Time dependence of dipyridamole-induced increase in skeletal muscle capillarization.

40 young Sprague-Dawley rats in groups of 10 were subjected to treatment with dipyridamole for 1, 2, 4 and 8 weeks. Another 40 sham-treated rats served as controls. The medial portion of the quadriceps femoris muscle was evaluated for total capillarization, fiber area and fiber type composition. Capillary/fiber ratio and mean number of capillaries in contact with each fiber were significantly increased in the rats treated with dipyridamole for 2 weeks and longer. The capillary density, however, did not increase until after 4 weeks of dipyridamole treatment. The fiber area increased gradually with age and was significantly enlarged by dipyridamole treatment after 2 and 4 weeks. All fibers in the examined muscles were of the low oxidative (white) type and no change was recorded in this respect after dipyridamole treatment.

The prophylactic effect of dipyridamole on the size of myocardial infarction following coronary artery occlusion.

Experimental myocardial infarction was induced in adult female Sprague-Dawley rats by ligation of the left coronary artery. During four weeks preceding the operation the animals were given dipyridamole (3 mg/kg initial body weight) i.p. twice a day 5 days per week. Control animals were given saline. Ten days after the operation the rats were killed and the hearts serially sectioned and stained. The volumes of necrotic and non-necrotic myocardial tissue were determined by morphometry on the histological sections. In the dipyridamole-treated animals the volume of non-necrotic myocardium was larger and the necrotic tissue volume smaller than in the control animals.

Capillary increase in the skeletal muscle of trained young and adult rats.

1. Capillaries in skeletal muscle were studied before and after physical training by prolonged swimming, in young and adult rats. 2. The mean area of striated muscle fibres was unchanged in both young and adult rats after training. 3. Training increased the capillary vasculature of skeletal muscle in both young and adult rats, but proportionally less in the adults.

Causes of death in leukemia and lymphoma with modern treatment.

An attempt was made to see if new forms of treatment of fungal and bacterial septicemia, or new cytostatic combinations have changed the causes of death in leukemia and lymphoma. Autopsies were studied of 16 cases with acute leukemia, 2 with chronic granulocytic leukemia and 24 with lymphoma. 10 of the 18 patients with leukemia and 10/14 lymphoma patients died although they had no, or only slight, tumor infiltration in the bone marrow and parenchymatous organs; only 1/18 leukemias had extensive infiltration. There was a statistically significant correlation between the extent of marrow and organ infiltration. Cardiac failure (5/18 patients) was almost as common as the main cause of death as septicemia (7/18) in leukemia. The corresponding figures in lymphoma were 5/24 and 9/24, respectively. The present findings contrast with earlier ones, where more septicemia and tumor infiltration were found at autopsy, and less cardiac failure was observed. 15 of 16 cases with septicemia at autopsy had terminal fever. Bacteriological and histological signs of septicemia at autopsy agreed satisfactorily. There was a surprising absence of agreement between terminal granulocytopenia and septicemia; 13 of 16 patients with septicemia had over 0.1 x 10(9) and 10 over 0.5 x 10(9) granulocytes/liter blood.

Capillary neoformation in skeletal muscle of dipyridamole-treated rats.

Rats were given 0.3 mg dipyridamole i.p. twice a day, 5 days/week for 3 weeks. Morphometric determinations of the hindlimb skeletal muscle of these animals showed that they had a higher degree of capillary vascular supply than the muscles of sham-injected control rats. The number of capillaries per muscle fiber was increased by 24%, the number of capillaries/mm2 by 22% and the number of capillaries in contact with each muscle fiber by 22%.

Cardiomyopathy in leukemia, with reference to rubidomycin cardiotoxicity.

Sixteen patients who had died with leukemia were studied at autopsy between September 1975 and Februrary 1977. Special attention was given to degenerative changes in the heart. Five of the patients died of cardiac failure, all with no or only slight leukemic infiltration in various organs at autopsy. Five patients showed basophilic necroses in the myocardium, and two of these also showed necroses in the bone marrow. The myocardial lipofuscin was significantly (P less than 0.01) higher in the autopsies of leukemic patients (mean age 45 years) than in autopsies performed at the Department of Forensic Medicine in 18 cases of accidental death (mean age 36 years). No dose-response relationship could be found between the amount of myocardial lipofuscin and the total dose of rubidomycin. Eight of twelve patients with malignant lymphoma (mean age 45 years) also had increased amount of myocardial lipofuscin.

The proliferative activity of myocardial capillary wall cells in variously aged swimming-exercised rats.

Cellular proliferation was studied by incorporation of 3H-thymidine in the hearts of young, adult and old swimming-exercised rats. The proliferative activity was measured by scintillation counting. The swimming exercise induced a significant proliferation of capillary wall cells in the younger age groups, suggesting a neoformation of myocardial capillaries in these rats, whereas no cell proliferation was recorded in the old rats.

Proliferative activity of myocardial capillary wall cells in dipyridamole--treated rats.

The hearts of rats treated with dipyridamole were studied by light microscope autoradiography and measurement of tissue radioactivity after intravenous injection of 3H-thymidine. Total tissue radioactivity was significantly increased in the hearts of the dipyridamole-treated animals. Only capillary wall cells were labelled in the autoradiograms. The observations indicate a proliferation of capillary wall cells, suggesting a neoformation of myocardial capillaries during dipyridamole treatment.
